England`s top-flight is in the midst of its busiest time of the season, with teams involved in the League Cup playing at least twice a week until the first week of January, and further cancellations could leave it facing an unwelcome fixture pile-up. 
 
United`s game against Brentford was the second game to be postponed this season due to a COVID-19 outbreak after Tottenham Hotspur`s game at Brighton &amp; Hove Albion over the weekend was called off. 
 
From Wednesday, fans will also need to show proof of double vaccination under England's "plan B" of measures to slow the spread of the Omicron variant that are being voted on by legislators on Tuesday. 
 
Clubs have been enjoying full stadiums again this season after playing for months in front of empty or restricted capacity stands from the start of the pandemic in March 2020.

Gerrard, whose side also have had players and staff test positive this week, told British media on Monday their thorough recruitment process meant vaccine status could be considered when they speak to potential recruits. 
 
"We look at everything. So I`m sure it will come up," Gerrard said. 
 
"But we`ll obviously make decisions as and when we decide a certain person is the one who could come in and make us better," he said. "But I don`t think it`s right for me to comment on if I would or wouldn`t in that situation. But it would certainly come into conversations in the background." 
 
Villa, who are 13th, travel to bottom side Norwich City in the league later on Tuesday.

The national medical body of Australia has legally recognised two made in India COVID-19 vaccines (Covaxin and Covishield), which are used across the world but not in Australia, opening the way for Indian tourists in the coming months.&nbsp;

Previously, the Therapeutic Goods Administration (TGA) suggested that only Australian-approved vaccinations, as well as Covishield from India and Sinovac from China, be recognised for travel and other limitations.&nbsp;

It has now added two more to the list — Covaxin, manufactured in India and Sinopharm, which is made in China.

"Today, the TGA determined that Covaxin (manufactured by Bharat Biotech, India) and BBIBP-CorV (manufactured by Sinopharm, China) vaccines would be 'recognised' for the purpose of establishing a traveller's vaccination status. This recognition is for travellers aged 12 and over who have been vaccinated with Covaxin, and those 18 to 60 who have been vaccinated with BBIBP-CorV," the TGA said in a statement.

"Importantly, recognition of Covaxin, and BBIBP-CorV, along with the previously announced recognition of Coronavac (manufactured by Sinovac, China) and Covishield (manufactured by AstraZeneca, India), means many citizens of China and India as well as other countries in our region where these vaccines have been widely deployed will now be considered fully vaccinated on entry to Australia," it added.

Across the developing world, hundreds of millions of people are unable to get a vaccine to protect themselves from the ravages of COVID-19, and millions of them have already become infected and died.

Depending on wealthy nations to donate billions of doses is not working, public health experts say. The solution, many now believe, is for the countries to do something that the big U.S. mRNA vaccine makers say is not feasible: manufacture the gold-standard mRNA shots themselves.

Despite mounting pressure, the CEOs of Moderna and Pfizer have declined to license their mRNA technology in developing countries, arguing it makes no sense to do so. They say that the process is too complex, that it would be too time- and labor-intensive to establish centers that could do it, and that they cannot spare the staff because of the urgent need to maximize production at their own network of facilities.

“You cannot go hire people who know how to make mRNA; those people don’t exist,” the CEO of Moderna, Stéphane Bancel, told analysts.

But public health experts in both rich and poor countries argue that expanding production to the regions most in need is not only possible but essential for safeguarding the world against dangerous variants of the virus and ending the pandemic.

Setting up mRNA manufacturing operations in other countries should start immediately, said Tom Frieden, the former director of the Centers for Disease Control and Prevention in the United States, adding, “They are our insurance policy against variants and production failure” and “absolutely can be produced in a variety of settings.”

The vaccine needs of poorer countries were supposed to be met through COVAX, a multinational body meant to facilitate global vaccine distribution — but donations have been slow and limited. Wealthier countries have locked up supply. Just 4% of people in low-income countries are fully vaccinated.

Experts in both the development and production of vaccines say the mRNA vaccines involve fewer steps, fewer ingredients and less physical capacity than traditional vaccines. Companies in Africa, South America and parts of Asia already have much of what they would need to make them, they say; the technology specific to the mRNA production process can be delivered as a ready-to-use modular kit.

