In an effort to avoid stigmatising patients who could delay seeking care, New York City has requested the World Health Organization (WHO) to rename the monkeypox virus on Tuesday (July 26). In a letter to WHO Director Tedros Adhanom Ghebreyesus, New York City public health commissioner Ashwin Vasan stated, “We have a growing concern for the potentially devastating and stigmatizing effects that the messaging around the 'monkeypox' virus can have on... already vulnerable communities.” Vasan further stated in his letter that the WHO had suggested changing the name of the virus, connected to smallpox, during a press conference last month, AFP reported.

Currently, with 1,092 infections discovered so far, New York has more cases of the illness than any other city in the US. So far, no deaths have been reported in Europe or the United States.

The second can be, that after the onset of a fever, the skin eruption often starts one to three days later. Instead of the trunk, the rash is more frequently found on the face and limbs. In nearly 95 per cent of the instances, it affects the palms of the hands and soles of the feet with the face.&nbsp;

In general, the case fatality ratio of monkeypox has traditionally fluctuated from 0 to 11 per cent; higher in young children. The case fatality rate recently has been between 3 and 6 per cent, WHO reported.&nbsp;

 
2.&nbsp;Vaccination:

Several observational studies have shown that smallpox vaccination is almost 85 per cent effective in preventing monkeypox.

Smallpox vaccinations cidofovir, ST-246, and vaccina immune globulin can be used to help in treating the virus.

However, in 2019 a brand newer vaccine based on the Ankara strain of the modified attenuated vaccinia virus was authorised for the prevention of monkeypox. This is a two-dose vaccine that is still only partially available.

The WHO said the primary prevention methods for monkeypox include increasing public awareness of risk factors and teaching individuals about the step they may take to avoid exposure to the virus.&nbsp;

As per the guidelines issued by CDC for the prevention. The instructions a persons need to follow include avoiding getting in touch with anyone who has a rash that resembles monkeypox, never touching a person infected with monkeypox, sharing utensils with someone who has monkeypox, using an alcohol-based hand sanitiser, or often washing your hands with soap and water.

CDC also issued preventive guidelines for people who have monkeypox: Stay alone at home or stay in a separate room or area from people and pets you live with.

The virus currently does not have any specific treatment. Typically, patients must stay in a specialised hospital to prevent infection from spreading and to manage general symptoms.&nbsp;

The national capital Delhi has registered its first case of monkeypox virus, officials said on Sunday.

According to PTI news agency, which has cited official sources, the man has no history of foreign travel. However, as per sources, the patient attended a party in Himachal Pradesh. The Delhi government has been asked to track the patient's contacts. Meanwhile, more than 10 close contacts of the patient have already been isolated at his West Delhi residence.&nbsp;

The 31-year-old man, who has not been named, is admitted to Maulana Azad Medical College, confirms Health Ministry. The patient was admitted to the hospital with fever and skin lesions.

Notably, this is India’s fourth monkeypox case. The previous three have been reported from southern Kerala state.

The country's very first case was reported on July 14, while the second one was reported on July 14 in Kerala's Kannur district, and the third one was reported on July 22 in Kerala's Malappuram district.

Kerala Health Minister Veena George had said on Friday, "A 35-year-old man contracted the infection and is currently undergoing treatment at a hospital where he was admitted for fever after returning from UAE on July 6."

On Saturday, the World Health Organisation declared monkeypox a global public health emergency of international concern.

The UN health agency has called on nations to adopt measures that protect the health, human rights and dignity of affected communities.

It informed that more than 16,000 cases have now been reported from 75 countries and there had been five deaths so far.

4.Avoid kissing.

The rash tends to be more concentrated on the face and extremities rather than on the trunk. Spread through sexual activity: Report The largest study to date, involving 528 individuals in 16 nations, was published in the New England Journal of Medicine and found that 95% of cases were spread through sexual activity. Approximately a third of those affected were known to have gone to sex-on-site locations like sex parties or saunas within the previous month. According to WHO director-general Dr Tedros Adhanom Ghebreyesus, more than 16,000 cases of the virus have been reported from 75 countries across the globe. “We have an outbreak that has spread around the world rapidly, through new modes of transmission, about which we understand too little. For all of these reasons, I have decided that the global monkeypox outbreak represents a public health emergency of international concern." said Dr Tedros. WHO director-general took the call himself However, it is interesting to note that the Health Regulations Emergency Committee which had remained undivided on declaring the outbreak a 'global emergency' in a meeting last month had not differed in its stance in the Saturday meeting as well. Nine members were against and six were in favour of the declaration. Reportedly, the WHO director-general, in an unusual departure from the protocol, took the call himself and declared an emergency anyway. It is pertinent to note that the last time WHO issued such a global health emergency, it was for COVID-19, back in early 2020. The entire planet was soon brought to a standstill as the virus created havoc. While WHO has sounded the alarm with its declaration, it cannot mandate the countries and their governments to swing into action immediately. All WHO can do is issue guidelines and give its expertise as well as recommendations to the member countries. The final call of action resides with the national governments. There were 2,593 confirmed monkeypox cases in the USA. Moreover, as reported by WION on Friday, US health officials detected two cases of monkeypox in children while the Centers for Disease Control Prevention and Control (CDC) stated that kids under 8 were at 'increased risk'. The virus has spread rapidly in Europe and United States after it was first detected in May. Blood tests collected from two victims in the southern Ashanti area earlier this month revealed the Marburg virus. As per an AFP report, the Ghana Health Service (GHS) reported that the samples were submitted to the Pasteur Institute in Senegal, which confirmed the diagnosis. The World Health Organization (WHO) reports that Angola, the Democratic Republic of the Congo, Kenya, South Africa, and Uganda have seen previous outbreaks and isolated cases of Marburg.

