Coronavirus cases rise again in Germany; officials warn people

Children have been largely spared from the worst impacts of COVID-19 but now children around the world infected with the COVID-19 are developing new symptoms of a rare inflammatory disease. This highlights a new potential risk for children in the pandemic. The symptoms developed are similar to the Kawasaki disease or toxic-shock syndrome. Kawasaki disease is a mysterious illness that primarily affects children up to the age of five. It causes the walls of arteries to become inflamed, resulting in fever and joint pain.

Though frightening, most recover without serious issues from the Kawasaki disease but with this new illness, some critically affected children are ending up in intensive care units with shock-like symptoms. New York has seen its first death from this rare illness caused by COVID-19.

A 5-year-old boy in New York has died from this inflammatory illness linked to COVID-19. Health officials are also investigating 73 similar cases reported across New York. All of these children have exhibited symptoms similar to Kawasaki disease when infected with COVID-19.

"While rare, we're seeing some cases where children affected with the coronavirus can become ill with symptoms similar to the Kawasaki disease or toxic shock-like syndrome that literally causes inflammation in their blood vessels. This past Thursday, a five-year-old boy passed away from COVID-related complications. And the state department of health is investigating several other cases," says New York Governor Andrew Cuomo.

So how is this illness linked to COVID-19 similar to the Kawasaki disease?

Some of the symptoms of both the diseases overlap in children but the mysterious illness in COVID-19 patients causes severe abdominal pain, nausea and vomiting than the Kawasaki disease. The Kawasaki disease also majorly impacts heart vessels but in the COVID-19 associated illness-- only a few cases describe any vessel inflammation.

The age of the children affected by this mysterious illness is also not typical of the Kawasaki disease. Many children infected with this illness are above the age of 5.

More than 1,000 people queued up on Saturday to get free food parcels in Geneva, underscoring the impact of the coronavirus epidemic on the working poor and undocumented immigrants even in wealthy Switzerland.

The line of people stretched for more than 1 km (half a mile) outside an ice rink where volunteers were handing out around 1,500 parcels to people who started queuing as early as 5 a.m.

"At the end of the month, my pockets are empty. We have to pay the bills, the insurance, everything," said Ingrid Berala, a Geneva resident from Nicaragua who works part-time. "This is great, because there is food for a week, a week of relief...I don`t know for next week."

In a nation of nearly 8.6 million, 660,000 people in Switzerland were poor in 2018, charity Caritas says, particularly single parents and those with a low level of education unable to find work after losing a job.

More than 1.1 million people were at risk of poverty, which means they have less than 60% of the median income, which was 6,538 Swiss francs ($6,736) for a full-time job in 2018.

Swiss bank UBS has calculated that Geneva is the second-most expensive global city for a family of three to live in, behind only Zurich. While average incomes are also high, that helps little for people struggling to make ends meet.

"I think a lot people are aware of this, but it is different to see this with your own eyes," said Silvana Matromatteo, head of the aid group Geneva Solidarity Caravan.

"We had people in tears who said `It is not possible that it is happening in my country`. But it is here and maybe the COVID-19 brought everything out and this is good, because we will be able to take measures to support all these workers, because they are workers above all."

Patrick Wieland, chief of mission for the Doctors Without Borders group, said a survey last week showed just over half the food recipients interviewed were undocumented, while others had attained legal status, were Swiss or were seeking asylum.

Just over 3% had been tested positive for COVID-19, three times the overall rate in Geneva, which he attributed to poor and overcrowded housing.

"In Geneva, one of the richest cities in the world, there have always been people living precariously, especially all the people who work as housekeepers, in agriculture, on construction sites or in hotels, and they found themselves overnight without a job because of COVID-19," he said.

One illegal immigrant who called himself Fernando said he lost his restaurant job during the crisis and had no pay.

"I`m very grateful to receive this help and if the situation changes for me, I am committing to do the same thing that they are doing for me," he said.

South Korea will further relax social distancing rules from May 6, allowing a phased re-opening of businesses, as the nation has largely managed to bring the coronavirus outbreak under control, Prime Minister Chung Sye-kyun said on Sunday.

