The case fatality rate of Indian coronavirus has been reduced to 1.86 per cent — one of the lowest globally, the Union health ministry said on Sunday.

The constantly rising number of recoveries has ensured that the active cases of coronavirus infection, which is the “actual caseload” of the country, currently comprises only 23.24 per cent of the total cases.

The ministry said the average daily number of recoveries is on a "constant upward move" from 15,018 between July 1 and 7 to 60,557 between August 13 and 19.

Meanwhile, India's COVID-19 tally zoomed past 29 lakh, a day after it crossed 28 lakh, with a single-day spike of 68,898 infections, while the recoveries have increased to 21, 58,946 on Friday pushing the recovery rate to over 74 per cent, according to the Union Health Ministry data.

The total cases of coronavirus infections mounted to 29, 05,823, while the death toll climbed to 54,849 with 983 new fatalities being reported in a span of 24 hours, the data updated at 8 am showed.

Maharashtra's covid-19 case count grows by 10,441; death count by 258

At least 30 people died and a dozen are missing after a ferry capsized and sank Monday in the Bangladeshi capital Dhaka following a collision with another vessel, rescue officials said.

The Morning Bird vessel was hit from behind by another ferry around 9:30 am local time (0330 GMT) during the morning rush hour, when the country's largest river port is packed with vessels.

"We have collected 30 bodies, including 20 males, seven women and three children," Abul Khair, a diver in the fire brigade, told AFP.

"There were at least 50 people on board... Our rescue divers are still searching," coastguard spokesman commander Hayet Ibne Siddique said.

The ferry -- which departed from central Munshiganj district -- sank as it was about to moor at Sadarghat, Dhaka's main river port used by hundreds of boats to travel to the country's south.

Bangladesh Inland Water Transport Authority's chief, Commodore Golam Sadeqk, told AFP the single-deck ship was "not overcrowded" and sank "due to carelessness".

He said the vessel had been cleared to carry passengers until September.

Witnesses told local television stations many passengers appeared to be stuck in the ferry's cabins.

Divers were still pulling bodies from the wreck, in waters some 40-50 feet (12-15 metres) deep. The deceased were put in body-bags before they were laid in rows at the harbour-front.

Another boat would later arrive to lift the damaged vessel from the water, Siddique said.

Botswana is investigating the mysterious deaths of at least 154 elephants over two months in the northwest of the country, a wildlife official said on Monday, although poaching or poisoning have been ruled out.

"We are still awaiting results on the exact cause of death,” Regional Wildlife Coordinator Dimakatso Ntshebe told Reuters.

The carcasses were found intact, suggesting they were not poached. Further investigations have also ruled out poisoning by humans and anthrax, which sometimes hits wildlife in this part of Botswana.

Africa's overall elephant population is declining due to poaching, but Botswana, home to almost a third of the continent’s elephants, has seen numbers grow to 130,000 from 80,000 in the late 1990s, owing to well managed reserves.

However, they are seen as a growing nuisance by farmers, whose crops have been destroyed by elephants roaming the southern African country.

President Mokgweetsi Masisi last year lifted a five-year ban on big game hunting, imposed by previous president Ian Khama, but the hunting season failed to take off in April as global travel restrictions meant hunters from many coronavirus-hit countries could not enter Botswana.

Meanwhile, the Wildlife Department has undertaken an operation to relocate and dehorn all rhinos to tackle poaching in Botswana - mirroring efforts elsewhere in the region.

The Okavango Delta rhino population has been the hardest hit, with 25 reported poached between December and the beginning of May, government figures show, as poachers take advantage of the absence of safari tourists during the pandemic.

That compares with a total of 31 rhinos poached from October 2018 to December last year.

"Both white rhino and black rhinos have been severely affected, necessitating the ... relocation of highly endangered black rhinos (and) intensification of surveillance," the Department said.

A French court on Wednesday ordered Rwandan genocide suspect Felicien Kabuga be handed over to a United Nations tribunal for trial.

UN prosecutors accuse Kabuga of bankrolling and arming ethnic Hutu militias that killed 800,000 Tutsis and moderate Hutus in Rwanda during a 100-day period in 1994. He is indicted for genocide and incitement to commit genocide, among other charges.