Most estimates put the cost of setting up production at $100 million to $200 million. A few large pharmaceutical producers in developing countries have these funds at hand; others would need loans or investors. The U.S. International Development Finance Corp. and the International Finance Corp. both have billions of dollars in funding available for this kind of project, as low-interest loans or a share of equity.

The New York Times interviewed dozens of executives and scientists at vaccine, drug and biotechnology companies across the developing world and from those conversations found 10 strong candidates to produce mRNA COVID vaccines in six countries on three continents. The key criteria include existing facilities, human capital, the regulatory system for medicines and the political and economic climate.

The candidates include companies that are already making other COVID vaccines, such as the Serum Institute of India, the world’s largest vaccine maker; public institutions that are already testing their own mRNA vaccines for the coronavirus; and firms tapped by the World Health Organization to be regional centers for mRNA development.

Two companies in Asia are already making their own mRNA vaccines against COVID.

Gennova Biopharmaceuticals in Pune, India, has one in phase 2 and 3 clinical trials. Gennova says that unlike the mRNA shots currently in use, its vaccine can be stored at the temperature of a standard medical refrigerator.

Gennova is headed by Sanjay Singh, a biochemist who worked on malaria vaccines at the National Institutes of Health in the United States for six years before returning to India. The company is negotiating with contract manufacturers to make its vaccine while also working to expand its existing production capacity from 100 million to 1 billion doses a year, and it could be in production with its COVID shot within months, Singh said.

BioNet-Asia, a Thai drugmaker, is producing test batches of a COVID mRNA vaccine developed at Chula Vaccine Research Center in Bangkok that is in phase 2 trials.

If results continue to be positive, the vaccine could go to Thailand’s drug regulator by March, and BioNet would be ready for commercial production on approval, said Kiat Ruxrungtham, who heads the research team making the ChulaCov19 vaccine.

“Having this capability and capacity of this technology platform within the country — the goal is, when you have a new variant spreading or you have the next pandemic, you can start things very quickly instead of waiting to buy vaccines like we have been doing so far,” Ruxrungtham said.

Other drug companies would like to license one of the existing mRNA vaccines — pay a fee to receive the formulation and instructions, then share a royalty on each dose they sell — and start making it as quickly as possible. Stephen Saad, CEO of Aspen Pharmacare in Durban, South Africa, said that with an investment he estimated at $100 million, his firm could be producing 1 billion doses of mRNA vaccine within a year — more than enough to supply all of Africa, across which Aspen already has a distribution network.

Bio-Manguinhos, the immunobiology arm of a venerated Brazilian public health research organization, will soon start clinical trials of an RNA-based COVID vaccine, said Sotiris Missailidis, deputy director of technology development for the research center.

This year, Bio-Manguinhos nearly doubled its production capacity — to 215 million doses — of other vaccines, including AstraZeneca’s COVID shot, which it produces under contract. Brazil has a medical regulatory agency that maintains the same standards as the U.S. Food and Drug Administration and the European Medicines Agency.

A range of factors has restricted access to vaccines in developing countries, including supply chain and shipping bottlenecks, and politics. The Serum Institute was supposed to supply COVAX, but India’s government banned exports at the height of that country’s second wave. Aspen in South Africa won a contract to bottle the Johnson &amp;amp; Johnson vaccine, but it had to export many of the shots right back to Europe and to Canada until activists created a public outcry.

“What we’ve learned through this pandemic is that it really does matter where the doses are coming off the production line,” said Andrea Taylor, who tracks vaccine production for the Duke Global Health Innovation Center.

Making mRNA vaccine shots differs in significant ways from traditional vaccine production. It is an enzymatic process, not a biological one that involves live cells, and in many ways is closer to the work of making drugs, said Zoltan Kis, a chemical engineer who analyzed mRNA production capacity for the Future Vaccine Manufacturing Research Hub at Imperial College in London.

In fact, when BioNTech was ready to start production of its vaccine, it went not to a vaccine maker but to a cancer drug plant in Germany. Moderna’s contract manufacturer in Switzerland employed former food scientists from Nestle, enlisted to transfer their chemistry skills.

“It’s a game changer because you don’t have to deal with the same stakeholders anymore,” said Alain Alsalhani, a vaccines expert with Doctors Without Borders’ access-to-medicines campaign.