Widespread testing, intensive contact tracing and tracking apps have enabled South Korea to limit the spread of the virus rather than rely on the lengthy lockdowns seen elsewhere.

The government “will allow businesses to resume at facilities in phases that had remained closed up until now, and also allow gatherings and events to take place assuming they follow disinfection guidelines,” the prime minister told a televised meeting of government officials.

Further easing of rules means public facilities such as parks, libraries as well as schools could reopen in phases, although the Korea Centers for Disease Control and Prevention recommended citizens to continue to exercise some caution in everyday life.

The new guidelines from the government advise everyone to stay home for three to four days if they feel unwell, stay arms-apart with others in public spaces, and wash hands frequently.

Seoul extended its social distancing policy until May 5 even as it managed to bring down daily infection cases of more than 900 in late February to around 10 per day in the past week.

The national tally is at 10,793 as of midnight Saturday, with 250 deaths.

Despite experts' warnings, Bangladesh has reopened hundreds of its garment factories this week after nearly one month of closures. The factory owners said they were operating with fewer workers than usual and following safety guidelines.

A spokesman of the Bangladesh Garments Manufactures and Exporters Association (BGMEA) said that some 850 among nearly 4,000 factories were reopened maintaining cautions, engaging a limited number of workers who live nearby.

BGMEA Secretary Mohammad Abdur Razzak claimed that the factories were complying with the health guidelines while the decision followed a government directive allowing companies which were under buyers' pressures to resume production.

A BGMEA business analyst said the factories were opened fearing that they might lose business to competitors like Vietnam, Cambodia, Sri Lanka and India.

But a health ministry official preferring anonymity said the factories should have taken some more days for safety preparedness for their workers, who are at worst risk of infections.

There are nearly 4.1 million garment workers in the country.

Global lockdowns led the international apparel brands and retailers, who rely on the cheap labour that Bangladesh provides, to cancel or suspend an estimated USD 3.17 billion worth of orders in the country.

DGHS Additional Director General Professor Nasima Sultana said that Bangladesh was witnessing a rapid rise of coronavirus cases since March 24 as 8,784 people tested positive for the virus since then. The number of infection was only 6 until March 23. She said the COVID-19 cases were found in 63 out of the country's 64 administrative districts, Bangladesh confirmed the first coronavirus death on March 18, ten days after the detection of the first three cases. As of May 2, of the total fatalities, 95 deaths were recorded in Dhaka city and 39 in Narayanganj, the river port town on the outskirts of the capital. Dhaka is the worst-affected area with 54.73 per cent of COVID-19 cases.

The United Kingdom now has Europe's second-highest official death toll from the novel coronavirus pandemic, according to new figures on Wednesday that cover fatalities in all settings, including in nursing homes.

Some 26,097 people died across the United Kingdom after testing positive for COVID-19 as of April 28 at 1600 GMT, Public Health England (PHE) said. That means the United Kingdom has had more COVID-19 deaths than France or Spain have reported.

"These more complete data will give us a fuller and more up to date picture of deaths in England and will inform the government's approach as we continue to protect the public," Yvonne Doyle, medical director at PHE, said.

Such a high UK death toll increases the pressure on Prime Minister Boris Johnson's government which is facing criticism from opposition parties for being too slow to impose a lockdown and too slow to introduce mass testing.

In mid-March, the government's chief scientific adviser said keeping the UK death toll below 20,000 would be a "good outcome".

Although international comparisons are difficult, the new figures confirm Britain's place among the European countries worst hit hardest by the pandemic.

Italy, which has the world's second-highest death toll after the United States, said on Wednesday that 27,682 people had died after testing positive for coronavirus.

Like Britain, its figures are based on deaths following positive coronavirus tests in all settings.

Spain reported 24,275 deaths from the coronavirus at the last count, less than Britain's new toll published on Wednesday. Still, Spain's population is around 20 million smaller so it has a higher prevalence of deaths per capita.

Research from Germany and Italy suggests that footballers and other athletes face a particular risk of the coronavirus infecting their lungs, raising major questions over attempts to restart professional football.

