Earlier this year, US Food and Drug Administration (FDA) approved Leqembi (lecanemab-irmb), a medicine which gained much traction last year, for the treatment of Alzheimer’s disease.&nbsp;

“Alzheimer’s disease immeasurably incapacitates the lives of those who suffer from it and has devastating effects on their loved ones,” Billy Dunn, MD, director of the Office of Neuroscience in the FDA’s Center for Drug Evaluation and Research had said.&nbsp;

The new medicine from Eli Lilly also works in a similar manner to that of Leqembi.&nbsp;

Both Leqembi and donanemab, the two breakthrough medicines for the treatment of a disease which results in memory problems and disrupts daily life,&nbsp;work as antibodies.&nbsp;

Normal antibodies attack the viruses but these medicines have a difference in how they function. Both medicines are engineered in such a way as to clear sticky gunk from the brain, called beta-amyloid.

"The decades-long battle to find treatments that change Alzheimer's disease is changing," said Dr Cath Mummery, the clinical lead for the cognitive disorders clinic at the UK's National Hospital for Neurology and Neurosurgery.

"We are now entering the time of disease modification, where we might realistically hope to treat and maintain someone with Alzheimer's disease, with long-term disease management rather than palliative and supportive care."

<h2>Side effects of donanemab</h2>

Brain swelling is the most commonly seen side effect of the medicine donanemab. The volunteers in the research of the medicine included a total of 1,734 people. As part of the procedure, the medicine was infused on a monthly basis till distinctive plaques were cleared from the brain.&nbsp;

Though the future of Alzheimer's treatments is making headways through astounding feats like these, Lilly said the incidence of serious brain swelling in the donanemab study was 1.6 per cent. Two volunteers lost their lives because of the condition and a third death was also reported to have died after an incident of serious brain swelling.

Researchers from Griffith University in Australia conducted experiments in 2022 with the Chlamydia pneumoniae bacterium, which can infect people and result in pneumonia. It is pertinent to note that this bacteria have been found in the majority of human brains with late-onset dementia.

It has been proven that germs may go up the olfactory nerve in mice, connecting the nasal cavity and the brain. Additionally, nerve infections were worse when the nasal epithelium, the thin tissue that lines the roof of the nasal canal, was damaged.

As a result, the amyloid-beta protein, which is secreted in response to infections, was deposited in the mouse brains in greater amounts. People with Alzheimer's disease have substantial amounts of plaques (or clumps) of this protein as well.

Neuroscientist James St John from Griffith University in Australia reportedly said,&nbsp;"We're the first to show that Chlamydia pneumoniae can go directly up the nose and into the brain where it can set off pathologies that look like Alzheimer's disease."

"We saw this happen in a mouse model, and the evidence is potentially scary for humans as well."

The rapidity with which C. pneumoniae infected the central nervous systems of the mice, infection occurring within 24 to 72 hours, surprised the researchers. Bacteria and viruses may view the nose as a direct path to the brain, according to certain theories.

"These findings may eventually lead to the development of imaging techniques that allow us to diagnose Alzheimer's disease earlier and more accurately," Isaacson said, "and monitor its progression noninvasively by looking through the eye."

If doctors are able to identify the disease in its earliest stages, people could then make healthy lifestyle choices and control their modifiable risk factors, like high blood pressure, high cholesterol, and diabetes, Isaacson said. This research shows promise for early Alzheimer's detection, and could ultimately lead to better treatment and prevention strategies for this devastating disease.

A team of researchers from the University of East Anglia (UEA) and the University of Edinburgh in the United Kingdom have found that hormone replacement therapy (HRT), used to relieve symptoms of menopause in women, may help prevent Alzheimer’s disease. The study published in the Alzheimer’s Research &amp; Therapy journal, earlier this week, shows evidence of HRT reducing the risk of the aforementioned disease in women carrying the APOE4 gene. While APOE4 is the strongest risk factor gene for the disease, inheriting it does not necessarily mean that a person will definitely develop the condition.&nbsp;

The recent study found that HRT was most effective when taken before periods stop and the onset of symptoms which then build up for months or years, also known as perimenopause. However, researchers have also said that they cannot say with absolute certainty that HRT will reduce the risk of Alzheimer’s disease in women, but the findings were important given the limited treatment and prevention options for dementia.&nbsp;