India has reported its biggest single-day jump in the number of coronavirus cases. Over the last 24 hours, India has recorded close to 4,000 confirmed cases, taking the total over 46,000 cases.

The surge in cases is concerning, as India had just begun to ease the lockdown restrictions. Over the last one week -- the daily number of new cases in India had been on a steep rise.

However -- today's surge is unprecedented. India reported close to 4,000 cases, which is the highest single-day rise since the first case was detected on January 30.

With the unexpected spike today, India has recorded one of its worst weeks since the pandemic broke out.

It took India 75 days to record its first 10,000 cases from January 30 to April 14.

The addition of every 10,000 cases since then has been steep. India took eight days to breach the 20,000 mark and seven days to hit the 30,000 marks.

But, the rise from 30,000 to 40,000 cases has occurred over a period of just four days. Despite the steep rise in cases, the Health Ministry today shared an optimistic number. At the moment -- the number of cases in India are doubling once every 12 days.

Before the enforcement of lockdown on March 25 -- cases were doubling once every three days.

In terms of the death toll, India reported 194 deaths in the last 24 hours. That's the first time India has recorded over 100 deaths in a single day. A day ago -- the death toll stood at 83.

Now -- India's total death toll stands at 1,583. On the positive side, over 13,000 people have been cured or discharged. Today, the Ministry of Health said India's recovery rate stands at 27.41 per cent. That's an improvement in the recovery rate of 25 per cent reported a day earlier.

India's testing rate has improved too. Over the last 24 hours -- India conducted more than 80,000 tests. That accounts for seven per cent of all the testing done in India. So far, India has tested over one million people.

And, the Indian council of medical research has said that India could very soon start testing over 100,000 people per day.

So, the results have been mixed for India since the third phase of the lockdown began on May 4. The lockdown has been extended for two weeks. Containment zones remain sealed. But, some relaxations are allowed in other areas.

Also, 62 trains have transported around 70,000 migrant workers, who could be potential carriers of the deadly virus to rural India.

Experts are concerned, they say India's lockdown may have helped flatten the curve, but the curve is not yet showing a downward trend.

So, it has sparked a debate over the easing of restrictions. As incidents of open disregard for social distancing norms have been reported across the country.

India experiences biggest ever spike in cases and deaths with 46,000 cases and 1,583 deaths

One cold night in October, Gerel Ganbaatar decided to stay with her parents in one of Mongolia's traditional ger communities on the outskirts of the capital -- a decision which would prove fatal.

Within hours of arriving, she and her parents both became breathless and nauseous. They desperately called for help, but by the time medics arrived Gerel, who was four months pregnant, was already dead.

The 29-year-old's parents were using smokeless briquettes to warm their home for the first time that night: A government-issued fuel, made of coking coal from the southern Gobi region and washed coal powder, it has been linked to deaths and ill-health.

Since residents started using the briquettes in October, there have been seven other deaths, mainly children, pregnant women, and the elderly. Nearly one thousand have been hospitalised in Ulaanbaatar's outer slum districts, prompting fears the fuel can cause carbon monoxide poisoning.

Gerel's grieving mother, Jargalsaikhan Mishigdorj, warns the fuel should be used with care.

"I have burned coal all of my life, we have never ever been suffocated," she explains.

The Mongolian government is trying to battle dangerous smog levels in one of the world's most polluted cities by trying to offer replacements for raw coal, which produces a dense, dangerous smoke, but is often used by the poor to warm their homes in a place where temperature can plummet to minus 40C.

Mongolian officials say the briquettes last longer and emit fewer fumes.

Toxic air

One nurse at the National Poison Emergency hospital, who declined to say her name, told AFP she had never seen the numbers of carbon monoxide poisoning patients so high.

"We are working under an enormous amount of pressure," she says.

The city's emergency unit said issues were easing once people were told the "correct ways to burn the briquettes".

They said public service workers were spending time in the ger district to teach people about the new fuel as well as to report if any raw coal was still being used.

Its the latest example of governments in Asia struggling to find a safe, workable solution to replacing coal, in a region that is battling toxic levels of air pollution.

