Iran's civil aviation body, in a final report, has blamed an error by an air defence operator for the shooting down of a Ukrainian passenger plane in January 2020 that killed all 176 people aboard, the agency said on Wednesday.&nbsp;

Ukraine's foreign minister criticised the report, calling it a cynical attempt by the Islamic Republic authorities to cover up the true reasons for the crash.&nbsp;

The Iranian report said, "The plane was identified as a hostile target due to a mistake by the air defence operator...near Tehran and two missiles were fired at it," according to the agency's website.&nbsp;

"The flight's operation did not have a role in creating the error by the air defence battery," the report added.&nbsp;

"We will not allow Iran to hide the truth, we will not allow it to avoid responsibility for this crime."&nbsp;

The Tehran government has allocated $150,000 for damages to be paid to families of the crash victims said several people have been put on trial over the disaster.&nbsp;

Ukraine has said the compensation should be set through talks, taking into account international practice, once the causes of the tragedy are established and those responsible are brought to justice.&nbsp;

A pilot and co-pilot, both of major rank were killed along with two soldiers, Xinhua news agency quoted the statement as saying. 
 
No loss of life was reported at the ground by the military. 
 
A local source from GB told Xinhua that the incident occurred at a remote valley with no residential settlement on ground. 
 
The source added that military teams arrived at the crash site following the accident and recovered the bodies from the wreckage of the completely destroyed helicopter.

A two-seater trainer aircraft crashed in Dhenkanal district in Odisha on early Monday morning. The aircraft was carrying a trainee pilot and an instructor.

Pakistan recently tried to launch a cruise missile called “Babur II” but suffered a major setback as the launch was marred by a crash.

The US Federal Aviation Administration has identified a new risk that Boeing Co must address on its 737 MAX before the grounded jet can return to service, the agency said on Wednesday.

Two people briefed on the matter told Reuters that an FAA test pilot during a simulator test last week was running scenarios seeking to intentionally activate the MCAS stall-prevention system. During one activation it took an extended period to recover the stabilizer trim system that is used to control the aircraft, the people said.

It was not clear if the situation that resulted in an uncommanded dive can be addressed with a software update or if it is a microprocessor issue that will require a hardware replacement.

In a separate statement, Boeing said addressing the new problem would remove a potential source of uncommanded movement by the plane's stabilizer.

A hardware fix could add new delays to the plane's return to service.

The FAA also said on Wednesday that it continues "to evaluate Boeing's software modification to the MCAS and we are still developing necessary training requirements. We also are responding to recommendations received from the Technical Advisory Board. The TAB is an independent review panel we have asked to review our work regarding 737 Max return to service."

American Airlines Group Inc and Southwest Airlines Co earlier cancelled flights through early September as a result of the grounding. On Wednesday, United Airlines said it also was removing MAX flights from its schedule through Sept. 3.

It said the IAF had been coordinating with the state administration at all levels and a ground team comprising its Garud commandos, the Indian Army's special forces personnel, civilian porters and hunters reached the crash site late Wednesday.

"The mortal remains will (now) be sent to their hometown, where their funeral will be carried out as per service customs," the IAF said.&nbsp;

"The IAF stands with the families and next-of-kin of all the air-warriors who lost their lives in the line of duty," it added.

After tracing the wreckage on June 11, a team of 15 mountaineers was air-dropped near the crash site the very next day to locate the bodies. Later, three more mountaineers had joined the team.

The site is about half a mile from Trump Tower, where US&nbsp;President Donald Trump maintains an apartment. The area has been under a temporary flight restriction since his election in November 2016.

The pilot would have required permission from air control at La Guardia Airport in New York, and investigators are trying to determine if the pilot was in contact with the tower there or at any other airport, de Blasio said.

Furthermore, the authority extended its condolences to their families and relatives of the deceased, whose names have not been revealed yet.

German holidaymakers were the second largest group after British tourists to visit the islands -- known as the Pearl of the Atlantic and the Floating Garden in the Atlantic -- in 2017, according to Madeira's tourism office.&nbsp;

The islands are home to just 270,000 inhabitants.&nbsp;

Filipe Sousa, mayor of Santa Cruz where the accident happened, said 17 women and 11 men were killed in the crash, with another 21 injured.&nbsp;

A doctor told reporters another woman died of her injuries in hospital.