“In addition to living longer, the reason behind the higher female prevalence is thought to be related to the effects of menopause and the impact of the APOE4 genetic risk factor being greater in women,” said professor Anne-Marie Minihane from UEA’s Norwich Medical School who co-led the study with the University of Edinburgh, professor Craig Ritchie. She added, “We wanted to find out whether HRT could prevent cognitive decline in at-risk APOE4 carriers”.&nbsp;

The findings were based on the data analysed from 1,178 women who are a part of the European Prevention of Alzheimer’s Dementia initiative, which was set up to study the participants’ brain health over time. The initiative has reportedly tracked over 1,900 people across 10 countries who were aged over 50 and did not have dementia at the beginning of the study.&nbsp;

Notably, while the study does not look at dementia cases, Minihane said that cognitive performance and lower brain volumes are predictive of future dementia risk. The team of researchers from the UK looked at MRI scans which recorded the results of the cognitive tests and brain volumes and found that APOE4 carriers who used HRT had better cognition and higher brain volumes. This data was then compared to those who did not take the HRT and also were non-APOE4 carriers.&nbsp;

“An effect on both cognition and brain changes on MRI supports the notion that HRT has tangible benefit. These initial findings need replication, however, in other populations,” said Ritchie. Additionally, he also highlighted the need to challenge many assumptions about Alzheimer’s disease and its treatment, particularly in women.&nbsp;

Scientists in UK believe that they are on the cusp of “treating or preventing ” Alzheimer’s disease.

Researchers at the University of Leicester said they have come up with a “transformative” treatment — a vaccine that would cost only 15 euros (around US $6) per dose.

Experts said the new vaccine could enter clinical trials in humans within two years.

According to the scientists, the “groundbreaking” therapy works by preventing the formation of harmful deposits of protein in the brain linked to memory loss.

Alzheimer’s disease is thought to be caused by the abnormal build-up of proteins in and around brain cells.

One of the proteins involved is called amyloid, deposits of which form plaques around brain cells. The other protein is called tau, deposits of which form tangles within brain cells.

A new study has revealed that four weeks on a diet of highly processed food can lead to a strong inflammatory response in the brains of ageing rats, accompanied by behavioural signs of memory loss.

Also, replacing processed foods with omega-3 fatty acid DHA reduced the inflammatory effects in older rats.&nbsp;

However, no such cognitive issues were found in young rats that were given a processed food-enriched diet.

Senior study author Ruth Barrientos, an investigator in The Ohio State University Institute for Behavioral Medicine Research and associate professor of psychiatry and behavioural health said, "The fact we're seeing these effects so quickly is a little bit alarming."

He further highlighted that the findings also indicated that processed diet can lead to abrupt memory deficit and rapid memory decline in older people. This can be a cause of Alzheimer's.

The research team randomly assigned 3-month-old and 24-month-old male rats to their normal chow (32 per cent calories from protein, 54 per cent from wheat-based complex carbs and 14 per cent from fat), a highly-processed diet (19.6 per cent of calories from protein, 63.3 per cent from refined carbs, cornstarch, maltodextrin and sucrose, and 17.1 per cent from fat), or the same processed diet supplemented with DHA.

According to the findings, people that carry&nbsp;the newly identified OAS1 gene variant called 'rs1131454', were more likely to have Alzheimer's disease, increasing carriers' baseline risk of Alzheimer's by an estimated 11-22 per cent.&nbsp;

The new variant identified by researchers is common, as it affects more than half of all Europeans, and it is more predictive of Alzheimer's risk than several known risk genes.

Study suggests that Alzheimer's and COVID-19 share genetic risk factor

According to a report by Alzheimer’s Disease International (ADI), over 41 million people living with dementia worldwide remain undiagnosed.

Experts are of the opinion that getting diagnosed with the disease is vital, as it will ensure that those affected would receive timely support and treatment, which is more effective the earlier it begins, and to take part in clinical trials.

Meanwhile, another research done by McGill University in Montreal, Canada, showed that as many as 90% of people with dementia have not been diagnosed in some countries.

Dementia is one of the biggest health challenges worldwide. According to the World Health Organisation (WHO), it is currently the seventh leading cause of death among all diseases and one of the major causes of disability and dependency among older people worldwide

Paola Barbarino, the chief executive of ADI, said a lack of awareness and stigma within healthcare systems was severely hampering efforts to support people living with dementia.

“This misinformation in our healthcare systems, along with a lack of trained specialists and readily available diagnosis tools, have contributed to alarmingly low diagnosis rates,” said the Guardian quoted Barbarino, who also serves as a board member of the World Dementia Council.