Moderna’s contract manufacturers use a modular production kit that can make 100 million doses; some in the business compare the concept to an Ikea kitchen.

The COVID mRNA vaccines have already earned more in a single year than any previous product in pharmaceutical history and are on track to bring in more than $53 billion in revenue this year alone. The longer Pfizer and Moderna have a proprietary lock on the technology to make them, the greater the edge they will have on any future vaccine for cancer or other diseases, said Zain Rizvi, an expert on access to medicines with the advocacy organization Public Citizen. When a company has a functioning production line, it is a straightforward process to swap the mRNA content and make vaccines for a different pathogen, such as malaria or HIV.

This argument over manufacturing COVID mRNA vaccines echoes one that was made two decades ago about treatments for HIV. Hundreds of thousands of people died of AIDS in Africa long after antiretroviral drugs were widely available in wealthy nations because the patented medications were being sold at a price far too high for governments in the worst-affected countries to purchase.

Treatment access advocates staged a global campaign demanding that the drugmakers license low-cost producers or release the rights to their intellectual property to allow someone to fill the gap.

Although major Western pharmaceutical companies insisted there was no way to make the drugs more cheaply, Indian, Brazilian, Thai and South African drugmakers said they could do it. Indian generic companies reverse-engineered many of the formulas, and today, the bulk of the world’s AIDS drugs are made in these countries.

Now the WHO has taken on a similar challenge.

Because efforts to win licensing deals or other cooperation from Pfizer and Moderna have been unsuccessful to date, the organization is backing an effort to reverse-engineer Moderna’s vaccine at a technology transfer hub in South Africa, said Martin Friede, who runs the Initiative for Vaccine Research at the WHO. Biotechnology company Afrigen Biologics will make the mRNA, and the Biovac Institute will manufacture the vaccines.

Patrick Tippoo, head scientist at Biovac, which has a contract to bottle the Pfizer COVID vaccine, said the institute would prefer to own the technology to make an mRNA vaccine. But the fastest route to production would be a partnership with the maker of one of the existing mRNA vaccines. If Biovac had access to the “recipe,” instruction from people who have made the vaccine, and were to purchase modular production suites, the company could make the vaccines in 12-18 months, he said.

Instead of sharing its recipe, Moderna announced this month that it would spend up to $500 million to build its own vaccine plant in Africa. (The company did not specify which of the 54 countries it planned to build in or how long it would take.) And BioNTech, the inventor of Pfizer’s mRNA process, has announced plans to build plants in Africa in the next four years.

The Western pharmaceutical industry and some supply chain and health experts say the fastest route to closing the COVID vaccine gap is to focus on a more equitable distribution of vaccines made by the existing players.

“No enforced technology transfer would do more to address equity than what’s already lined up,” said Thomas Cueni, director of the International Federation of Pharmaceutical Manufacturers and Associations, an industry lobby group in Geneva.

Intent on spreading production across the developing world, the WHO is also working on a second track that will seek partnerships with institutions such as Ruxrungtham’s research center in Bangkok, with their own mRNA vaccines. These vaccines would potentially be cheaper to produce and, critically, could be heat-stable — not requiring ultracold storage — and thus far more appropriate for use in low-resource settings, Friede said.

Some of the efforts are producing innovations that could benefit their regions far beyond the current pandemic.

Gennova in India has worked with a Seattle startup called HDT Bio to develop a new method to deliver mRNA with a lipid nanoparticle, one that does not require extreme cold storage. “We wanted to solve the problem of the scalability issue and the temperature issue,” Singh, CEO of Gennova, said. “If we can solve these problems, we are building a solution not just for India but also a global solution.”

Gennova received seed funding from the government of India, and it plans to use internal resources — and a potential partnership it is negotiating with a large multilateral agency — to take its vaccine forward, Singh said.

While Gennova has the technology, other producers such as the Brazilian and Argentine vaccine makers would need infrastructure — such as the kits Moderna used to set up production with its contractor in Switzerland.

For the producers not yet working with mRNA, the fastest way to start making COVID shots would be a partnership with Pfizer or Moderna, but the technology transfer process would involve hundreds of steps, said Prashant Yadav, a supply chain expert with the Center for Global Development.

“Can you do it for mRNA just by sending the transfer document blueprints and having a few Zoom calls, or a team to visit for a few days? In most cases, probably not,” he said. An experienced team that stayed on site for a period of time would be crucial.

