Loss of smell or taste has been added to the list of COVID-19 symptoms, according to the revised clinical management protocols released by the Union Health Ministry on Saturday.

The ministry listed muscle pain, diarrhea, congested nasal cavity, sputum production as COVID-19 symptoms, besides fever, cough, fatigue, sore throat and shortness of breath.

People have also complained of loss of smell (anosmia) or loss of taste (ageusia) preceding the onset of respiratory symptoms, it said.

According to an expert, though it is not specific to COVID-19 as one may even suffer from loss of smell and taste when they have flu or influenza, but then it could be one of the early signs of the onset of the disease and may help in timely detection and treatment.

In its revised, 'Clinical Management Protocols for COVID-19 released on Saturday, the health ministry said older people and immune-suppressed patients in particular may present with atypical symptoms such as fatigue, reduced alertness, reduced mobility, diarrhoea, loss of appetite, delirium, and absence of fever, the ministry said.

Children might not have reported fever or cough as frequently as adults.

The US's national public health institute, the Centers for Disease Control and Prevention (CDC), had in early May incorporated "a new loss of taste or smell" in the list of COVID-19 symptoms.

According to data from the Integrated Health Information Platform and Integrated Disease Surveillance Programme, portal case investigation forms for COVID 19 (n=15,366), the details on the signs and symptoms reported are (as on June 11), fever (27 per cent), cough (21 per cent), sore throat (10 per cent), breathlessness (8 per cent), Weakness (7 per cent), running nose (3 per cent ) and others 24 per cent.

According to the health ministry, people infected by the novel coronavirus are the main source of infection.?

Direct person-to-person transmission occurs through close contact, mainly through respiratory droplets that are released when the infected person coughs, sneezes, or talks.

These droplets may also land on surfaces, where the virus remains viable. Infection can also occur if a person touches an infected surface and then touches his or her eyes, nose, or mouth.

The median incubation period is 5.1 days (range 2?14 days). The precise interval during which an individual with COVID-19 is infectious is uncertain.

As per the current evidence, the period of infectivity starts two days prior to onset of symptoms and lasts up to 8 days.
The extent and role played by pre-clinical/ asymptomatic infections in transmission still remain under investigation.

The document while listing the clinical management protocols for treating mild, moderate and severe cases, reiterated that RT-PCR should be used for COVID-19 diagnosis and antibody tests are not recommended for diagnosis of the infection.

Under the case definition category, a patient with acute respiratory illness (fever and at least one sign/symptom of respiratory disease such as cough, shortness of breath), and a history of travel to or residence in a location reporting community transmission of COVID-19 disease during the 14 days prior to symptom onset is considered as a suspect case.

Anyone with any acute respiratory illness and having been in contact with a confirmed or probable COVID-19 case in the last 14 days prior to symptom onset also to be considered as a suspect case.

Besides, a patient with severe acute respiratory illness (fever and at least one? symptom of respiratory disease like cough, shortness of breath; and requiring hospitalization) and in the absence of an alternative diagnosis that fully explains the clinical presentation also will be categorised as a suspect case.

A suspect case for whom testing for the COVID-19 virus is inconclusive or? for whom testing could not be performed for any reason will fall under 'probable case'.

A person with laboratory confirmation of COVID-19 infection, irrespective of clinical signs and symptoms will be considered a confirmed case.

Mild COVID-19 cases can be managed at home may be given symptomatic treatment such as antipyretic (paracetamol) for fever and pain, adequate nutrition and appropriate rehydration.

Hydroxychloroquine tablet may be considered for any of those having high risk features for severe disease (such as age&gt; 60; hypertension, diabetes, chronic lung/kidney/ liver disease, cerebrovascular disease and obesity) under strict medical supervision.

The Ministry of External Affairs (MEA) on Saturday said it has noted Nepal Parliament's Lower House passing a constitutional amendment bill to update the country's new political map, laying claim over the strategically key areas of Lipulekh, Kalapani and Limpiyadhura along the border with India.

MEA spokesperson Anurag Srivastava said that the move is violative of the current understanding to hold talks on outstanding boundary issues, adding the government has already made its position clear on the matter.

India has been maintaining that these three areas belong to it.

A two-thirds majority was required in the 275-member House of Representatives or the lower house to pass the bill.

Major Opposition parties including Nepali Congress (NC), Rastriya Janata Party-Nepal (RJP-N) and Rastriya Prajatantra Party (RPP) voted in favour of the government bill to amend Schedule 3 of the Constitution to update the national emblem by incorporating the new controversial map.