In India, Prime Minister Narendra Modi's promise to provide cooking gas connections to millions of rural residents has been beset by corruption.

In neighbouring China, three million homes near Beijing were ordered to switch from coal to gas or electric heating in 2017, but a difficult transition left many without any heating at all.

Ulaanbaatar is the world's coldest capital and only half of the residents have central heating, and Ulaanbaatar regularly exceeds World Health Organisation recommendations for air quality.

The government has been trying to force low-grade coal out of commission to reduce the thick air pollution that has plagued the city.

And experts say there is evidence that the move is already having a positive impact on the environment.

Sonomdagva, associate professor at the National University of Mongolia, told AFP that PM 2.5 -- tiny particles that get into the lungs and bloodstream -- had decreased by 40 per cent compared with October last year.

"However, an incredibly long and warm autumn, plus the usage of the new refined fuel, and fear of suffocation meaning people use only wood (for fuel) can also be reasons of the 40 per cent decrease," he said.

Fear of suffocation

Byambajargal Losol, a physicist at the Mongolian Science Academy, told AFP that he believes the smokeless briquettes are the best way to reduce air pollution -- but that more needs to be done to help their safe use.

"People are using briquettes just like the old raw coal and burning it the same way," he said, adding: "The briquettes are thick and compact so they require twice as much oxygen to burn, compared to raw coal."

Carbon monoxide poisoning happens when there is a restricted oxygen supply.

The state-owned factory that produces the briquettes said families should also clean their stoves before burning the new fuel.

It has started providing free stove repair services after issues were highlighted in the local press. The government has also promised to distribute free smoke detectors in ger districts.

While citizens say the briquettes have benefits, some remain wary.

Ger resident Turbold Sainbuyan told AFP that the new briquettes save time and last longer.

"I had to break up the raw coal, (but) the briquettes are ready to use," he says.

"Now we use one bag of briquettes for three days compared to one bag of raw coal for only one day," he adds.

His mother Enkhjargal Chuluun, a nurse at a private dental clinic, said the briquettes are warmer than raw coal.

She says: "But we still have fear of suffocation, so we started opening the window at night."

"However, an incredibly long and warm autumn, plus the usage of the new refined fuel, and fear of suffocation meaning people use only wood (for fuel) can also be reasons of the 40 per cent decrease," he said. Fear of suffocation Byambajargal Losol, a physicist at the Mongolian Science Academy, told AFP that he believes the smokeless briquettes are the best way to reduce air pollution -- but that more needs to be done to help their safe use. "People are using briquettes just like the old raw coal and burning it the same way," he said, adding: "The briquettes are thick and compact so they require twice as much oxygen to burn, compared to raw coal." Carbon monoxide poisoning happens when there is a restricted oxygen supply.

Under pressure after a deadly season on traffic-clogged Mount Everest, Nepal is considering tightening access to the world's highest peak, but mountaineering experts fear the proposed changes could amount to little more than lip service.

Eleven people died during the climbing season that ended this week, as record numbers lined the route to the summit. Although overcrowding was blamed for at least four deaths, many say inexperience is a bigger killer.

"People who know nothing of climbing, never been on a mountain, came and tried to climb Everest," Chilean mountaineer Juan Pablo Mohr told AFP after returning to Kathmandu.

"A lot of people didn't know how to put (on) crampons or (use) the fixed ropes," he said, adding they relied on an army of sherpas or Nepali guides to help them accomplish such basic tasks.

For years, Kathmandu has issued permits to anyone willing to pay $11,000, regardless of whether they are rookie climbers or skilled mountaineers.

But after a devastating spring season, officials told AFP they are considering imposing more restrictions.

"We are looking into having a minimum requirement for climbers, fixing more ropes or taking more oxygen and sherpas," said Mohan Krishna Sapkota, secretary at Nepal's tourism ministry.

For veteran mountaineers, the announcement of new rules amounts to little more than a futile annual exercise -- with the government each year promising tougher measures that fail to materialise by the following spring.

'Just stupid words'

Russell Brice, whose company Himalayan Experience (Himex) has been organising Everest expeditions for decades, told AFP his meetings with government officials over the years had left him in no doubt about their indifference towards the industry.

