It is this snake that is suspected of preying on the victim. After we caught him, we found the victim’s body in the snake’s stomach,” the local police chief told the Detik news site.

After nearly 100 children died this year from acute kidney injury, the Indonesian government issued a ban on all syrup and liquid pharmaceutical prescription and over-the-counter sales on Wednesday.

The ban was implemented as the health officials of the Southeast Asian nation looked into an unexplained increase since January in the number of paediatric deaths from acute renal damage.

It also follows a controversy involving four cough syrups made in India that resulted in the deaths of nearly 70 kids in The Gambia earlier this year from AKI.

The syrups brought into The Gambia, according to the Indonesian food and drug regulator, are unavailable in the Southeast Asian nation.

"As a precaution, the ministry has asked all health workers in health facilities not to prescribe liquid medicine or syrup temporarily... we also asked drug stores to temporarily stop non-prescription liquid medicine or syrup sales until the investigation is completed," he added.

The increase in AKI cases started in January of this year and has picked up speed since late August, according to the ministry spokesperson, who also noted that an investigation was started last week.

A stampede at a football stadium killed over 130 people in Indonesia last month. The Indonesian president has announced plans to demolish and replace the ground. Molly Gambhir brings you the details.

WION Fineprint | Indonesia to demolish stampede stadium

Indonesia will demolish and rebuild a football stadium where a stampede killed more than 130 people this month, President Joko Widodo said on Tuesday as he vowed to "thoroughly transform" the sport in the soccer-mad nation.

The president, popularly known as Jokowi, was speaking to reporters at the state palace after meeting Gianni Infantino, head of world soccer governing body FIFA.

"Kanjuruhan stadium in Malang...we will demolish it and rebuild according to FIFA standards," he said.

The deadly stampede after a league match in the city of Malang on Oct 1 has been blamed on police firing tear gas in the stadium, a crowd control measure banned by FIFA.

The president said he had agreed with Infantino on significant changes to how the sport was managed in Indonesia.

"We agreed to thoroughly transform Indonesian soccer," he said. "Every aspect of preparation...needs to be based on FIFA standards."

The meeting between Jokowi and Infantino comes after Indonesia and FIFA agreed to form a joint task-force in the wake of the stadium tragedy, and as the country prepares to host the Under-20 World Cup next year.

Speaking alongside Jokowi, Infantino said FIFA's first priority was to ensure the safety of both players and fans in the Southeast Asian nation.

"This is a football country, a country where football is a passion for over 100 million people," he said. "We owe it to them that when they see a match they are safe and secure."

Infantino, who presented the president with a red FIFA jersey with his name printed across it, said the global soccer body would work closely with the government to ensure all stadiums met safety requirements, and that the Under-20 World Cup ran smoothly next year.

Under pressure to explain what caused one of the world's deadliest stadium disasters, a fact-finding team formed by the government released a report last week which concluded that the "excessive" and "indiscriminate" use of tear gas was the leading cause of death.

As fans attempted to exit the stadium after the defeat of the home side Arema FC by Persebaya Surabaya, they were caught in a deadly crush, with many, including more than 40 minors, dying from asphyxiation.

The report also identified other contributing factors, including the stadium being filled beyond capacity, locked exit doors, and a push by league officials to hold the game at night to secure better ratings for a local broadcaster, despite a request by police to hold a match between fierce rivals during the day.

The fact-finding team also said that more than three hours of CCTV footage about an hour after the match had been deleted. The team has requested that police provide this footage.

The team also called on the head of the Indonesian Football Association to resign.

Indonesia to demolish and rebuild football stadium where stampede killed over 130

Investigation into the deadly Indonesia soccer stampede by a fact-finding team has revealed that tear gas was the main cause of death in one of the world's worst stadium disasters, the country's security minister said on Friday. On October 1, over 130 people were killed at the Kanjuruhan stadium in East Java after a match. The team, which included government officials, soccer and security experts, has been investigating how the massive tragedy happened.