Yadav estimated it would take one of the producers in South Africa or Brazil up to 18 months to have an mRNA vaccine ready to go in arms, with a recipe-share. There would also need to be a parallel process of boosting the capacity of national regulatory agencies, which will be critical to maintaining quality.

Kis said that any manufacturer that had a site at the standard set by the U.S. Food and Drug Administration-governed system of best practice for pharmaceutical manufacturing could switch to mRNA production in six months, using the pop-up production kits.

“We will build facilities in these regions to enable them to respond to this current pandemic — or maybe the pandemic will be over — but this time, the world has seen what the cost of not being prepared is, so hopefully this time, we’ve got the political will,” Friede of the WHO said. “Richer countries are going to have to put their hands in their pockets and contribute to the construction and maintenance of these facilities. Because in a pandemic, you’re not safe until your neighbor is safe.”

If results continue to be positive, the vaccine could go to Thailand’s drug regulator by March, and BioNet would be ready for commercial production on approval, said Kiat Ruxrungtham, who heads the research team making the ChulaCov19 vaccine. “Having this capability and capacity of this technology platform within the country — the goal is, when you have a new variant spreading or you have the next pandemic, you can start things very quickly instead of waiting to buy vaccines like we have been doing so far,” Ruxrungtham said. Other drug companies would like to license one of the existing mRNA vaccines — pay a fee to receive the formulation and instructions, then share a royalty on each dose they sell — and start making it as quickly as possible. Stephen Saad, CEO of Aspen Pharmacare in Durban, South Africa, said that with an investment he estimated at $100 million, his firm could be producing 1 billion doses of mRNA vaccine within a year — more than enough to supply all of Africa, across which Aspen already has a distribution network. Bio-Manguinhos, the immunobiology arm of a venerated Brazilian public health research organization, will soon start clinical trials of an RNA-based COVID vaccine, said Sotiris Missailidis, deputy director of technology development for the research center. This year, Bio-Manguinhos nearly doubled its production capacity — to 215 million doses — of other vaccines, including AstraZeneca’s COVID shot, which it produces under contract. Brazil has a medical regulatory agency that maintains the same standards as the U.S. Food and Drug Administration and the European Medicines Agency. A range of factors has restricted access to vaccines in developing countries, including supply chain and shipping bottlenecks, and politics. The Serum Institute was supposed to supply COVAX, but India’s government banned exports at the height of that country’s second wave. Aspen in South Africa won a contract to bottle the Johnson &amp; Johnson vaccine, but it had to export many of the shots right back to Europe and to Canada until activists created a public outcry. “What we’ve learned through this pandemic is that it really does matter where the doses are coming off the production line,” said Andrea Taylor, who tracks vaccine production for the Duke Global Health Innovation Center. Making mRNA vaccine shots differs in significant ways from traditional vaccine production. It is an enzymatic process, not a biological one that involves live cells, and in many ways is closer to the work of making drugs, said Zoltan Kis, a chemical engineer who analyzed mRNA production capacity for the Future Vaccine Manufacturing Research Hub at Imperial College in London. In fact, when BioNTech was ready to start production of its vaccine, it went not to a vaccine maker but to a cancer drug plant in Germany. Moderna’s contract manufacturer in Switzerland employed former food scientists from Nestle, enlisted to transfer their chemistry skills. “It’s a game changer because you don’t have to deal with the same stakeholders anymore,” said Alain Alsalhani, a vaccines expert with Doctors Without Borders’ access-to-medicines campaign. Moderna’s contract manufacturers use a modular production kit that can make 100 million doses; some in the business compare the concept to an Ikea kitchen. The COVID mRNA vaccines have already earned more in a single year than any previous product in pharmaceutical history and are on track to bring in more than $53 billion in revenue this year alone. The longer Pfizer and Moderna have a proprietary lock on the technology to make them, the greater the edge they will have on any future vaccine for cancer or other diseases, said Zain Rizvi, an expert on access to medicines with the advocacy organization Public Citizen. When a company has a functioning production line, it is a straightforward process to swap the mRNA content and make vaccines for a different pathogen, such as malaria or HIV. This argument over manufacturing COVID mRNA vaccines echoes one that was made two decades ago about treatments for HIV. Hundreds of thousands of people died of AIDS in Africa long after antiretroviral drugs were widely available in wealthy nations because the patented medications were being sold at a price far too high for