Sexual health clinics on the frontline of the monkeypox response are already financially stretched, leaving the United States and UK ill-equipped to tackle the first major global health test since the COVID-19 pandemic.

Infectious disease experts say sexual health clinics - which offer confidential walk-in diagnosis and treatment – are best placed to identify and treat cases of monkeypox, which is largely affecting men who have sex with men.

Yet such programs are doing so largely without additional funding despite years of financial neglect. There is little data on funding for sexual health services globally, but experts agree that the sector is under-resourced.

That has impeded the monkeypox response and diverted scarce resources needed to curb rising rates of other sexually transmitted diseases (STD), sexual health experts in the UK and the United States told Reuters.

This lack of resources could result in further spread of monkeypox, they said. According to the World Health Organization (WHO), the outbreak has reached 63 countries.

"This is a gap and a weakness in our public health system&nbsp;that monkeypox has exposed," Dr. Meg Doherty, director of the WHO's Global HIV, Hepatitis and STIs Programmes, said in an email.

"Even among high income countries, funding for sexual health globally is decreasing or remains unfunded," she said.

Although monkeypox is being transmitted through sexual contact, the virus can infect anyone who comes in contact with pus-filled sores, body fluids, respiratory droplets or contaminated linens.

Clinic staff must take extra time cleaning exam rooms and buy additional protective gear. When added to chronic low staffing, a problem exacerbated by the COVID pandemic, appointments can be hard to come by. Testing equipment in the United States is also in short supply.

Monkeypox has been endemic in parts of Africa for years, but since early May, the virus has been spreading rapidly in other countries mainly among gay and bisexual men, who often present with rashes in the genital and anal area that doctors may confuse with herpes or syphilis. Out of 11,500 confirmed cases globally, 1,469 are in the United States and 1,856 are in the UK, according to a Reuters tally.

In Spain, with 2,447 cases - the most in the world - patients are being seen in hospitals and health clinics. Local health authorities said they added extra testing capacity early in the outbreak and the health system was not under pressure.

FUNDING CUTS 

Data from a UK charity called the Health Foundation show England's sexual health services' budget was cut by 14% over a six-year period ending in fiscal 2022. UK clinics have yet to receive any new funding to meet the additional monkeypox demand.

In the United States, funding for sexual healthcare and research likewise has dwindled, said David Harvey, executive director of the National Coalition of STD Directors (NCSD).

"Monkeypox has shone a glaring light on this fact. Broadly speaking, the STD field has been vastly underfunded for 20 to 30 years," said Harvey, whose group has requested $100 million in funding to address the monkeypox outbreak.

Federal funding for STD programs has remained relatively flat in the past two decades - $168.5 million in 2003 vs $152.5 million in 2022 - a 40% drop when accounting for inflation, according to the NCSD. State and local investments also have fallen, even as STD rates reached all-time highs, according to a 2021 National Academies of Sciences, Engineering and Medicine report.

A survey of NCSD's membership released on June 12 found that 42.7% of U.S.-based sexual health clinics have access to appropriate testing swabs, and less than 20% have appropriate tubes for collecting monkeypox specimens for transport to clinical labs.

With limited providers and appointment slots, clinics must triage patients, prioritizing those with pain or rash or suspected monkeypox. That has left asymptomatic patients and those in need of other services without care.

Harun Tulunay, 35, a sexual health advocate who is HIV positive and was recently hospitalized with monkeypox in London, knows of friends whose sexual health appointments were delayed or unobtainable "because they (clinics) are too busy dealing with monkeypox."

He is not surprised, as he experienced the time-consuming decontamination efforts when he was diagnosed.

"We see it overwhelming clinics," he said. "If we suddenly have 5,000 cases, what will happen?"

Last week, a coalition of sexual health/HIV organizations in the UK called for urgent funding support, seeking £51 million ($60 million) for sexual health services to "contain and eliminate monkeypox" as some clinics reported drastic drops in essential services.

Part of the issue is that funding for HIV and other sexually transmitted infections is largely separate, especially in the United States.

"There's been a lot of money put into HIV, which is appropriate, however, STI has been a pauper," said Dr. Matt Golden, director of the HIV/STD clinic serving Seattle and Washington state's King County. His clinic has diagnosed half of the area's monkeypox cases as well as provided vaccines and treatment guidance.