The usage of tear gas triggered a massive stampede in the stadium as fans panicked and tried to escape through the exit gates. There was a huge accumulation of fans at the exit gates as many got trampled to death while others succumbed due to suffocation.

Coordinating Security Minister Mahfud MD said a separate team was still investigating the toxicity of the gas used. However, no matter what the results of the other probe are, it could not "diminish the conclusion that the massive (number of) deaths was mainly caused by tear gas", he said.

Indonesian football Association (PSSI) and other officials have faced questions over the use of tear gas, a crowd control measure that the world soccer governing body FIFA does not endorse and has banned. Dozens of officers are currently under investigation for their actions on the day.

The fact-finding team found the police personnel on duty did not know about the the prohibition of tear gas at soccer matches. The tear gas had been fired "indiscriminately" and the officers had employed "excessive" measures, the team said. Investigation into the deadly Indonesia soccer stampede by a fact-finding team has revealed that tear gas was the main cause of death in one of the world's worst stadium disasters, the country's security minister said on Friday.

Also Read | What do FIFA rules state about use of tear gas to control crowds? In an effort to clear their name, police has been pushing across the fact that narrow doors in the over-capacity stadium were responsible for worsening the crush. The investigators also found PSSI guilty of ignoring regulations and have called for the resignation of its chairman and executive committee. The Indonesian Football Association (PSSI) and world soccer governing body FIFA will form a joint taskforce in a bid to improve crowd control and safety measures after a deadly soccer stampede earlier this month, officials said on Thursday (October 13).

The joint taskforce will also include members of the Asian Football Confederation (AFC) as well as the police and the ministries of sport, home affairs, health and public works. Under the plan, Indonesian police would ensure their standard operating procedures are synchronised with FIFA regulations, PSSI head, Mochamad Iriawan, told reporters after a meeting with FIFA officials.

Indonesian president Joko Widodo has said FIFA won't impose any sanctions on the country over the recent stadium disaster which left at least 131 dead and hundreds injured. In one of the worst football tragedies of recent times, fans were crushed to death after a deadly stampede took place at the Kanjuruhan Stadium in Malang, Indonesia.

The tragic incident took place after the game between hosts Arema FC and their bitter rivals Persebaya Surabaya on October 1. Angry Arema FC fans stormed onto the pitch after their 3-2 defeat in the game and some reportedly clashed with the police officers present at the ground.

The police fired tear gas shots to control the crowd which led to a deadly crush at the exit gates. Hundreds were crushed and suffocated due to lack of oxygen while many sustained injuries in the stampede. FIFA president Giani Infantino had earlier called it a 'dark day' for those involved in football.

However, the Indonesian president said that FIFA won't sanction the country's football federation over the incident. He said FIFA and the Indonesian government will collaborate to transform football in the nation. He also stated that FIFA will help Indonesia in establishing safety standards for football stadiums and will also formulate the guidelines on security procedures to be carried out by the police.

"Thank God Indonesian football is not sanctioned by FIFA. Furthermore, FIFA and the Indonesian government will jointly set up a transformation team for Indonesian football, and FIFA will be based in Indonesia during the process," Widodo said in a video message.

"The Indonesian government, AFC and FIFA will collaborate to, firstly, establish safety standards for stadiums in Indonesia, and will formulate standards and protocols for security procedures carried out by the police based on international security standards," he added.

Following the tragic incident, Widodo ordered a probe into the mishap and had temporarily suspended the Indonesian football league until further orders. The Indonesian football federation had revealed that a number of exit games at the stadium were still locked at the time when police used tear gas on fans which led to the stampede.

While two Arema FC supporters have been banned for life, a number of police officers have been suspended. Criminal charges will be brought against six people, including three police officers.

