<p>The World Health Organization's cancer agency recently warned that the number of new cancer cases will rise to more than 35 million in 2050, which is 77 per cent higher than the figure in 2022. <p>WION explored how to redefine support for breast cancer patients in India and understand the concept beyond diagnosis. To learn more about this, WION got in touch with <strong>Dr Kanchan Kaur, who is the Senior Director, Breast Cancer, Cancer Care, Medanta, Gurugram</strong>, who&nbsp;said, "Breast cancer is the most common cancer in women globally, overtaking lung cancer in 2020 with around 2.3 million new cases, making up 11.7% of all cancer cases."&nbsp;</p>

<p>"Studies predict that global breast cancer cases will reach nearly 2 million by 2030. In India, the incidence of breast cancer in females has increased by 39.1% over the last 26 years. According to 2020 data, breast cancer accounted for 13.5% (178,361) of all cancer cases and 10.6% (90,408) of all deaths in India," Dr Kaur added.&nbsp;</p>

<p>Dr Kaur weighed in on the holistic approach to breast cancer care. She said that the treatment for breast cancer involves using medical measures to cure or alleviate a condition.&nbsp;</p>

<p>"Survivorship, which runs alongside treatment, aims for an improved quality of life. In the context of cancer care, survivorship covers the health and life of a person with cancer from diagnosis to end of life. It includes the lasting physical, social, mental, and financial effects of cancer beyond diagnosis and treatment completion. It also deals with issues like post-treatment follow-up, potential recurrences, second cancers, and overall quality of life," she said.&nbsp;</p>

<p>In recent years, there has been a growing awareness of the need for a holistic approach to breast cancer care, extending beyond medical interventions. Organisations and healthcare providers in India are increasingly focussing on providing comprehensive support services that address the multifaceted needs of individuals grappling with breast cancer, Dr Kaur said.&nbsp;</p>

<p>“Their risk of dying from their breast cancer in the first five years after diagnosis is now five&nbsp;per cent.&nbsp;It can also be used to estimate risk for individual women in the clinic. Our study shows that prognosis after a diagnosis of early breast cancer varies widely, but patients and clinicians can use these results to predict accurate prognosis moving forward," he said.</p>

<p>Breast cancer is more effective to treat if diagnosed early and the effectiveness of treatment in the later stages is poor. The report in the PMC titled- "Artificial Intelligence in Breast Cancer Screening and Diagnosis" says that the incorporation of AI into screening methods is a relatively new and emerging field that&nbsp;shows a lot of promise in the early detection of breast cancer, thus resulting in a better prognosis of the condition.&nbsp;</p>

<p>"Human intelligence has always triumphed over every other form of intelligence on this planet. The defining feature of human intelligence is the ability to use previous knowledge, adapt to new conditions, and identify meaning in patterns. The success of AI lies in the capacity to reproduce the same abilities," it adds.&nbsp;</p>

<p>Incorporating AI into the screening methods such as the examination of biopsy slides enhances the treatment success rate. Machine learning and deep learning are some of the important aspects of AI which are required in breast cancer imaging.&nbsp;</p>

<p>Machine learning is used to store a large dataset, which is later used to train prediction models and interpret generalisations. On the other hand, deep learning- the newest branch of machine learning- works by establishing a system of artificial neural networks that can classify and recognise images, as per the report.&nbsp;</p>

<p>Regarding breast cancer treatment, the use of AI for early detection by making use of data obtained by radiomics and biopsy slides is done. This is backed by a global effort to manufacture learning algorithms for understanding mammograms by reducing the number of false positives as an outcome.&nbsp;</p>

<p>"AI has increased the odds of identifying metastatic breast cancer in whole slide images of lymph node biopsy. Because people's risk factors and predispositions differ, AI algorithms operate differently in different populations," the report further says.</p>

And for AI machines to be accepted, people must be able to independently replicate and produce the machine like any other scientific finding. <p>AI models used for managing cancer are centred on image data, and the report says the problem with this aspect is the underutilisation of patient histories saved as electronic health records in hospitals.&nbsp;</p>

<p>The study is published in PLOS Medicine based on data from 9,498 women who developed breast cancer in the age group between 20s and 40s. After five years of usage of oral contraceptives, almost 8 in every 100,000 women were found to have developed breast cancer in the age group 16 to 20. On the other hand, 265 in every 100,000 women from the age group 35 to 39 were found to have developed breast cancer.</p>

<p>However, researchers are of the view that the drawbacks of using oral contraceptives must be rationally weighed against the proven benefits. Claire Knight of Cancer Research UK made it clear that the findings of the new study must not discourage women from using oral contraceptives as they were one of the most effective ways to avoid pregnancy.</p>

<p>Claire said, “There are lots of possible benefits to using contraception, as well as other risks not related to cancer. That’s why deciding to take them is a personal choice and should be done after speaking to your doctor so you can make a decision that is right for you.”</p>

<p><strong>2) Imaging tests:</strong>&nbsp;These tests help in producing images of the breast tissue that allow doctors to spot any abnormalities. Breast X-rays (mammograms) and ultrasounds, which use sound waves to create images are some examples of imaging tests.&nbsp;</p>

<p><strong>3) Biopsy:</strong>&nbsp;Breast cancer can only be diagnosed definitively through a biopsy. An analysis of the biopsy sample also helps in determining the type of cells involved in breast cancer, cancer's aggressiveness (grade), and whether the cancer cells have hormone receptors or other receptors that may influence the treatment options.&nbsp;</p>