Experts in both countries agreed that monkeypox was a wake-up call for more investment in sexual health. "It is time we do something," said NCSD's Harvey.

Number of monkeypox infections rose last week in New York, a city of more than eight million people. On Friday (July 15), number of infections reached 461 new since the start of the outbreak. On Monday last week, this number was at 223.

Robert, who declined to give his last name, told AFP he sat at his computer refreshing the website "like a crazy person" until he got a slot.

"This shouldn't be an issue because there's already a vaccine, and it (the rollout) should be... more efficient to prevent it from becoming more of an issue," said the 28-year-old. His partner and his best friend have not been able to get a vaccine yet.

Russia on Tuesday said it had detected the first case of monkeypox in the country in a man who returned from a trip to several European nations.&nbsp;

"The infection was found in a young man who came back from a trip through European countries and went to a medical facility with a characteristic rash," health watchdog Rospotrebnadzor said in a statement.&nbsp;

It said the man has been isolated in hospital and that he has a "mild" form of the disease.&nbsp;

Russia reports first case of monkeypox

German researchers recently discovered that patients with monkeypox have high chances for virus load that can be ‘potentially’ contagious. With assistance from the Bernhard Nocht Institute for Tropical Medicine (BNITM) and University Medical Centre Hamburg-Eppendorf (UKE), the researchers examined the hospital rooms of two monkeypox. The results were reported in the journal Eurosurveillance, which revealed that all surfaces that the patients’ hands had directly touched viral contamination, with the largest amounts being found in patients' bathrooms and also detected on the fabrics of chairs of patients, IANS reported.

However, it was unknown during the time of the test if and to what extent the patients had touched the surfaces, researchers found viral DNA on “all other investigated surfaces in the patients’ rooms.”&nbsp;

All of the hand-contact locations in the anteroom that were investigated produced positive PCR results. Both the room door handles, placed in the hallway outside the rooms, had DNA traces on them.&nbsp;

Researchers discover that monkeypox patients shed significant virus load that are ‘potentially’ infectious

The World Health Organisation (WHO) on Thursday issued its first report on the spread of monkeypox. The report details typical profile of those affecrted by the outbreak. Available statistics say that almost all monkeypox patients are male. The median age of those infected is 37. The WHO said that three-fifths of those infected identify as men who have sex with men.

A surge in monkeypox infections has been reported since early May outside the West and Central African countries where the disease has long been endemic.

From January 1 to July 4, 6,027 laboratory-confirmed cases of monkeypox and three deaths have been reported to the WHO from 59 countries.

Since June 27, case numbers have gone up by 77 per cent. During this time period two new deaths have been recorded. Both have taken place in Central African Republic

The WHO said 10 countries have not reported new cases for more than 21 days -- the disease's maximum incubation period.

Some 82 percent of the cases are in Europe and 15 percent are in the Americas.

The countries with more than 1,000 confirmed cases are Britain (1,351), Spain (1,256) and Germany (1,242), according to the European Centre for Disease Prevention and Control, and WHO Europe.

Outside Europe, the WHO report counted 460 confirmed cases in the United States and 300 in Canada.

Sexual contact transmission route

In its first bi-weekly situation report on monkeypox, the WHO said that gender data was available on 4,406 cases, of which 99.5 percent were men.

"The outbreak continues to primarily affect men who have sex with men who have reported recent sex with one or multiple male partners, suggesting no signal of sustained transmission beyond these networks for now," the report said.

Men aged 18 to 44 account for 79 percent of cases, with the median age being 37.

There are six known cases among children aged under 18, and 25 known cases among healthcare workers.

WHO issues first situation report on spread of monkeypox

The World Health Organization (WHO) has warned that the risk of the spread of Monkeypox in Europe, including Russia, is high.

"According to the WHO assessment, at the regional level, the risk in the WHO European Region is high over reports about the widespread outbreak which engulfed several previously untouched countries and some atypical cases," the head of the World Health Organization (WHO) Melita Vujnovic told Russian news agency Sputnik.

"The WHO recommends that all the countries should make every effort to register new cases and trace contacts to control this outbreak and prevent its further spread", Vujnovic added.

Monkeypox is a rare viral disease that is usually transmitted to people from wild animals and is endemic in some African countries.