Indonesia stampede: Cops, match organisers among six facing criminal charges

The Indonesia Police have been accused of firing tear gas into the spectator stands and blocking the gates of the football stadium during the stampede which killed at least 131 people and left hundreds injured.

The witnesses believe that had the police acted swiftly and not blocked the stadium’s exits, they could have minimised the casualties during the chaos.

Several fans who witnessed the riot spoke to Al Jazeera and revealed how the situation had already become tense 15 minutes before the end of the match as it became clear that the home team – Arema Football Club – was losing to arch-rivals Persebaya, a club from nearby Surabaya City.

“Police were 100 per cent to blame”, Fitria Rahmawati, who attended the match in Malang city with her husband and children, told Al Jazeera.

She claimed that the spectators, some of whom were drunk, broke the barricades and stormed into the match after the match ended, prompting the police to counter with the indiscriminate firing of tear gas.

The tear was not only directed at those who invaded the pitch but also at those people who were in the stands, Rahmawati added.

“Why in the hell did they shoot tear gas into the upper grandstand?” she was quoted as saying.

The allegations were supported by another witness, a university student, who came to the match with his seven friends. One of them was killed in the carnage.

“Some fans began chanting racist slurs at Persebaya players so the MC started telling the crowd to be calm and go home early,” Surya was quoted as saying.

“After the siren rang, one fan ran across the field carrying a flag that said ‘You play a s**t game’,” he said.

Witnesses blame Indonesian police for their response to football riot

Locked exit gates and the delay in opening them for panicked supporters contributed to the deadly stampede inside Kanjuruhan Stadium in Malang, the Indonesian football association said on Monday. At least 131 people lost their lives while more than 300 supporters sustained injuries in one of the worst football tragedies of recent times.

Erwin Tobing, the chief of the Indonesian football association's discipline commission admitted all exit gates should have been open but some of them were closed. "The doors should have been open, but were closed," said Tobing.

The Kanjuruhan Stadium was packed with the supporters of Arema FC for their game against bitter rivals Persebaya Surabaya. Surabaya fans had already been banned from attending the game owing to the possibility of violence. However, the game took a deadly turn after the full-time whistle as hundreds of angry Arema supporters stormed onto the pitch.

It resulted in chaos as some fans reportedly also clashed with the police officers present inside the stadium. The police fired tear gas shots to control the crowd, triggering a massive stampede as fans tried to flee the stadiums through narrow exit gates, some of which had still not been opened.

The Indonesian football association said all exit gates at the stadium should have been opened at least ten minutes before the end of the match, however, due to the lack of workers, only a few people were employed to open the gates and some had not even reached the doors at the time the stampede took place.

"Because of a lack of workers, only a few people were ordered to open the gates, and they had not yet reached some doors when spectators began rushing to escape tear gas fired by police in an attempt to control fans who had entered the field," Ahmad Riyadh, the spokesperson of Indonesian football association said.

Shocking! Exit gates were closed during deadly stadium stampede, says Indonesia football association

Indonesian President Joko Widodo said Wednesday that he would order an audit of all the football stadiums in the country, vowing to find the "root" cause of one of the deadliest disasters in the sport's history.

Widodo was in the city of Malang to visit relatives of the victims and talk to the wounded at a hospital, and to see the stadium where the stampede killed at least 131 people on Saturday.

"I want to know the root of the problem that caused this tragedy so that we can get the best solution," he said.

"I will order the public works minister to audit all stadiums used for the (football) league," he said outside the Saiful Anwar hospital in Malang, adding he had spoken to FIFA's president the night before about improving Indonesia's "football management".

He entered the hospital to speak with several wounded patients and urged them to "stay spirited". He will travel to the Kanjuruhan stadium, the scene of the disaster on Saturday evening, according to an official from the presidential office.

The Indonesian leader's visit came as anger grew over police officers' response to a pitch invasion after fans of Arema FC tried to approach players following their defeat to fierce rivals Persebaya Surabaya.