According to the GHS statement, 98 individuals who have been identified as contact cases are presently quarantined. It also stated that no more Marburg cases have been found in Ghana.

What is the Marburg virus?

What are its symptoms?

The time from infection to the start of symptoms, or incubation period for Marburg, ranges from 2 to 21 days.

As per WHO, the Marburg virus causes sudden onset illness characterised by a high temperature, excruciating headaches, and severe fatigue. Another typical symptom is aches and pains in the muscles. On the third day, severe watery diarrhoea, cramps and discomfort in the abdomen, nausea, and vomiting can be there.

Many patients develop severe haemorrhagic symptoms between 5 and 7 days, and fatal cases typically have some sort of bleeding, often from numerous sites. Bleeding from the nose, gums, and vagina frequently accompanies fresh blood in vomit&nbsp;and faeces.

Is it fatal?

The disease has a case fatality ratio of up to 88 per cent that, with proper patient care, can be significantly lower.

Depending on the virus strain and case treatment, fatality rates in confirmed cases have ranged from 24 to 88 per cent in past outbreaks, according to the WHO.

Death in fatal instances often occurs 8 to 9 days after the beginning of symptoms and is typically preceded by significant blood loss and shock.

Transmission

Prolonged exposure to mines or caves where Rousettus bat colonies are present was earlier the primary cause of human MVD infection.

As long as the virus is present in a person's blood, they are contagious.

Treatment

There are currently no authorized vaccinations or antiviral medications for MVD. However, supportive care, such as rehydration with oral or intravenous fluids, and treatment of symptoms, increase the chances of&nbsp;survival.

More than 1,000 probable cases of unknown severe acute hepatitis have been detected in children and as many as 22 have died since the disease was discovered in on April 5, the World Health Organisation has said "As of 8 July 2022, 35 countries in five WHO Regions have reported 1010 probable cases of severe acute hepatitis of unknown aetiology in children, which fulfill the WHO case definition, including 22 deaths. Since the previous Disease Outbreak News published on 24 June 2022, 90 new probable cases and four additional deaths have been reported to WHO,” the UN health agency said in its update on Wednesday. “Additionally, two new countries, Luxembourg and Costa Rica, have reported probable cases."

Out of those five regions, almost half, i.e., 48 per cent of the probable cases have been reported from the European countries, in which 21 countries have registered 484 cases, the WHO said. Of those, UK and Northern Ireland combined have found 272 cases.

The second highest number of probable cases have been reported from the American continent, including 334 cases from the United States of America, followed by the Western Pacific Region (70 cases), the South-East Asia region (19) and the Eastern Mediterranean region (2).

Seventeen countries are reporting more than five probable cases, with the WHO suspecting it to be a case of undercounting due to limited enhanced surveillance systems.

“The actual number of cases may be underestimated, in part due to the limited enhanced surveillance systems in place. The case count is expected to change as more information and verified data become available,” the agency said.

According to Michael Ryan, executive director of the WHO Health Emergencies Program, the recent increase in the number of newly reported COVID-19 cases has been largely driven by Omicron subvariants BA.4 and BA.5, and by the lifting of public health and social measures.

Recent changes in testing policies have also been hindering the detection of new cases and the monitoring of the evolution of the virus, he said.

Tedros suggested that governments should regularly review and adjust their COVID19 response plans based on the current epidemiology and also the potential for new variants to appear. "Governments should also work to reverse the reduction in surveillance, testing and sequencing, and share anti-virals effectively.”

Europe is at the centre of a new wave of cases driven by the BA.4 and BA.5 omicron sub-variants as people attend large gatherings and resume traveling after two years of staying close to home.

In England, an estimated 2.1 million people, or one in 25, tested positive in the final week of June, according to the Office for National Statistics.

The&nbsp;WHO&nbsp;is worried that even as cases rise again, surveillance of the virus and new potential variants is on the decline.

‘Covid pandemic still not over’, WHO warns as new cases emerge

From January 1 to July 4, 6,027 laboratory-confirmed cases of monkeypox and three deaths have been reported to the WHO from 59 countries.

Since June 27, case numbers have gone up by 77 per cent. During this time period two new deaths have been recorded. Both have taken place in Central African Republic

The WHO said 10 countries have not reported new cases for more than 21 days -- the disease's maximum incubation period.

Some 82 percent of the cases are in Europe and 15 percent are in the Americas.

The countries with more than 1,000 confirmed cases are Britain (1,351), Spain (1,256) and Germany (1,242), according to the European Centre for Disease Prevention and Control, and WHO Europe.

Outside Europe, the WHO report counted 460 confirmed cases in the United States and 300 in Canada.

Sexual contact transmission route

In its first bi-weekly situation report on monkeypox, the WHO said that gender data was available on 4,406 cases, of which 99.5 percent were men.

"The outbreak continues to primarily affect men who have sex with men who have reported recent sex with one or multiple male partners, suggesting no signal of sustained transmission beyond these networks for now," the report said.

Men aged 18 to 44 account for 79 percent of cases, with the median age being 37.

There are six known cases among children aged under 18, and 25 known cases among healthcare workers.