"I express the sorrow and solidarity of all the Portuguese people in this tragic moment, and especially for the families of the victims who I have been told were all German," President Marcelo Rebelo de Sousa told Portuguese television.

He said he would travel to Madeira overnight.&nbsp;

Starting Friday, BEA investigators will try to retrieve information from the cockpit voice and flight data recorders, which were damaged in the disaster.

Thousands of miles (kilometers) away, distraught families were demanding answers as they visited the deep black crater where the plane smashed into a field outside the capital, disintegrating on impact.

Ethiopian Airlines, Africa's largest carrier, sent the black boxes to France because it does not have the equipment to analyze the data. &nbsp;

The information that they contain helps explain 90 per cent of all crashes, according to aviation experts.

The move came after a growing number of airlines and countries had already decided not to fly the planes or ban them from their airspace until it was ascertained there are no safety issues.

US President Donald Trump told reporters the "safety of the American people and all peoples is our paramount concern."

FAA acting chief Daniel Elwell said the agency had been "working tirelessly" to find the cause of the accident but faced delays because of the damage to the flight data recorders.

The new information shows "the track of that airplane was close enough to the track of the Lion Air flight... to warrant the grounding of the airplanes so we could get more information from the black boxes and determine if there's a link between the two, and if there is, find a fix to that link," Elwell said on CNBC.

Boeing's shares have fallen 12 per cent in the days after the crash in Ethiopia, wiping out nearly $30 billion in value.&nbsp;

The MAX series is Boeing's fastest-selling model, but it is still relatively new with fewer than 500 in service.

There are 74 registered in the United States and 387 in use worldwide with 59 carriers, according to the FAA.

Pilots concerns

The accounts of the recent crashes were echoed in concerns registered by US pilots on how the MAX 8 behaves.

At least four American pilots complained following the Lion Air crash that the aircraft would suddenly pitch downward shortly after takeoff, according to documents reviewed by AFP on the Aviation Safety Reporting System, a voluntary incident database maintained by NASA.

In two anonymous reports on flights just after the Lion Air disaster, pilots disconnected the autopilot and corrected the plane's trajectory.

It was unclear if US transport authorities review the database or investigated the incidents. However, the FAA said this week it had ordered Boeing to update its flight software and training.

Questions about the Lion Air crash have honed in on an automated stall prevention system, the MCAS, designed to automatically point the plane's nose downwards if it is in danger of stalling.

At least 20 people were killed and 43 injured when a locomotive smashed through the buffers at the main train station in the Egyptian capital Cairo on Wednesday, the health minister said.

Transport Minister Hisham Arafat resigned after the incident, the cabinet said in a statement. State television said authorities had arrested the train driver. 
Footage circulating on social media from a security camera inside the Ramses station showed the train failing to stop as it arrived at platform six, smashing through the buffers and a metal end railing and exploding into a huge ball of fire.

Passengers carrying luggage ran for their lives as the fire spread, and several people were running covered in flames, witnesses said.

The health minister, Hala Zayed, said authorities had not been able to identify many of the 20 bodies recovered because they were too badly burned.

Witness Ibrahim Hussein said: "I saw a man pointing from the locomotive as it entered the platform, and screaming `There are no brakes! There are no brakes!`, before he jumped out of the locomotive. And I don`t know what happened to him."&nbsp;

Security sources said there was no indication that the crash was deliberate. The transport minister said the train`s diesel tank had exploded.

Egypt has one of the oldest and largest rail networks in the region and accidents causing casualties are common. A train collision in Alexandria killed more than 40 people in 2017.&nbsp;

Egyptians have long complained that successive governments have failed to enforce basic safety standards for the railways.

Ahmed Youssef, a doctor at Al Helal hospital, close to the station, said more than 20 injured had been admitted, most of them in serious condition.&nbsp;

Witness Ahmed Abdeltawab said the platform had been crowded with passengers waiting for another train and people fled in flames.&nbsp;

"The fire overwhelmed them and they ran while they were on fire until they fell metres from the incident," he said. "The scene was terrifying."

Prime Minister Mostafa Madbouly, announced from the station that the incident would be investigated and anyone found responsible would be held to account.&nbsp;

Student Ali Ramadan said he had suffered burns and injured his foot when he ran into a concrete bench on the platform.&nbsp;

"I don`t know when these train accidents will end ... They told us they got millions of dollars` worth of new locomotives and trains, and people are still dying because of train accidents," he said.