Police described the incident as a riot and said two officers were killed, but the survivors accused them of overreacting. Officers responded to the pitch invasion with force, kicking and hitting fans with batons, and firing tear gas shells, according to witnesses and video footage, pushing fans back into the stands where many were trampled or suffocated to death.

In response to the tragedy, Widodo ordered all matches suspended, an investigation into what happened, and compensation for victims. Indonesia's chief security minister said a task force had been created and that the probe would take two to three weeks.

Police said the investigation was focusing on six gates at the stadium using CCTV footage from cameras placed outside them. It said the exits were open but too small for the crowds attempting to pass through them.

But Indonesia's football association spokesperson said Tuesday some gates that should have been opened 10 minutes before the final whistle remained closed. They stayed shut "because of late commands" and officers "had not arrived", he told a press conference.

The Malang police chief was replaced Monday, nine officers were suspended and 19 others were put under investigation over the disaster in the stadium, according to police.

Witnesses described being shrouded in smoke, their eyes stinging as they rushed for small exit doors. Several present said the police stood by and refused to help victims.

"The place looked like a mass cemetery. Women and children were piling on top of one another," Eko Prianto, 39, told AFP.

Indonesia's football association also moved Tuesday to sanction Arema FC, banning its organising committee chairman and a security officer from football for life and fining the club 250 million rupiah ($16,500).

Maike Ira Puspita, the association's deputy secretary-general, told AFP the away fans were banned due to fears of fan violence and said the match passed without incident until fans entered the pitch after the final whistle.

She said the association sanctioned the club and its officials "due to the... negligence of the whole situation". The actions of the police were outside of the association's scope, she said. I want to know the root cause for deadly stampede: Indonesian President to order stadiums' audit

Spectators of the deadly Indonesian football match have said that the police locked some exits as it fired tear gas on the rowdy crowd. This comes as police investigation noted that police wrongly used tear gas in the stadium.

125 killed in stampede as cops use tear gas at Indonesia stadium after football match

Seventeen children were among at least 125 people killed in a soccer stampede in Indonesia at the weekend, officials said, as pressure builds on the Southeast Asian nation to explain how one of the world's worst stadium disasters unfolded.

Violence and hooliganism have long been features of Indonesian football, especially in places such as Jakarta, the capital, but Saturday's disaster in a small town in Java has thrown a spotlight on the problem. "My family and I didn't think it would turn out like this," said Endah Wahyuni, the elder sister of two boys, Ahmad Cahyo, 15, and Muhammad Farel, 14, who died after being caught in the melee. "They loved soccer, but never watched Arema live at Kanjuruhan stadium, this was their first time," she added at her brothers' funeral on Sunday, referring to the home side they backed.

Indonesia's chief security minister Mahfud MD said on Monday the government would form an independent fact-finding team which would include academics and soccer experts as well as government officials to probe what happened. The team will investigate for the next few weeks with the aim of finding who was responsible for the tragedy, he said. Indonesian daily Koran Tempo ran a black front page on Monday, centred on the words "Our Football Tragedy", printed in red along with a list of the dead.

Seventeen children were among the dead, with seven others being treated in hospital said Nahar, an official at the women's empowerment ministry. Saturday's deadly crush came as panicking spectators tried to escape the overpacked stadium after police fired tear gas to disperse fans from the losing home side who ran onto the pitch at the end of the match.

Home side Arema FC had lost the match 3-2 to Persebaya Surabaya, though authorities had said tickets were not issued to Persebaya fans over security concerns. Mahfud said on Sunday the stadium had been filled beyond its capacity. Some 42,000 tickets had been issued for a stadium designed to hold 38,000 people, he said. The incident was a "dark day for all involved", said FIFA, the governing body for world soccer, which has asked Indonesian football authorities for a report on the incident.

Its safety regulations say firearms or "crowd control gas" should not be used at matches. A tearful Arema FC president Gilang Widya Pramana apologised on Monday to the victims of the stampede and said he took full responsibility for the disaster.

