The Guardian reported that some insulin rights activists, and those who have diabetes, raised their voices to handle the serious issue of the high costs of insulin in the United States.

Insulin, the main anabolic hormone of the body, is produced in the pancreas by special cells, called beta cells. In terms of medication, it is used in any pharmaceutical preparation of the protein hormone insulin that is used to treat high blood glucose.&nbsp;

The study published last month on October 18 in the Annals of Internal Medicine estimated that about 16.5 per cent of adults with diabetes, who are dependent on insulin, did not take full doses as prescribed. It happened because of the issues related to the affordability of the drug.&nbsp;

According to the study, there are eight key proteins that have been attributed to keeping the bears diabetes-free for months. Notably, these proteins also have a human counterpart which the experts theorise could lead to a better understanding of diabetes in humans as well as a potential cure.&nbsp;

While bears are not completely sedentary and spend some of their time in hibernation being awake and moving around, they do not eat. “If you saw this in humans, it would be very medically concerning,” said the researcher at the university as well as who worked on this study, Blair Perry.&nbsp;

Reportedly, in humans, obesity and inactivity are major factors that contribute to Type 2 diabetes, which is also the far most common form of the disease. Furthermore, this type of diabetes occurs when the body can no longer use the hormone insulin properly, which is responsible for regulating the amount of glucose in the blood.&nbsp;

Insulin helps move the sugars from the food to the body cells for energy, however, when people suffer from diabetes these sugars get accumulated in the blood, said the report. Therefore, the cells over time lose their sensitivity to insulin and can’t take up sugar from the bloodstream, this is a precursor to this type of diabetes and is called insulin resistance.&nbsp;

The study elucidates how bears also develop insulin resistance while they hibernate but their blood sugar levels as well as insulin levels remain stable and do not turn into diabetes. The samples were collected from some of the bears housed at the Washington State University Bear Center and the researchers for this study analysed the blood serum and fat cell samples.&nbsp;

The research indicates that some samples were collected during the active season and others during hibernation. During the hibernation period for two weeks, researchers also fed the bears honey water which disrupted their daily routine. Subsequently, the scientists combined different cell cultures and serum samples collected during the active and hibernation period to study the altered gene activity in the cells.&nbsp;

The gene activity in the cells was also noticed when they put the serum from the disrupted-hibernation period onto cells taken from regularly hibernating bears and indicated how the cells showed similar gene activity to that of cells from the active season. This in turn allowed the researchers to narrow down the eight key proteins that appeared between insulin resistance and insulin sensitivity.&nbsp;

In an interview, earlier this year, Perry said how the study published on September 21, lays the foundation for future studies. “The more we understand about how this is working in bear the better footing we'll have for translating this to human studies,” said Perry.&nbsp;

He added these findings could also lead to new medications for treating or preventing diabetes in humans. Speaking about the broader implications of the study the researcher also indicated that during hibernation despite very little movement by the bears they also do not suffer from the negative effects of muscle or bone mass (another health consequence that sedentary humans face). &nbsp;

Well-fed, hibernating bears give clues to possible cure of human diabetes, says study

Did you know diabetes, which is a major cause of bone and joint problems, blindness, kidney failure, heart attacks, stroke and lower limb amputation, was the direct cause of 1.5 million deaths in 2019? And, most people don't even consider it a public health problem that the ability to approach epidemic proportions globally.

According to medical experts at Global Hospital, diabetes is a chronic disease characterised by elevated levels of blood glucose, which if untreated over time can cause damage to vital organs like the heart, blood vessels, eyes, kidneys and nerves. And, recent studies have shown that this chronic disease can also severely affect bone and muscle health, making them more prone to fractures.

According to research published in 2016, there is a 7-fold increase in hip fractures in patients with type 1 diabetes(T1DM). In type 2 diabetes(T2DM), this risk is increased by 1.3 times. Therefore, Dr Sneha Kothari says that it is extremely important for diabetic patients to take extra care of their bone and muscle health and prevent all the musculoskeletal discomforts associated with long-standing, uncontrolled diabetes.

In an exclusive conversation with WION, Dr Sneha Kothari, Consultant Endocrinologist at Global Hospital, Mumbai, shared important tips to maintain bone and muscle health for diabetic patients. Read on!

<h2>How does diabetes affect bone health?</h2>

Our bones undergo continuous repair, says Dr Kothari. However, diabetics and lack of insulin levels in the body interfere with the formation of new bone leading to weak bone and muscle health. Other factors that attribute to poor musculoskeletal health are:

	<li>Reduced release of insulin-like growth factor-1 (IGF-1)</li>
	<li>Poor absorption of calcium in the body&nbsp;</li>
	<li>Increased oxidative stress&nbsp;</li>

	<li>Loss of calcium in the urine&nbsp;</li>
</ul>

Medical expert shares ways to maintain muscle and bone health with diabetes

A new study on the effects of drinking several cups of tea per day has found that the practice has numerous health benefits ranging from lowering the risk of diabetes, stroke, and heart disease to overall mortality.&nbsp;

The study will be presented next week at the annual meeting of the European Association for the Study of Diabetes. According to the researchers, the study involved more than one million adult participants across eight countries.&nbsp;

The result of the study indicated that drinking just one to three cups of your favourite tea beverage lowered the risk of type 2 diabetes by four per cent. Meanwhile, it also reduced the risk of developing diabetes by one per cent for every extra cuppa.&nbsp;

"Our results are exciting because they suggest that people can do something as simple as drinking four cups of tea a day to potentially lessen their risk of developing type 2 diabetes," said a researcher related to the study.&nbsp;

The researchers associated with the study pointed out that the benefits of drinking tea could be related to the presence of&nbsp;polyphenols in tea leaves which produce antioxidants.