<p>Post diagnosis, the doctor determines the stage of cancer which is done similarly to female cancer i.e. through PET scan, or ultrasound abdomen, chest x-ray and bone scan. Post this, a feasible treatment option based on the overall health and preferences of the patient is suggested to the patient. However, male breast cancer is often treated by surgery and may include other treatment methods as well, such as:&nbsp;</p>

<p><strong>1) Surgery:&nbsp;</strong>The surgery helps in removing the tumour and surrounding breast tissue. The surgery involves - mastectomy i.e., removing all the breast tissues including the nipple and areola and sentinel lymph node biopsy i.e., removing of few lymph nodes if the cancer is found during the biopsy.</p>

<p><strong>2) Radiation therapy:</strong>&nbsp;In this high-energy beams such as X-rays and protons are used to kill the cancer cells. In male breast cancer, it is often performed to remove the remaining cancer cells (if any) in the breast, chest muscles or armpit to prevent the recurrence of cancer.</p>

<p><strong>3) Hormone therapy:</strong>&nbsp;In most cases of breast cancer in males the development or spread of the tumour is hormone sensitive. The hormone-sensitive tumour tends to have oestrogen and/or progesterone receptors in them. In such cases, hormone therapy as a treatment is suggested. The therapy includes medications such as Tamoxifen is used mainly to treat hormone receptor-positive breast cancer. However, the same should be taken only in consultation with the doctor.&nbsp;</p>

<p><strong>4) Chemotherapy:</strong>&nbsp;Chemotherapy is performed to kill cancer cells by using medications. A doctor may recommend chemotherapy after surgery to kill the remaining cancer cells (if any) that might have spread outside your breast. <p>Dr Kataria said that if the breast cancer has been treated and as we know that one of the treatments is surgery, women opt for breast removal because most of the women would go for investigation in advanced stage breast cancer so that breast won't be there for feeding.&nbsp;</p>

<p>She added, "But the other breast should be there, and if the women become pregnant in future after the breast cancer treatment, they are able to feed. And it does not in any way compromise lactation, breastfeeding, or produce any complication in the child."&nbsp;</p>

<p>Dr Kataria also noted that the "woman should wait for at least two years after the treatment is over". She said that after a gap, all the side effects of the chemotherapy or the treamment that is given to her, or medicines that she has been taking are not in her body to affect the future child.&nbsp;&nbsp;</p>

<p>In India, breast cancer is the most common cancer in women. It accounts for 26.3% of all female cancers. Around 60% of breast cancer cases in India are discovered at an advanced stage, despite the fact that it is generally preventable and treated if discovered early.&nbsp;</p>

<p>To know more about breast cancer and the importance of its awareness in India, WION spoke to Dr Tejinder Kataria, Chairperson, Radiation Oncology, Cancer Institute, Medanta, Gurugram, India.&nbsp;</p>

<p>During an online conversation, Dr Kataria said that the population in India is spread out over a big area of around a 3.2million square kilometres, and out of that 35% - 40% of patients that's amongst women, the cancer is breast cancer.&nbsp;</p>

<p>She said that approximately 200,000 women per year, and is expected to be 250,000 by 2025, will be diagnosed with breast cancer. And of these, 60% of the women come to us in stage 3 and stage 4.&nbsp;</p>

<p>In stage 3 and stage 4, the cure rates are between 40% to 60%, whereas in stage 1 and stage 2, (or early breast cancer when we can pick it up via breast mammography and or breast cancer awareness programmes), the cure rates are 85% to 95%.&nbsp;</p>

<p>"So it makes sense that we make people aware that for breast cancer, they should go in for investigation or they should go in for an examination, a self-breast examination should be done, and mammography needs to be made aware of to people so that we pick early breast cancers," said Dr Kataria.&nbsp;</p>

<p>"It will reduce not only the emotional and financial burden on the family, but also give us better cure rates and also better outcomes for the women," she further said, adding that a breast cancer awareness programme is a must for such a big country like India.&nbsp;</p>

<p><strong>The right approach to spread awareness at the grassroots level&nbsp;</strong></p>

<p>Dr Kataria said that to reach the grassroots level, "we need to integrate the breast cancer awareness programmes into our primary health centres, mohalla clinics, or dispensaries. So when a woman goes for her regular examination, especially at the time of childbirth they would always go to the hospital and would go to the doctor."&nbsp;</p>

<p>"So we should rope in the gynaecologists because normally the breast cancer surgeons have been traditionally males so the women are not very forthcoming about their symptoms. But with their gynaecologists, they are very open and share their body structure and any complaints they have," she added.&nbsp;</p>

<p>Dr Kataria suggested the implementation of respective school education so that adolescents should be made aware of what breast cancer is and how their body form may change. They should not be ashamed of it.&nbsp;</p>

<p>Dr Kataria also added that the stigma and shame that is generally associated with cancer do not allow the women to open up about their symptoms.&nbsp;</p>

<p>"We need to work, even the media and the communications that have to go out from various agencies should highlight these things," she said.&nbsp;</p>

<p>"So this has to be an integrated programme from top to bottom to the grassroot level, where at every level we need to make the trainers and train the trainers, who can teach the women how to self-breast examine themselves and make the facilities avalable to them," Dr Kataria said.&nbsp;</p>

<p>"We need to look at these things to know how to create systems, where the women feel empowered to come in and get themselves examined. It should be an annual check, starting at an age of 40 years," Dr Kataria said.&nbsp;&nbsp;</p>