"The ministry is in denial of overcrowding, of issuing too many permits, not checking what people are doing and so on," Brice said.

"It is just stupid words," he said, adding, "they are not interested."

In 2014, Kathmandu said it would double the number of fixed ropes near the summit to prevent traffic jams.

It never happened.

The government also said it would station soldiers and police at Everest base camp for assistance. An AFP visit to the area in 2015 revealed no such deployment.

Unlike Tibet, which caps the number of Everest climbers at 300, there are no limits on the Nepali side, making for an exceedingly profitable -- and dangerous -- business.

As cheaper operators have entered the fray, the number of climbers has shot up, creating deadly bottlenecks en-route to the top of the 8,848-metre (29,029-feet) peak -- especially when bad weather cuts the number of summit days, as it did this year.

Phurba Tenjing Sherpa, who has guided clients on Everest for over a decade, told AFP one of the climbers who died should never have been allowed to go up, given her excessively slow pace.

But the woman, who was in her fifties, "would not listen" because she had paid the fee and wanted to see the summit, he said.

"Such climbers are increasing on Everest. They force us, they want to climb no matter what. Their stubbornness is killing people on the mountain," he said.

Rush to profits

The situation has deteriorated to such a degree that Nepal's Mountaineering Association, which represents operators, is lobbying the government for stricter rules.

The association's president, Santa Bir Lama, told AFP the high number of deaths this year was due to the increased presence of inexperienced climbers.

"Climbers should be self-reliant. It would be best if there is a criteria set for issuing permits," he said.

For years after Edmund Hillary and Tenzing Norgay made the first ascent of Everest in 1953, access to the world's most famous peak was a rare prize, offered only to solo mountaineers charting new routes or to climbers belonging to national teams or boasting major sponsorship deals.

But after Nepal scrapped its limit in the 1990s, operators have crowded the slopes for a slice of the multi-million dollar industry.

This year, the Himalayan nation made around $4 million from Everest permits alone.

"It is nice to see this income," Himex's Brice said.

But he warned the rush to reap profits was "detracting from the major attraction of Nepal, which is Mount Everest".

"If we ruin that business, it's going to have long-term effects".

Nepal may limit access to Everest after mountaineering deaths

An Indian mountaineer who was forced to turn back before summiting Mount Everest said on Monday (May 27) that the lack of trained guides and delays on the crowded route up the 8,850-metre (29,035 feet) summit were key factors in the spate of deaths on the mountain.

Desperate workers leaped to their deaths as a huge fire tore through a Dhaka office block Thursday, killing at least 19 people and trapping others in the latest major inferno to hit the Bangladesh capital.

Rescue workers warned the death toll could rise sharply as fire fighters recovered charred bodies from the complex where an unknown number of office workers were engulfed by intense smoke and flames.

At least six people died after jumping from the 22-floor building, officials said.

Dhaka police chief Asaduzzaman Mia told reporters at least 73 people were injured and being treated in hospitals across Dhaka.

People were seen screaming for help as hundreds of panicked onlookers crowded the streets of the upmarket Banani commercial district.

Some workers slid down a television cable on the side of the building. Others grabbed ropes lowered by emergency service helicopters which pulled them out of the blaze.

The inferno erupted barely a month after at least 70 people were killed in Dhaka apartment buildings where illegally stored chemicals exploded.

The latest disaster brought new scenes of horror amid fears that the toll would rise. More than 100 ambulances were parked in streets around the building.

Shoikot Rahman heard colleagues raise the alarm and ran to safety before smoke and flames engulfed the building.

"When I heard a fire broke out in the building, I quickly rushed out," he told AFP. "Many of my colleagues are still trapped in the office."

Firefighters on long ladders smashed windows to create escape routes. More than one hour after the blaze erupted people could still be seen on the 13th and 14th floors desperately waving for help amid clouds of black smoke.

Army helicopters dangled ropes that victims grabbed so they could be lifted to safety, with crowds below cheering and applauding every time someone was rescued.

Rescuers kept at bay

Three hospitals reported that six men and women had died or arrived with fatal injuries after jumping from the office block. They included a Sri Lankan man whose body was taken to the army's Kurmitola Hospital.