The disease can be transmitted through body fluids, respiratory droplets and other contaminated materials.

The illness usually results in fever, rash and swollen lymph nodes.

"Compounding the challenge is the stigmatization of men who have sex with men in several countries. Many may simply choose not to present to health authorities, fearful of possible consequences. We know from our lessons in dealing with HIV how stigma further fuels outbreaks and epidemics, but allowing our fear of creating stigma to prevent us from acting may be just as damaging", he said.&nbsp;

A study published in The Lancet Infectious Diseases journal on Saturday has suggested that the symptoms of monkeypox cases in the UK are different than the previous virus outbreak reported in the rest of the world.

The peer-reviewed study came to the conclusion after analysing 54 patients who attended sexual health clinics in London and were diagnosed with monkeypox during a 12-day period in May 2022.

The findings suggested that these patients had a higher prevalence of skin lesions in the genital and anal area, suggesting transmission during close skin-to-skin contact, such as sexual activity. And lower prevalence of tiredness and fever than in cases from previously studied outbreaks of monkeypox.

Based on this study’s findings, the authors called for a review of the definition of the current ‘probable cases’ of monkeypox to help identify the cases.

“Currently, the UK and several other countries are seeing a rapid increase in monkeypox cases among individuals attending sexual health clinics, with no apparent links to countries where the disease is endemic. Monkeypox is a novel diagnosis within the sexual health setting and our study, the first to publish on cases from this UK outbreak, will support future case finding and clinical care.” says Dr Nicolo Girometti, from the Chelsea &amp; Westminster Hospital NHS Foundation Trust.&nbsp;

The 54 patients observed in this study represent 60 per cent of the cases reported in the UK during the 12-day study period during May 2022, the Lancet study said.

All except two of the patients in the cohort were not aware of having been in contact with a known case and none reported travel to sub-Saharan Africa, however many had recently visited other European countries.

The researchers have called for additional resources to support services in managing this condition.

“Monkeypox is a novel diagnosis within the sexual health setting and our study, the first to publish on cases from this UK outbreak, will support future case finding and clinical care,” Girometti said.

"Today, I am intensifying my call for governments and civil society to scale up efforts in the coming weeks and months to prevent monkeypox from establishing itself across a growing geographical area. Urgent and coordinated action is imperative if we are to turn a corner in the race to reverse the ongoing spread of this disease," WHO Regional Director for Europe, Hans Henri Kluge said.

Majority of the countries where monkeypox cases have reached three figure mark are in Europe.

Earlier this week, WHO Director General Tedros Adhanom Ghebreyesus said that he was 'concerned' about 'sustained transmission' of monkeypox virus.

"I am concerned about sustained transmission because it would suggest that the virus is establishing itself and it could move into high-risk groups including children, the immunocompromised and pregnant women," &nbsp;he said. He was talking with reporters.

Last week the UN health agency convened an emergency committee of experts to decide if monkeypox constitutes a so-called Public Health Emergency of International Concern (PHEIC), the highest alarm that the WHO can sound.&nbsp;

WHO calls for 'urgent' action in Europe to contain monkeypox spread

The World Health Organisation (WHO) has said that the rapidly spreading monkeypox does not represent a ‘global health emergency at this stage’ but called for close surveillance.

At least 3,000 monkeypox cases across more than 50 countries have been identified since early May, WHO data states.

The WHO convened its emergency committee to determine the level of threat monkeypox currently poses to the international community.

WHO Director-General Tedros Adhanom Ghebreysus convened the committee on Thursday after expressing concern about the epidemic of monkeypox in countries that haven’t previously reported the disease.

In a statement on Saturday, the WHO said that many aspects of the outbreak were “unusual” and acknowledged that monkeypox — which is endemic in some African countries — has been neglected for years.

“While a few members expressed differing views, the committee resolved by consensus to advise the WHO director-general that at this stage the outbreak should be determined to not constitute” a global health emergency, WHO said in a statement.

Declaring monkeypox to be a global emergency would mean the UN health agency considers the outbreak to be an extraordinary event and that the disease is at risk of spreading across even more borders&nbsp; It would also give monkeypox the same distinction as the COVID-19 pandemic and the ongoing effort to eradicate polio.