governments in the worst-affected countries to purchase. Treatment access advocates staged a global campaign demanding that the drugmakers license low-cost producers or release the rights to their intellectual property to allow someone to fill the gap. Although major Western pharmaceutical companies insisted there was no way to make the drugs more cheaply, Indian, Brazilian, Thai and South African drugmakers said they could do it. Indian generic companies reverse-engineered many of the formulas, and today, the bulk of the world’s AIDS drugs are made in these countries. Now the WHO has taken on a similar challenge. Because efforts to win licensing deals or other cooperation from Pfizer and Moderna have been unsuccessful to date, the organization is backing an effort to reverse-engineer Moderna’s vaccine at a technology transfer hub in South Africa, said Martin Friede, who runs the Initiative for Vaccine Research at the WHO. Biotechnology company Afrigen Biologics will make the mRNA, and the Biovac Institute will manufacture the vaccines. Patrick Tippoo, head scientist at Biovac, which has a contract to bottle the Pfizer COVID vaccine, said the institute would prefer to own the technology to make an mRNA vaccine. But the fastest route to production would be a partnership with the maker of one of the existing mRNA vaccines. If Biovac had access to the “recipe,” instruction from people who have made the vaccine, and were to purchase modular production suites, the company could make the vaccines in 12-18 months, he said. Instead of sharing its recipe, Moderna announced this month that it would spend up to $500 million to build its own vaccine plant in Africa. (The company did not specify which of the 54 countries it planned to build in or how long it would take.) And BioNTech, the inventor of Pfizer’s mRNA process, has announced plans to build plants in Africa in the next four years. The Western pharmaceutical industry and some supply chain and health experts say the fastest route to closing the COVID vaccine gap is to focus on a more equitable distribution of vaccines made by the existing players. “No enforced technology transfer would do more to address equity than what’s already lined up,” said Thomas Cueni, director of the International Federation of Pharmaceutical Manufacturers and Associations, an industry lobby group in Geneva. Intent on spreading production across the developing world, the WHO is also working on a second track that will seek partnerships with institutions such as Ruxrungtham’s research center in Bangkok, with their own mRNA vaccines. These vaccines would potentially be cheaper to produce and, critically, could be heat-stable — not requiring ultracold storage — and thus far more appropriate for use in low-resource settings, Friede said. Some of the efforts are producing innovations that could benefit their regions far beyond the current pandemic. Gennova in India has worked with a Seattle startup called HDT Bio to develop a new method to deliver mRNA with a lipid nanoparticle, one that does not require extreme cold storage. “We wanted to solve the problem of the scalability issue and the temperature issue,” Singh, CEO of Gennova, said. “If we can solve these problems, we are building a solution not just for India but also a global solution.” Gennova received seed funding from the government of India, and it plans to use internal resources — and a potential partnership it is negotiating with a large multilateral agency — to take its vaccine forward, Singh said. While Gennova has the technology, other producers such as the Brazilian and Argentine vaccine makers would need infrastructure — such as the kits Moderna used to set up production with its contractor in Switzerland. For the producers not yet working with mRNA, the fastest way to start making COVID shots would be a partnership with Pfizer or Moderna, but the technology transfer process would involve hundreds of steps, said Prashant Yadav, a supply chain expert with the Center for Global Development. “Can you do it for mRNA just by sending the transfer document blueprints and having a few Zoom calls, or a team to visit for a few days? In most cases, probably not,” he said. An experienced team that stayed on site for a period of time would be crucial. Yadav estimated it would take one of the producers in South Africa or Brazil up to 18 months to have an mRNA vaccine ready to go in arms, with a recipe-share. There would also need to be a parallel process of boosting the capacity of national regulatory agencies, which will be critical to maintaining quality. Kis said that any manufacturer that had a site at the standard set by the U.S. Food and Drug Administration-governed system of best practice for pharmaceutical manufacturing could switch to mRNA production in six months, using the pop-up production kits. “We will build facilities in these regions to enable them to respond to this current pandemic — or maybe the pandemic will be over — but this time, the world has seen what the cost of not being prepared is, so hopefully this time, we’ve got the political will,” Friede of the WHO said. “Richer countries are going to have to put their hands in their pockets and contribute to the construction and maintenance of these facilities. Because in a pandemic, you’re not safe until your neighbor is safe.”