Dilkhosh Ahmed at the Banani Clinic said one of the victims had attempted to use the television cable to climb down, but slipped and fell around the eighth floor.

A seventh death from burns was recorded at the Dhaka Medical College hospital.

Helicopters were deployed to drop water on the blaze as scores of firefighters backed by navy and air force specialists struggled to bring it under control.

A top fire official said the flames had been stopped from spreading to adjoining buildings.

"Teams have entered the building and they are scouring the floors for any remaining victims. The building did not have fire fighting equipment," said Lieutenant Colonel Julfikar Rahman of the Dhaka fire service told reporters.

Rescue crews were soon discovering bodies and carrying them out one after the other in white bags.

Some workers told of risky escapes.

"My uncle and two more people jumped from their floor. His hand and leg are broken and his eye is damaged," one man said without giving his name.

A man who gave his name as Jico said he had been working on the 19th floor. "The fire started in a restaurant on the sixth floor. We ran to the roof as soon as we heard about it and then used a wooden plank to get over to the next building."

Fire disasters regularly hit Bangladesh's major cities where safety standards are notoriously lax.

Fire service officials said chemicals illegally stored in an apartment building exploded and set alight five buildings and nearby streets. That blaze took more than 12 hours to control.

A June 2010 fire in the nearby neighbourhood of Nimtoli, one of the most densely populated districts of the capital, killed 123 people.

In November 2012, a fire swept through a nine-story garment factory near Dhaka killing 111 workers. An investigation found it was caused by sabotage and that managers at the plant had prevented victims from escaping.

Experts said inspections of buildings in the city frequently found fire stairs blocked with stored goods and exit doors locked.

"No Oseltamivir (medicine) and testing is required for category A patients. They should confine themselves at home and avoid mixing up with public and high risk members in the family," the advisory said.

In addition to all the signs and symptoms mentioned under category A, if a patient has high-grade fever and severe sore throat, he or she may require home isolation and Oseltamivir.

In addition to all the signs and symptoms mentioned under category A, individuals having one or more of the high-risk conditions shall be treated with Oseltamivir, it added.

High-risk group includes, children with mild illness but with predisposing risk factors, pregnant women, persons aged 65 years or older, patients with lung diseases, heart disease, liver disease, kidney disease, blood disorders, diabetes, neurological disorders, cancer and HIV/AIDS, or those on long-term cortisone therapy, the advisory said.

In addition to the signs and symptoms of categories A and B, if the patient has one or more danger signs, like breathlessness, chest pain, drowsiness, fall in blood pressure, sputum mixed with blood, bluish discolouration of nails, then these patients (under category C) require testing, immediate hospitalisation and treatment, it added.

After holding a state-level review meeting on Infuenza A (H1N1), the Delhi government recently said that all government hospitals in the city are equipped with necessary logistics required for the management of the disease and drug Oseltamivir along with personal protective equipment (PPE kits) and N95 masks are also available.

The advisory also asked people to avoid crowded places in the swine flu season and stay more than an arm's length distance away from persons sick with flu.

According to a data issued by the government, 2,572 swine flu cases have been reported across the country with Rajasthan recording 1,508 cases and 56 deaths this year till Thursday.

It is followed by Gujarat with 438 cases and eight deaths and Delhi reporting the third highest number with 387 people being infected with the virus. No death has been reported due to the flu in the national capital so far.

Haryana reported 272 cases till Thursday.

With the numbers rising, the health ministry recently held a meeting with the states and asked them to strengthen their surveillance for early case detection along with reserving beds in the hospital to deal with acute cases.

"On the operational side, we have asked the states to reserve some beds in hospitals as in acute cases patients have to be put on a ventilator. They have also been asked to issue advisories for people to take precautions. Also, there is sufficient stock of swine flu drug Oseltamivir and also there is no scarcity of N95 masks," a senior health ministry official said.

Meanwhile, the Delhi government has issued an advisory mentioning Dos and Don'ts for the people.