These compounds help the human body by reducing inflammation and stress in the body. Consequently, it helps in reducing the risk of certain cardiovascular conditions.&nbsp;

As reported by WION, a study published in the Annals of Internal Medicine last month also showed the positive effects of drinking tea.&nbsp;

Notably, scientists from the US National Cancer Institute studied the tea habits of nearly a half million adults in the United Kingdom and followed them for up to 14 years, taking into consideration risk factors such as health, socioeconomics, smoking, alcohol intake, gender, etc.&nbsp;

In the end, researchers concluded that drinking two or more cups daily lowers risk of death from any cause vs. non-tea drinkers by around 9 per cent&nbsp;to 13 per cent.&nbsp;

“We first tested the binding of PK2 on GLP1R proteins in human cells and found that it is able to bind well toGLP1R proteins. This showed that PK2 can potentially trigger insulin release by the beta cells,” said co-author Khyati Girdhar.

In Type 1 diabetes, the pancreas produces little or no insulin while Type 2 diabetes affects the way the body processes blood sugar or glucose.

“Beyond increasing insulin release, PK2 was also able to prevent and even reverse beta cell loss, a cell essential for insulin production, making it effective for both Type 1 and Type 2 diabetes,” added Mondal.

The study was conducted by universities of Oxford and Bristol. In this study 145,000 people were observed for 5 years. These people took part in 19 global randomised clinical trials. The researchers found that 5 mmHg reduction in systolic blood pressure resulted in reduction of risk of type 2 diabetes by 11 per cent.

In this study, researchers investigated major blood pressure drugs compared with a placebo. They found that angiotensin-converting enzyme (ACE) inhibitors and angiotensin II receptor blockers (ARBs) had the strongest protective effect. Both of these drug types reduced risk of developing diabetes by 16 per cent.

Other types of blood pressure-lowering drugs were not found to be as effective. Drugs like Calcium channel blockers had no effect on type 2 diabetes.

The study was funded by British Heart Foundation National Institute for Health Research (NIHR) Oxford Biomedical Research Centre and the Oxford Martin School.

“COVID triggered it,” Buelna said the doctors told him.

Buelna’s ordeal and similar cases reflect a new worry about the dangerous relationship between diabetes and COVID-19 that’s being urgently studied by doctors and scientists around the world. Many experts are convinced that COVID-19 can trigger the onset of diabetes - even in some adults and children who do not have the traditional risk factors.

It’s already been well-documented that people with diabetes face much higher risks of severe illness or death if they contract COVID-19. In July, U.S. health officials found that nearly 40% of people who have died with COVID-19 had diabetes. Now, cases like Buelna’s suggest the connection between the diseases runs both ways.

“COVID could be causing diabetes from scratch,” said Dr. Francesco Rubino, a diabetes researcher and chair of metabolic and bariatric surgery at King’s College London.

Rubino is leading an international team that is collecting patient cases globally to unravel one of the biggest mysteries of the pandemic. Initially, he said, more than 300 doctors have applied to share cases for review, a number he expects to grow as infections flare up again.

“These cases are coming from every corner of the world and every continent,” Rubino told Reuters.

In addition to the global registry, the U.S. National Institutes of Health is financing research into how the coronavirus may cause high blood sugars and diabetes.

In these situations, symptoms can escalate quickly and become life threatening. These cases may take months to surface after exposure to COVID-19, so the full extent of the problem and the long-term ramifications may not be known until well into next year. More intensive research is needed to definitively prove, beyond the mounting anecdotal evidence, that COVID-19 is triggering diabetes on a wide scale.

“We have more questions than answers right now,” said Dr. Robert Eckel, president of medicine and science at the American Diabetes Association. “We could be dealing with an entirely new form of diabetes.”

‘ABSOLUTELY TERRIFYING’ DIAGNOSIS

Type 1 diabetes occurs when the body’s immune system mistakenly destroys insulin-producing cells in the pancreas, preventing the regulation of blood sugar levels. About 1.6 million Americans have the disease.

This year, doctors also are seeing some people without the risk factors for type 2 diabetes - such as being older or overweight - experience a diabetic emergency after exposure to COVID-19.

In type 1 diabetes, initial symptoms can include extreme thirst, fatigue, frequent urination and weight loss. Arthur Simis had no idea those were signs for the disease.

This summer, he and his wife, Sarah, noticed their 12-year-old son, Atticus, appeared thin and slept a lot. They figured he was stressed out from being trapped at home in the pandemic, or going through a growth spurt.