The research, produced by Italian immunologists and lung specialists based at institutes in Berlin, Rome and Verona, suggests that due to strenuous exercise, elite athletes are more likely to inhale virus particles and direct them to the lower areas of the lung.

COVID-19, the disease caused by the coronavirus, can cause lung damage and complications such as pneumonia and, in severe cases, acute respiratory distress syndrome (ARDS).

The preprint paper here which has yet to be peer-reviewed, also suggests that athletes who are asymptomatic could make their condition worse by infecting their lungs during strenuous exertion.

Football has ground to a halt in all major leagues in Europe and none have yet to resume. European football's governing body UEFA has set a 25 May deadline for leagues to outline their plans to re-start.

Leagues, governing bodies and clubs, however, have said they will only return when play is safe and that they will take medical advice.

In their paper: “The First, Comprehensive Immunological Model of COVID-19”, Paolo Matricardi, Roberto Dal Negro and Roberto Nisini raise questions over the safety of playing while the virus remains at large.

“The pattern of breathing during strenuous exercise changes dramatically by a tremendous increase of ventilation (i.e.: inspiratory and expiratory volumes of air), and of alveolar ventilation in particular,” the authors state.

“Professional athletes (are) particularly exposed (much more than individuals of common population) due to their frequent practice of extreme and long-lasting exercise.”

Also read: Irrfan Khan (1967-2020): The man who spoke a thousand words through his eyes

The researchers state that the “ideal lungs” of athletes, while helpful in normal conditions, significantly favour the deep inhalation of infectious agents.

“Even the SARS-CoV-2 can then spread more easily to the deepest areas of the lungs during strenuous exercise, and there start its aggressive action,” they said.

Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) is the name given to the 2019 novel coronavirus. COVID-19 is the name given to the disease associated with the virus.

“Not by chance, a great proportion of professional football players claimed the occurrence of fever, dry cough and malaise (and dyspnea in some cases) immediately after, or a few hours following their last official match,” note the authors.

Asymptomatic athletes

Adding to the dangers, the research says that players who have the virus but do not show symptoms, could make their condition worse by allowing the virus to move from their upper to lower airways.

Asymptomatic but infected athletes could exhale or eliminate aerosolised particles that may contain viruses which are then re-inhaled.

“These droplets or aerosol might be re-inhaled and facilitate the spread of the virus from the upper to the lower airways,” the paper states.

The authors also look at the risk of the virus being transmitted during a game.

“In sports where many athletes are in close contact, such as team sports or marathons, the same particles have high chances to be inhaled by other athletes, facilitating viral transmission.

“To emphasise that strenuous exercise induces a much more frequent spitting of secretions and this can further contribute to the environmental SARS-CoV-2 spreading, particularly if the distancing recommendations are not strictly followed.”

A separate new study from Aarhus University, in Denmark, looking at how much exposure players would have to a single infected player on the field, showed that, on average, a player is positioned within an ‘exposure zone’ for one minute and 28 seconds during a match.

On Tuesday, the World Players Association, which represents some 85,000 athletes from different sports in over 60 countries, said competitors should not be rushed back to action.

“At the moment there is a lot of pressure from the leagues on all continents to resume,” WPA Executive Director Brendan Schwab told Reuters in an interview.

“The players can only agree to that (return) if they know that their interests will be protected. “

Soccer’s global players’ union FIFPro has also urged caution.

“We need guidance and protocols on how to return in a healthy and safe manner. Football is a contact sport and we feel very high protection standards are required,” said FIFPro secretary general Jonas Baer-Hoffmann.

According to Office for National Statistics, deaths at care homes from suspected coronavirus had contributed to the record number of deaths in England and Wales in April even as Britain's health minister said he was "very worried" at signs of coronavirus-related syndrome emerging in children.

"I'm very worried about the early signs that in rare cases, there is an impact of an auto-immune response in children that causes a significant disease," Health Secretary Matt Hancock said, adding,"it's a new disease that we think may be caused by coronavirus and the COVID-19 virus."

"We're doing a lot of research now. What I would also stress is that it is rare. Although it is very significant for those children who do get it, the number of cases is small," the minister added.