The advisory states that seasonal influenza (H1N1) is a self-limiting viral, airborne disease spread from person-to-person, through large droplets generated by the act of coughing and sneezing, indirect contact by touching a contaminated object or surface (fomite transmission like telephone, cell phones, computers, door handles, doorbells, pens, toys etc) and close contact, including handshaking, hugging and kissing.

The Do's and Don't mention covering nose and mouth with disposable tissue or handkerchief while coughing or sneezing, washing hands with soap and water frequently, avoiding crowding places, maintaining isolation, drinking plenty of liquid and consulting doctor.

As per the advisory, one should not touch eyes, nose or mouth with unwashed hands or hug, kiss and shake hands while greeting or spit in public places, dispose of the used napkin or tissue paper in open areas, smoke in public places.

The death toll from landslides and devastating floods in the central Philippines triggered by a tropical depression climbed to 85, officials said on Wednesday, and 20 people were missing as rescuers slowly reached cut-off communities.

The casualties, including young children, were mostly killed when their homes collapsed in landslides after days of heavy rain in several provinces in the central Philippines, said Ricardo Jalad, executive director of the national disaster agency.

"If we don't recover the missing or we recover them dead, that is 105 deaths, which we hope not," Jalad said.

The tropical depression, which weakened into a low-pressure system before leaving the Philippines on Sunday, brought heavy rain that triggered landslides and flooding in the Bicol and Eastern Visayas regions.

Officials put three provinces under a "state of calamity" to give them access to emergency funds.

Bicol, with a population of 5.8 million, was the hardest hit, with 68 killed in intense rains and landslides. Damage to agriculture in Bicol, which produces rice and corn, was estimated at 342 million pesos ($6.5 million).

Rescuers, including the police and military, used heavy-lifting equipment to clear roads leading to landslide sites and entered flooded communities using rubber boats.

"The sun is already out, with occasional light rains. We hope floods will subside," Ronna Monzon, a member of the operations personnel at the disaster agency in Bicol, told Reuters.

About 20 tropical cyclones hit the Philippines every year, with destroyed crops and infrastructure taking a toll on human lives and weighing down one of the fastest growing economies in Asia.

At least two people were killed and 11 wounded on Tuesday by a car bomb in the northern Iraqi city of Tal Afar, a former Islamic State stronghold, the military said.

Tal Afar, about 80 kilometres (50 miles) west of Mosul, experienced cycles of sectarian violence between Sunnis and Shi'ites after the US-led invasion of Iraq in 2003 and produced some of Islamic State's most senior commanders.

The city, which had about 200,000 residents, came under the militants' control when Islamic State overran swathes of Iraq's north in 2014.

It fell to Iraqi Security Forces, backed by the US-led coalition, in August 2017, the last area to be retaken in the north before fighting moved to the Syrian border. A heavy security presence has remained since then and the city has been mostly quiet.

Indonesian pop group 'Seventeen' singer to bury wife killed in tsunami

Environment ministers from about 100 countries gather in Nairobi next week to issue a clarion call against air, land and water pollution blamed for some nine million deaths in 2015.

Ministers under the UN Environment Assembly (UNEA) will finalise a global declaration vowing to combat "rampant pollution" as a growing threat to human life, economies and ecosystems.

Seven million of those deaths are caused by car exhaust, factory fumes and indoor cooking, according to the UN Environment Programme (UNEP), which hosts the assembly.

Water pollution is the second deadliest type, followed by exposure to toxins and carcinogens in the workplace.

Last month, UNEP said approximately one in six deaths in 2015 were due to pollution, mainly in poor and developing countries.

Almost half of the nine-million death toll came from just two countries - rapidly-industrialising India and China.

The December 4-6 meeting will be the third UNEA, created by world leaders in 2012 to strengthen world efforts to save the environment.

The assembly is the highest-level decision-making forum on environmental issues. All 193 UN states are members.

Nearly 2,500 participants have registered for this year's gathering, including a host of company CEOs, the heads of other UN agencies, scientists and activists.

The ministers will negotiate the wording for a number of resolutions on specific anti-pollution steps, such as limiting the amount of fish-choking plastic that finds its way into the ocean, and banning the use of toxic lead in paint.