"Lives are more precious than soccer," he told a news conference. In an address on Sunday, Pope Francis said he had prayed for those who have lost their lives and for the injured from the disaster. Police and sport officials have been sent to Malang to investigate an incident that ranks among the world's deadliest stadium disasters. "All those responsible should be held accountable for this disaster, regardless of their status or position," Phil Robertson, deputy Asia director of New York-based Human Rights Watch said on Monday.

Indonesia Football Tragedy: 125 killed in stampede at Indonesia football match

At least 32 children died in Indonesia's stadium crush, an official said Monday as police moved to punish those responsible for one of the deadliest disasters in football history.

The tragedy on Saturday night in the city of Malang saw a total of 125 people killed and 323 others injured after officers fired tear gas in a packed stadium to quell a pitch invasion, triggering a stampede.

Dozens of children caught in the chaos lost their lives, an official at the women's empowerment and child protection ministry told AFP.

"From the latest data we received, out of 125 people who died in the accident, 32 of them were children, with the youngest being a toddler age three or four," said Nahar, who like many Indonesians goes by only one name.

As anger mounted against police, Indonesia's chief security minister Mahfud MD announced a task force had been formed to investigate and called for those responsible to be punished.

"We asked (police) to unveil who has perpetrated the crimes and take action against them and we also hope the national police will evaluate their security procedures," he said in a broadcast statement.

The police force sacked its local chief in Malang within hours of the minister's speech.

"Tonight the national police chief has made a decision to relieve Malang police chief Ferli Hidayat from his duty and replace him," national police spokesman Dedi Prasetyo told a press conference.

East Java police also suspended nine officers on the instruction of the national police force, he said, without providing details about their role in the tragedy.

Saturday's incident unfolded when fans of home team Arema FC stormed the pitch at the Kanjuruhan stadium after their loss 3-2 to bitter rivals Persebaya Surabaya.

Police responded by launching tear gas into packed terraces, prompting spectators to rush en masse to small gates where many were trampled or suffocated, according to witnesses.

"It felt like people were packed into a small tube with a tiny hole, and then they were smoked," said 29-year-old spectator Ahmad Rizal Habibi, who escaped before the crush.

Police described the incident as a riot and said two officers were killed, but survivors accuse them of overreacting and causing the deaths of scores of spectators.

"One of our messages is for the authorities to investigate this thoroughly. And we want accountability. Who is to blame?" said 25-year-old Malang resident Andika, who declined to give his last name.

"We want justice for our fallen supporters."

One witness outside the stadium said police refused to help when the crush happened.

"The place looked like a mass cemetery. Women and children were piling on top of one another," Eko Prianto, 39, told AFP.

"I ran to the police or soldier to help. There were no medics in sight. The police did not help and the soldier threatened to beat me."

In a tearful live address, Arema FC president Gilang Widya Pramana apologised for the tragedy.

"I, as the president of Arema FC, will take full responsibility for the incident that occurred," he said.

The Arema squad visited the site of the crush on Monday wearing black shirts to pay their respects and lay flowers before gathering on the pitch to pray for victims

Newspaper Kompas published a black front page with the word "tragedy" and a stadium bearing the names of victims. Graffiti daubed on the walls of the venue revealed bubbling anger towards authorities.

"My siblings were killed. Investigate thoroughly," read one message scrawled on the stadium's shutters, accompanied by a black ribbon and the date of the disaster.

"ACAB", an acronym for "all cops are bastards", was sprayed on another wall.

Indonesian President Joko Widodo ordered compensation for families of the victims to the tune of 50 million rupiah ($3,200) each, a minister said on Monday. He has also suspended football matches until security improves and announced a probe.

Rights groups said officers should be held accountable for using tear gas in a confined area. Mahfud said the task force for the investigation would consist of government and football officials, academics and members of the media.