On June 9, the Parliament unanimously endorsed a proposal to consider the Constitution amendment bill to pave way for endorsing the new political map amid the border row with India.

Now, the bill will be sent to the National Assembly where it will undergo a similar process. The ruling Nepal Communist Party commands two-thirds majority in the National Assembly.
The NA will have to give lawmakers 72 hours to move amendments against the bill's provisions, if any.

After the National Assembly passes the bill, it will be submitted to the President for authentication, after which the bill will be incorporated in the Constitution.

India rejects Nepal Parliament's Lower House passing bill to redraw political map

The Health Ministry on Saturday recommended the use of antiviral drug remdesivir in moderate stage of COVID-19 while backtracking from its earlier stance on hydroxychloroquine, saying the anti-malarial drug should be used in the early course of the disease and not on critically ill patients.

It also okayed off-label application of tocilizumab, a drug that modifies the immune system or its functioning, and convalescent plasma for treating coronavirus-infected patients in moderate stage of the illness.

In its revised 'Clinical Management Protocols for COVID-19' the ministry dropped the use of azithromycin in combination with hydroxychloroquine (HCQ) in severe cases and requiring ICU management.

Hydroxychloroquine has demonstrated in-vitro activity against SARS-CoV2 and was shown to be clinically beneficial in several small single-centre studies though with significant limitations, it stated.

"Nonetheless, several large observational studies with severe methodologic limitations have shown no effect on mortality or other clinically meaningful outcomes.

"As such, the evidence base behind its use remains limited as with other drugs and should only be used after shared decision making with the patients while awaiting the results of ongoing studies," the revised document stated.

As is the case with other antivirals, this drug should be used as early in the disease course as possible to achieve any meaningful effects and should be avoided in patients with severe disease, the health ministry said, adding an ECG should ideally be done before prescribing the drug.

Under emergency use authorization, remdesivir may be considered for patients in moderate stage requiring oxygen support. It is not recommended for those with severe renal impairment and high level of liver enzymes, pregnant and lactating women, and those below 12 years, the document stated. ?

The drug, administered in the form of injection, should be given at a dose of 200 mg on day one followed by 100 mg daily for five days.

According to the revised protocols, convalescent plasma therapy may be considered for patients with moderate illness with no improvement in condition despite the use of steroids.

The therapy involves taking antibodies from the blood of a person who has recovered from COVID-19 and transfusing those into a coronavirus patient to help kickstart the immune system to fight the infection. The recipient should be closely monitored for several hours after transfusion for any adverse events and its use should be avoided in patients with immunoglobulin A deficiency or immunoglobulin allergy.

"The dose is variable ranging from 4 to 13 ml/kg -- usually 200 ml single dose given slowly over not less than two hours, the revised document stated.

The guidelines state that tocilizumab, may be considered in patients with moderate illness with progressively increase in oxygen requirement and in mechanically ventilated patients not improving despite the use of steroids.

Long term safety data in COVID 19 remains largely unknown, the guidelines stated.

The presence of raised inflammatory markers should be checked before its use and the patient should be carefully monitored after tocilizumab is administered for secondary infections and neutropenia. Active infections and Tuberculosis should be ruled out before use, it stated.

For patients with progressive deterioration of oxygenation indicators, rapid worsening on imaging and excessive activation of the body's inflammatory response, glucocorticoids can be used for a short period of time.

Larger dose of glucocorticoids, a class of steroid hormones, will delay the removal of coronavirus due to immunosuppressive effects, the revised document stated.

According to the revised clinical management protocols, loss of smell or taste has been added to the list of COVID-19 symptoms.

The ministry said that coronavirus-infected patients reporting to various COVID-19 treatment facilities have been reporting symptoms like fever, cough, fatigue, shortness of breath, expectoration, myalgia, rhinorrhea, sore throat and diarrhoea. They have also complained of loss of smell (anosmia) or loss of taste (ageusia) preceding the onset of respiratory symptoms.

The ministry said the use of remdesivir, tocilizumab and convalescent plasma therapy, at present, is based on limited available evidence. As the situation evolves, and when more data become available, the evidence will be accordingly incorporated, and recommendation upgraded, it stated.

Further, use of these drugs is subject to limited availability in the country as of now, the document stated.
India currently does not manufacture remdesivir ? ?