"Our oceans are filling with trash so fast that by 2050 there will be more plastic in the seas than fish," according to a UNEP statement.

"A global ban on lead in paint is one of several resolutions under discussion at UNEA and, if agreed by ministers, will be a key step in removing an environmental hazard that harms the mental development of 600,000 children every year," it added.

Other resolutions, still in the draft phase, will aim for stricter air quality control in cities, and to reduce environmental degradation caused by armed conflict.

Pollution, according to the UNEP, is "the biggest human killer bar none."

"We need to beat pollution before it beats us," said UNEP head Erik Solheim.

"That means being able to breathe in our cities, keeping dangerous chemicals out of the food chain, and stopping our oceans from being transformed into a plastic soup."

The assembly must yield a political declaration by Wednesday, entitled "Towards a Pollution-Free Planet".

It will commit UN member countries to limiting humankind's soiling of the planet with chemicals, non-biodegradable litter and toxic smoke.

"Nine million people die prematurely every year because of environmental intoxication - this is clearly a moral scandal," said Joachim Schellnhuber, director of the Potsdam Institute for Climate Impact Research.

Aside from outright poisoning, pollution causes an array of deadly ailments such as heart disease, stroke, lung cancer and chronic obstructive pulmonary disease.

Spain's seaside city of Barcelona on Thursday unveiled a large digital counter that will track the number of refugees who die in the Mediterranean, next to one of its popular beaches.

"We are inaugurating this shame counter, which will update all known victims who drowned in the Mediterranean in real time," said Mayor Ada Colau. The monument consists of a large metal rectangular pillar that comes decked out with a digital counter above the inscription "This isn't just a number, these are people".

The counter kicked off with "3,034", the number of migrants and refugees who have died trying to cross the Mediterranean to Europe in 2016, according to the International Organization for Migration (IOM).

"We're here to look the Mediterranean in the face and look at this number, 3,034 people who drowned because they were not offered a safe passage," said Colau, as swimmers took advantage of the last rays of sunshine nearby.

The fatality rate this year is higher than in the same period in 2015, the IOM said this week. One reason for the higher death toll this year is that there have been a series of shipwrecks involving larger vessels carrying several hundred people rather than the more typical dinghies that carry a maximum of 100 migrants.

Syrians fleeing the country's brutal war again made up the largest group to have risked their lives to make the dangerous sea crossing so far this year. Afghans were next on the list, followed by Iraqis.

A heroin "epidemic" is gripping the United States, where cheap supply has helped push the number of users to a 20-year high, increasing drug-related deaths, the United Nations said today.

According to the U.N.'s World Drug Report 2016, the number of heroin users in the United States reached around one million in 2014, almost three times as many as in 2003. Heroin-related deaths there have increased five-fold since 2000.

"There is really a huge epidemic (of) heroin in the U.S.," said Angela Me, the chief researcher for the report which released today.

"It is the highest definitely in the last 20 years," Me said, adding that the trend was continuing.

The rise could be linked to U.S. legislation introduced in recent years which makes it harder to abuse prescription opioids such as oxicodone, a powerful painkiller that can have similar effects to heroin, Me said.

The law meant the texture of the pills was changed to make it more difficult to crush them and inject them into the blood stream, Me said.

"This has caused a partial shift from the misuse of these prescription opioids to heroin."

Another reason for the increase in the use of heroin, which in the United States mainly comes from Mexico and Colombia, is greater supply that has depressed prices in recent years, Me said.

The United States has also seen a spike in deaths related to fentanyl, a synthetic opioid 50 times stronger than heroin and 100 times more so than morphine, according to the U.S. Centers for Disease Control and Prevention.

Fentanyl has been named as the drug that killed pop singer Prince this year.

At least 207,000 deaths globally were drug-related in 2014, with heroin use and overdose-related deaths increasing sharply also over the last two years, according to the Vienna-based U.N. Office on Drugs and Crime (UNODC).

"Heroin continues to be the drug that kills the most people and this resurgence must be addressed urgently," Yury Fedotov, the executive director of the UNODC, said.

U.S. President Barack Obama earlier this year asked Congress for $1.1 billion in new funding over two years to expand treatment for users of heroin and prescription painkillers.
