“The NTSB expressed concern that this amount of force applied during landing or rollout could result in a large input to the rudder pedals and a sudden, large, and undesired rudder deflection that could unintentionally cause loss of control or departure from a runway,” the statement added.

The NTSB recommended that Boeing come up with an alternative solution and warned pilots about the issue.

The agency's concern is the latest setback for Boeing. Since the beginning of this year, the aircraft manufacturer has been facing severe flak- ranging from a mid-air blowout of a door plug in January to a strike by 33,000 workers that started earlier in September.

Two US transportation regulators said Monday they will investigate a fatal car crash involving a&nbsp;Tesla&nbsp;that apparently had no one behind the wheel.

The vehicle was speeding when it hit a tree late Saturday night in Spring, Texas, near Houston. It caught fire, destroying the car entirely and killing two men, police said.

The National Transportation Safety Board (NTSB) said on Twitter it had dispatched two investigators to the scene, who "will focus on the vehicle's operation and the post-crash fire. NTSB investigators will arrive in the area later this afternoon."

As of Sunday, local investigators had not determined whether the driver's side airbag deployed, or if the vehicle's driver assistance system was deployed at the time of the accident.

The circumstances of the accident have provoked numerous reactions on social media, re-launching the debate on&nbsp;Tesla's existing semi-autonomous capabilities, such as the "Autopilot" software that allows the car to park on its own or navigate on the highway.

People can also buy a more expensive version called "FSD," for "full self-driving," even though the driver is not supposed to let go of the steering wheel.

The crash is the 28th Telsa accident to be investigated by the National Highway Traffic Safety Administration (NHTSA), which regulates vehicle safety.

It is also being probed by the National Transportation Safety Board (NTSB), which removed&nbsp;Tesla&nbsp;as a party to an earlier investigation into a fatal crash in 2018 after the company made public details of the probe without authorisation.

In Saturday's accident, the 2019&nbsp;Tesla&nbsp;Model S was traveling at high-speed near Houston when it failed to negotiate a curve and went off the road, crashing into a tree and bursting into flames, Herman said.

Authorities found the bodies of two men in the car, one in the front passenger seat and the owner of the car in the backseat.

"We have witness statements from people that said they left to test drive the vehicle without a driver and to show the friend how it can drive itself," Herman said.

Tesla's Autopilot is a driver assistance system that handles some driving tasks and allows drivers to take their hands off the steering wheel at times, but&nbsp;Tesla&nbsp;says its features "require active driver supervision and do not make the vehicle autonomous."

Tesla boss Musk rejected the idea that the vehicle's semi-automated driving software was to blame: "Data logs recovered so far show Autopilot was not enabled &amp; this car did not purchase FSD," in a reference to Full Self-Driving,&nbsp;Tesla's separate beta semi-automated driver assistance system that still requires driver supervision.

Musk added that "standard Autopilot would require lane lines to turn on, which this street did not have," referring to road markers that need to be captured by a vehicle's cameras to enable autopilot.

Tesla&nbsp;has access to operational and diagnostic data delivered to its servers at "regular intervals" from the car, which has been impounded by police. It is unclear whether investigators will be able to retrieve data directly from the event data recorder in the severely burned vehicle.

Tesla&nbsp;warns that driver assistance systems Autopilot and Full Self-Driving Capability do not make the vehicles autonomous, and that active supervision is still required.

Musk, however, regularly praises advances made by his company in self-driving technology.

Two tractor-trailers then plowed into the bus and a third big rig slammed into the other trucks in a&nbsp;chain&nbsp;reaction&nbsp;that also involved a car, Limani said. All three trucks, at least one belonging to FedEx Corp, were hauling shipments of packages, he said.

Five&nbsp;people from the entangled vehicles were killed and about&nbsp;60&nbsp;others were injured, two of them described as critically hurt, although all of those taken to hospitals were expected to survive, according to Limani. Three area hospitals took in a total of 57 patients from the accident, according to a state police tally.

Limani said the bus, owned by Z&amp;D Tour Inc, was en route from Rockaway, New Jersey, where the company is based, to Cincinnati. But Z&amp;D owner Chen Dan Yu said the ill-fated bus trip originated from Manhattan's Chinatown district and was headed to multiple Ohio destinations, the New York Times reported.

Chen told the Times that Z&amp;D had contracted with a Chinatown company called Ohio Coach for ticket sales on that route, which he said his company drives daily.

Emergency personnel also faced a language barrier as some of the bus passengers spoke only Japanese, while others spoke only Spanish, Limani said. The injured ranged in age from seven years old to some believed to be their&nbsp;60s, he said.

The cause of the&nbsp;crash&nbsp;remained under investigation. But Limani said some motorists reported deteriorating weather conditions at the time of the accident in an area that lies in the snow belt of western&nbsp;Pennsylvania.

In its announcement on Friday, the company said that shortly before the crash, the vehicle's "Autopilot was engaged with the adaptive cruise control follow-distance set to minimum."

Autopilot allows drivers to take their hands off the wheel for extended periods under certain conditions. Tesla requires users to agree to keep their hands on the wheel at all times before they can use Autopilot. Users, however, routinely brag they can use the system to drive hands-free.&nbsp

In its Friday statement, Tesla also said vehicle logs from the accident showed no action had been taken by the driver right before the crash and that he had received earlier warnings to put his hands on the wheel.

The statement did not say why the Autopilot system apparently did not detect the concrete divider.

Tesla Inc shares fell more than 9 per cent on Wednesday following a downgrade by Moody's Investors Service, a probe by the National Transportation Safety Board of a recent fatal crash and concerns about Model 3 production.

The sell-off came after Tesla tumbled 8.2 per cent on Tuesday, the lowest close in almost a year. Tesla was trading on Wednesday at $258.44, down $20.74 a share, off a low of $252.10.

Late on Tuesday, Moody’s Investors Service downgraded Tesla’s credit rating to B3 from B2. Moody’s said the ratings “reflect the significant shortfall in the production rate of the company’s Model 3 electric vehicle.” It also “faces liquidity pressures due to its large negative free cash flow and the pending maturities of convertible bonds.”

Tesla has $230 million in convertible bonds maturing in November 2018 and $920 million in March 2019.

Moody’s said its negative outlook for Tesla “reflects the likelihood that Tesla will have to undertake a large, near-term capital raise in order to refund maturing obligations and avoid a liquidity shortfall.”

Moody’s said Tesla is targeting weekly production of 2,500 Model 3 vehicles by the end of March, and 5,000 per week by the end of June, down from the company's year-earlier production expectations of 5,000 per week by the end of 2017 and 10,000 by the end of 2018.

Tesla declined to comment on the downgrade. The company plans to provide an update on Model 3 production next week.