Four companies -- Hetero, Jubilant Life Sciences, Cipla and Mylan NV -- with which the original drugmaker Gilead Sciences Inc have entered into non-exclusive licensing agreements are still awaiting the nod from the DCGI for manufacturing and selling of remdesivir in India.

The drug regulator on June 1 had granted US pharma giant Gilead Sciences marketing authorisation for its drug remdesivir for "restricted emergency use" on hospitalised COVID-19 in the country.&nbsp;

Delhi High Court orders govt to get more beds and ventilators for COVID-19 patients

Nepal's brinkmanship is becoming more dangerous. Anti-India sentiments have been running high in the country, the prime minister of Nepal the man of mottos is himself fueling these sentiments has repeatedly blamed Indians for bringing the Wuhan virus to his country.

Nepal PM Oli says 85% of coronavirus cases in Nepal came from India. Today we saw the result of such dangerous rhetoric an Indian citizen has lost his life. He was killed by the armed police force of Nepal. This happened at the Lal mandi Janki Nagar border in the Sitamarhi district of Bihar.

Nepalese forces say a group of Indians was forcefully trying to enter their country so they resorted to firing India says this version is being investigated. A day before Nepal's parliament holds a special session to vote on a constitutional amendment for its new map house meeting has been scheduled for Saturday as per the parliament's notice. The constitution amendment bill is on top of the agenda the Nepal govt is busy altering maps in the middle of a pandemic. The youth of Nepal is up in arms against its govt's misplaced priorities there have been largescale protests on Thursday. Thousands hit the streets of Kathmandu for a peaceful demonstration this was the second protest in a week these people are demanding financial transparency.

<div class="videoWrapper trancodeVideo"><iframe allowfullscreen="" autoplay="1" class="includeVideo" frameborder="0" height="350" src="https://www.wionews.com/videos/gravitas-indias-border-bother-pakistan-china-nepal-305491/embed" style="width:100%;max-width:560px;" width="560"></iframe></div>

They are demanding better management of the pandemic Prime Minister KP Oli's attempts at blaming India for the Wuhan virus is not working I want to focus on the border crisis India has repeatedly conveyed its openness to dialogue but Nepal seems to be in a hurry why has a border dispute with a friendly neighbour become so politically charged in Nepal? What are its reasons? What could be the possible factors that escalated this dispute? Is domestic politics the reason.

Kalapani-Lipulekh Border Dispute

India has not only controlled this territory but it has invested in infrastructure projects under its administration on what basis is Nepal laying claim on it then?

Based on a census the Nepal govt conducted there in the 1950s. Kathmandu also refers to the 1815 Sugauli treat to legitimize its claims these facts give way to another question is Nepal acting at China's behest?

China is known to use India's neighbours as proxies young &amp; unstable democracies in political transition are an easy target for the dragon to add to that. India altered its foreign investment laws to hinder hostile takeover by Chinese companies. One cannot rule out how Beijing may have encouraged Nepal to take a bolder stance against India in response to China. After all, has a history of meddling in Nepal's internal affairs the Chinese ambassador is said to have mediated between the warring factions of the ruling Nepal communist party. Beijing has also put pressure on critical reporting by the Nepalese media. The Nepal govt did not even protest we did.&nbsp;

China's political influence has definitely grown in Nepal and India is aware of this.

New Delhi has expressed its displeasure over Nepal's actions. But it has largely remained silent on the issue. The silence is turning out to be a cause of concern.

We haven't seen any significant de-escalatory move by both sides it’s been more than a month. The sooner India settles this dispute the lesser chances of Nepal bowing down to the dragons will.

Kalapani-Lipulekh Border Dispute India has not only controlled this territory but it has invested in infrastructure projects under its administration on what basis is Nepal laying claim on it then? Based on a census the Nepal govt conducted there in the 1950s. Kathmandu also refers to the 1815 Sugauli treat to legitimize its claims these facts give way to another question is Nepal acting at China's behest? China is known to use India's neighbours as proxies young & unstable democracies in political transition are an easy target for the dragon to add to that. India altered its foreign investment laws to hinder hostile takeover by Chinese companies. One cannot rule out how Beijing may have encouraged Nepal to take a bolder stance against India in response to China. After all, has a history of meddling in Nepal's internal affairs the Chinese ambassador is said to have mediated between the warring factions of the ruling Nepal communist party. Beijing has also put pressure on critical reporting by the Nepalese media. The Nepal govt did not even protest we did.
