Rudakubana was already known for his violent tendencies and fixation on mass killings, as per a report in The Guardian.
One of these referrals came after he raised eyebrows with his interest in the killing of children in a school massacre, it reported.
"Yet between them, those agencies failed to identify the terrible risk and danger to others that he posed," said Cooper.
Authorities had been warned about his obsession with genocides, including the Holocaust and the Rwandan ethnic massacres. His father is believed to have fought with the Rwandan Patriotic Army (RPA).

Trump himself had made a decision in his final days in office in 2021 to put Cuba back on the list, reversing a reconciliation drive under former president Barack Obama.
The White House did not describe reasons for reversing Biden's order or whether it expected the move to impact freed prisoners.
But new Secretary of State Marco Rubio, a Cuban-American staunchly opposed to the communist island's government, told his confirmation hearing last week that he had "zero doubt" that Cuba belonged on the list.
The last Trump administration in 2021 had also cited Cuba's refusal to extradite back to Colombia members of the militant group ELN.
Colombia has since elected a leftist president, Gustavo Petro, who dropped the arrest warrants, although conflict with the ELN has erupted again in recent days, with at least 100 people dead.

"We know as well that the Cuban regime, for example, hosts not one but two countries' espionage stations within their national territory, 90 miles from the shores of the United States," Rubio said.

Trump reinstates Cuba on state sponsor of terror list: White House

South Korea's impeached President Yoon Suk Yeol, on Tuesday (Jan 21), denied giving instructions to the military to forcibly remove lawmakers from parliament, aiming to prevent them from voting against his declaration of martial law.&nbsp;
On being asked by a judge at the country's Constitutional Court if he had instructed top commanders "to drag out the lawmakers gathered in the National Assembly to lift the martial law", Yoon said "no", news agency AFP reported citing pool report of the court proceedings.
Yoon made his first appearance at the court on Tuesday, pledging to cooperate with the judges who will determine his fate if he will be removed from the office.

The walls of the office consist of portraits of George Washington, Thomas Jefferson, Alexander Hamilton and Andrew Jackson. Trump also kept the portrait of Benjamin Franklin, which was brought in by Biden, but he removed one of Franklin D Roosevelt. He has also reinstalled the flags for each brand of the military.

The move comes after Trump's repeated criticism of the WHO's handling of the COVID-19 pandemic.
Speaking at the White House hours after his inauguration, Trump said the US was paying far more to the UN health body, even more than China.
"World Health ripped us off, everybody rips off the United States. It's not going to happen anymore," the US president said.
In his new executive order, Trump directed agencies to "pause the future transfer of any United States Government funds, support, or resources to the WHO" and to "identify credible and transparent United States and international partners to assume necessary activities previously undertaken by the WHO."
His administration also announced plans to review and rescind Biden's 2024 US Global Health Security Strategy, which was aimed at preventing, detecting and responding to the contagions, and infectious disease threats "as soon as practicable."
The move is significant, as the US is the WHO's largest financial contributor, providing around 18 per cent of its total funding.
During several instances, Trump had accused the WHO of mishandling the Covid pandemic, failing to adopt necessary reforms, and being influenced by politics.&nbsp;

The United States released an Afghan prisoner in exchange for US detainees.
"An Afghan fighter Khan Mohammad imprisoned in America has been released in exchange for American citizens and returned to the country," the Afghan foreign ministry said in a statement.&nbsp;

US President Donald Trump on Monday (Jan 20) reiterated that he was considering imposing 25 per cent tariffs on Canada as well as Mexico with the possibility of implementation as early as February 1.

"We're thinking in terms of 25 per cent on Mexico and Canada because they're allowing vast numbers of people -- Canada's a very bad abuser also -- vast numbers of people to come in, and fentanyl to come in," he said in the Oval Office as he signed an array of executive orders.
Trump reignited his tariff threats against Canada and Mexico, two major US trading partners, just hours after taking office. He accused them of failing to curb illegal immigration and drug trafficking into the US.
Earlier on Monday, Trump vowed to "immediately begin the overhaul" of the US trade system "to protect American workers and families."
"Instead of taxing our citizens to enrich other countries, we will tariff and tax foreign countries to enrich our citizens," Trump said in his inaugural address.
As President-elect, Trump, during several instances, had threatened tariff hikes on Canadian and Mexican imports, along with an additional 10 per cent hike on Chinese goods, if these countries did not take tough measures to curb the illegal migration and flow of fentanyl into the United States.
Mexico, Canada and China are leading sources of goods imported by the US, as per official trade data.

Trump, who during his campaign trail had vilified diversity, equity, and inclusion (DEI) policies in the federal government, labelled them as "immoral discrimination programs" in an order to end them.

"The Biden Administration forced illegal and immoral discrimination programs, going by the name 'diversity, equity, and inclusion' (DEI), into virtually all aspects of the Federal Government, in areas ranging from airline safety to the military," read the order.
<h2>Campaign promises fulfilled</h2>
During his campaign, Trump frequently demonized gender diversity, attacking transgender people &mdash; most notably transgender women in women's sports &mdash; and gender-affirming care for children.&nbsp; &nbsp; On Monday, Trump issued an executive order requiring US federal agencies to only recognise two genders &mdash; male and female, removing the option for any other gender identities.

According to Trump's gender order, federal agencies "shall take all necessary steps, as permitted by law, to end the Federal funding of gender ideology."
The Trump administration would only use "clear and accurate language and policies that recognize women are biologically female, and men are biologically male," said the order.
The US president also wiped out 78 executive orders, actions and presidential memoranda issued by his predecessor Joe Biden, including those that prevented "discrimination on the basis of gender identity or sexual orientation," prejudice against LGBTQ Americans in education, as well as equity programmes for Black, Hispanic and Pacific Islander Americans.

<h2>Criticism emerges</h2>
According to AFP news agency, Trump's policies are sure to face legal challenges.
Jami Taylor, a politics professor at Toledo University and an expert on LGBTQ policy, warned that due to Trump's policies, gender-affirming care could be at risk.
This would be especially true in cases where federal funds like state-run insurances Medicare and Medicaid are involved, said Taylor.

The epicentre was recorded 12 kilometres (7.5 miles) north of Yujing, a mango-growing district in southern Taiwan, the USGS said.
Firefighters rescued three people including a child who were trapped in a collapsed house in nearby Nanxi district, video posted on Facebook and verified by AFP showed.
Elsewhere, authorities said a person was injured by falling debris while two people were rescued from elevators.
More than 50 aftershocks have been recorded, said Taiwan's Central Weather Administration which reported the initial quake at magnitude 6.4.&nbsp;
The ceilings of several homes collapsed, while roads were blocked by falling rocks and landslides, the National Fire Agency said.
But the agency reported "no major damage" from the quake, which injured 27 people according to the health ministry.
The aftermath saw classes and office work cancelled in Nanxi district as well as Dapu Township in mountainous Chiayi County, north of the epicentre.&nbsp;
Some roads in Dapu were "damaged and impassable", and water and electricity supplies affected, Chiayi County chief Weng Chang-liang said.
Taiwanese chipmaking giant TSMC said it evacuated workers from some of its central and southern factories when the quake struck.