WHO previously made similar declarations for diseases including COVID-19, Ebola in Congo and West Africa, Zika in Brazil and the ongoing effort to wipe out polio.

WHO says monkeypox not a global health emergency ‘at this stage’

With more countries coming under the grip of the monkeypox outbreak, the World Health Network (WHN), an independent coalition of scientists formed to tackle the COVID-19 threat, has declared the disease a pandemic.&nbsp;The WHN said that the essential purpose of declaring a pandemic is to achieve a concerted effort across multiple countries or over the world to prevent widespread harm.&nbsp;An infection is declared pandemic when it grows over a wide area, crossing international boundaries, and usually affecting a large number of people.

There are 3,417 confirmed monkeypox cases reported across 58 countries and the outbreak is rapidly expanding across multiple continents, it said in a statement.

The announcement comes ahead of the World Health Organisation (WHO) meeting which is currently taking place (June 23) to decide on monkeypox outbreak designation.

“There is no justification to wait for the monkeypox pandemic to grow further. The best time to act is now. By taking immediate action, we can control the outbreak with the least effort, and prevent consequences from becoming worse,” said Yaneer Bar-Yam, PhD, President of New England Complex System Institute and co-founder of WHN.&nbsp;

“The actions needed now only require clear public communication about symptoms, widely available testing, and contact tracing with very few quarantines. Any delay only makes the effort harder and the consequences more severe.”

More than 3,300 cases of monkeypox have been confirmed by the US Centres for Disease Control and Prevention so far in 42 nations where the virus is uncommon. Monkeypox had not previously produced sizable epidemic outside Africa until this month. However, the scientists haven’t discovered any significant genetic alterations in the virus.

WHO considers declaring monkeypox a global emergency

In order to unify the response to the virus, the World Health Organization says it has removed the distinction between endemic and non-endemic countries in its data on monkeypox.

42 countries have reported 2,103 confirmed cases of monkeypox between January 1 and June 15 to the Geneva-based organisation.

According to data published by WHO, 84 per cent of confirmed cases of monkeypox are largely centered in Europe.

On June 23, WHO will hold an emergency meeting to determine whether to classify the global monkeypox outbreak as a public health emergency of international concern, which is the highest alarm&nbsp;it can sound.

Exploring the possibility that the disease could be sexually transmitted, the UN health agency is looking into reports that the monkeypox virus is present in the semen of patients.

It comes after scientists detected viral DNA in the semen of a handful of monkeypox patients in Italy and Germany.

Catherine Smallwood, monkeypox incident manager at WHO/Europe, said in a press briefing, "It doesn't change our assessment of the current transmission routes that we're seeing at the moment, which are very much largely based on very close physical proximity between individuals, skin to skin contact, skin to mouth and that's really what's driving the transmission at the moment."

Although initial symptoms of monkeypox include a high fever, swollen lymph nodes and a blistery chickenpox-like rash, the US Centers for Disease Control and Prevention has said that current cases do not always present flu-like symptoms.&nbsp;

Patients also displayed anorectal pain, tenesmus and rectal bleeding. The CDC said it could be similar to syphilis, herpes and advised clinicians to perform skin,&nbsp;anal, vaginal and oral examinations.

As monkeypox cases rise in US, CDC issues new guidance

The World Health Organization’s European regional office said on Wednesday that the continent is currently at the “epicentre” of a major monkeypox outbreak. The last few months has seen a serious increase in cases all around Europe and although there have not been many deaths, the infection rate has caused a lot of concern among the authorities. According to the official WHO data, around 85 per cent of the total number of cases in the world – roughly 1500 – is currently present in 25 countries across Europe. &nbsp;Originating from Africa, the monkeypox virus has been spreading at an alarming rate and WHO will decide whether to categorise it as an international health emergency.

"The magnitude of this outbreak poses a real risk; the longer the virus circulates, the more it will extend its reach, and the stronger the disease’s foothold will get in non-endemic countries," said the regional director for Europe Hans Kluge according to a report by AFP.

The latest guidelines from WHO about Monkeypox asked patients to isolate if they think that they are infected with the virus. Kluge once again repeated the same and urged patients to follow rules.

In a number of countries, the emergency workers are being vaccinated with smallpox vaccines and Kluge said that there are "limited amounts of vaccines and antivirals for monkeypox and limited data on their use", according to AFP.