On July 9, as his symptoms persisted, Arthur Simis took his son to an urgent care center near their home in Gardnerville, Nevada. The medical staff detected dangerously high blood sugar levels and ketones in his urine, both indicators that Atticus was in diabetic ketoacidosis, or DKA.

The doctor told Simis that his son needed hospital care immediately to avoid slipping into a coma from his newly diagnosed type 1. An ambulance took them 50 miles to the nearest hospital in Reno.

His father spent three nights sleeping at his side in the pediatric ICU. He sobbed on the phone to his wife, because only one parent was allowed inside, a measure to control coronavirus infections.

“How could he have diabetes?” Simis recalls asking the doctors. “It was absolutely terrifying.”

Doctors say that’s not unusual in a fast-moving pandemic, as they focus on individual emergencies rather than big-picture research questions. But the lack of testing in many of these cases, they say, may complicate efforts to detect whether and how the coronavirus might be causing diabetes.

CHILDREN IN INTENSIVE CARE

The initial reports of COVID-related diabetes include more children with cases like that of Atticus.

In a study published in August, researchers at Imperial College in London and several hospitals there found that cases of type 1 diabetes among children nearly doubled to 30 during late March to early June - as the pandemic raged - compared to the same period in previous years. Five of the children tested positive for a prior coronavirus infection, but the study’s authors said many of the children were not tested.

In the United States, Children’s Hospital Los Angeles said the percentage of newly diagnosed type 2 patients who arrived in diabetic ketoacidosis, a potentially fatal buildup of acid in the blood, has nearly doubled for March through August compared to the same period in 2018 and 2019.

Dr. Lily Chao, director of the type 2 diabetes clinic there, said the hospital is still investigating whether this increase is driven by exposure to COVID-19.

Brandi Edwards, a registered nurse and diabetes educator at Alabama’s Huntsville Hospital, said calls about pediatric cases began to surge in May. Doctors summon her when a child arrives in the emergency room or ICU so she can counsel the family on insulin injections, glucose readings and how daily life will change going forward.

“We’ve seen more type 1 cases this year than I ever remember,” Edwards said. “There were three kids in the pediatric ICU at the same time. That is so rare.”

The disease knocked him out of work for weeks and wrecked his family’s finances. His wife, Erika, is eight months pregnant and they have a 3-year-old daughter, Katalina. The family got an eviction notice on Aug. 2, while Mario was in the ICU, and they rely on a food bank for some meals.

Buelna said he fell into a depression in the hospital, cut off from family visits, and credits his sister with lifting his spirits in phone calls.

“After a week on a western-style diet, palatable food such as snacks and chocolate becomes more desirable when you are full,” said co-author Richard Stevenson who is&nbsp;a professor of psychology at Macquarie University in Sydney.

To investigate how the western diet affects humans, the scientists recruited 110 lean and healthy students, aged 20 to 23, who generally had&nbsp;a good diet.

The group was divided into two parts. One which ate their normal diet for a week. The other half was&nbsp;put on a high energy western-style diet, which featured a generous intake of Belgian waffles and fast food.

"There are many other alternatives and other medication classes but many of them have side effects, are more expensive and some of the newest ones may not be covered by insurance."

The risk of diabetes rose by about 16 percent for an increase of 10 microgrammes per cubic metre in long-term PM2.5 particle exposure, researchers from Fuwai Hospital in Beijing and Emory University in the US found in a study published online by Environment International last week.

"Sustained improvement of air quality will help decrease the diabetes epidemic in China," Lu Xiangfeng, one of the study's authors, told AFP in an email.&nbsp;

Researchers collected data from over 88,000 subjects across 15 provinces, estimating their exposure to PM2.5 based on satellite data from 2004 to 2015.&nbsp;

PM2.5 includes toxins like sulfate and black carbon, which can penetrate deep into the lungs or cardiovascular system, and have been linked to higher rates of lung cancer, chronic bronchitis and heart disease.

While similar studies in North America, Europe, Hong Kong and Taiwan have linked air pollution with diabetes, researchers say this is the largest study of its kind in mainland China.

"Due to high levels of PM2.5, different exposure pattern and population susceptibility, results from developed countries with low PM2.5 levels were not applicable in China," Lu said.

His team adjusted for factors such as age, body mass index, smoking status, family history of diabetes and work-related physical activity levels, but did not directly factor in dietary habits and other types of pollutants.

Ho Kin-fai, a professor at the Chinese University in Hong Kong who studies air pollutants and is not involved in the study, told AFP the study shows air pollution is a factor in the diabetes epidemic "that we cannot ignore".

But scientists still need to find evidence showing how PM2.5 particles work in the human body to increase risk of the disease," Ho said.

Ho said the study excludes "some other factors in the environment that maybe we haven't considered ... so that's why we need to have more evidence from the biological mechanism to prove it's true."

"It is mere propaganda of our political opponents. Had Mr Sharif wanted a deal he would not have come back from London to go to jail along with his daughter Maryam," she said.&nbsp;

"We want best treatment of Mian Sahib in the light of his test reports," she added.