“Once you open up, more social interactions will happen,” he said. “And given the inherently highly transmissible nature of the delta variant, you will get big clusters emerging.”

The vaccines have worked to keep most of the population out of the hospital, with 98.3 per cent of cases presenting mild or no symptoms. In Buffalo, New York, the Erie County Medical Center plans to suspend elective inpatient surgeries and not take intensive-care patients from other hospitals because it may soon fire about 400 employees who have chosen not to get vaccinated against the coronavirus.

Officials at Northwell Health, New York’s largest provider of health care, estimate that they might have to fire thousands of people who have refused to get vaccinated.

And while the vast majority of staff members at New York City’s largest private hospital network, NewYork-Presbyterian, had been vaccinated as of this week, more than 200 employees faced termination because they had not.

These are just a fraction of the workers at risk of losing their jobs or being put on unpaid leave after Monday, when a state directive requiring hospital and nursing home employees in the state to have received at least one shot of a virus vaccine takes effect.

As of Wednesday, state data shows, around 84% of New York’s 450,000 hospital workers and 83% of its 145,400 nursing home employees had been fully vaccinated. But tens of thousands of people are estimated not to have gotten a shot despite being threatened with losing their jobs. The holdouts say they fear potential side effects from the vaccines, have natural immunity or believe that the mandate violates their personal freedom.

On Thursday, Gov. Kathy Hochul said that the Monday deadline was firm and that her administration was developing emergency plans to cover for those who are laid off, going so far as to look into recruiting temporary workers from the Philippines or Ireland.

“What is looming for Monday is completely avoidable, and there’s no excuses,” Hochul said, pleading for those who have not done so to get vaccinated.

In New York City, health care workers are not the only ones who face an imminent deadline to be vaccinated or face termination. A requirement that virtually everyone working in the public schools — well over 150,000 people — be vaccinated takes effect at midnight Monday. Although about 90% of the system’s educators and 80% of support staff members have gotten at least one shot, thousands of workers have not. That could cause staff shortages in some schools.

Educators who choose not to get vaccinated will be allowed an unpaid one-year leave with their health insurance intact. But they will not be allowed to enter school buildings starting Tuesday. Unions that represent educators and other school staff members are warning of disruptions for students and have urged Mayor Bill de Blasio to delay enforcement of the mandate.

New York state’s vaccination requirement for health care workers is among the largest mandates of its kind that is set to take effect in the United States, with weekly virus testing not permitted as a substitute measure. How it goes — and whether it leaves hospitals understaffed — will be closely watched. California is requiring health care workers there to be fully vaccinated by Sept. 30, and a similar mandate in Maine will not be enforced until Oct. 29. New York’s willingness to risk large-scale layoffs of health care workers comes amid a national nursing shortage, and the requirement is facing numerous legal challenges.

“We give patients a Bill of Rights, and they are able to choose what procedures or tests or medications they want to put in their system,” said Gregory Serafin, a registered nurse at the Erie County Medical Center and the lead plaintiff in a New York lawsuit that seeks to stop the mandate. “Health care workers deserve the same medical autonomy to make those decisions.”

Depending on how many health care workers are fired, the policy could also test the resiliency of New York’s health care system. Hospitals across the state are activating emergency staffing plans that they typically reserve for natural disasters or, more recently, surges in COVID-19 cases. Volunteers, students and retirees will fill vacancies, along with traveling nurses.

Northwell, which has 77,000 employees, believes it can weather any loss of employees without the care of patients being affected. The Erie County Medical Center is not as sure.

On Monday, the hospital had 553 inpatients, its busiest day on record. A big reason the hospital is so crowded is that it cannot discharge as many patients as usual to nursing or group homes, because they are also limiting admissions in anticipation of their own staff shortages because of the vaccine mandate.

“This is creating an unprecedented crisis for us,” said Tom Quatroche, the Erie County Medical Center Corp.’s president. “I think we need more time to comply, and I’ve asked for that. For all the right reasons, the vaccine mandate was put in place. But the reality is it is creating a public health crisis in hospitals, with nobody to care for patients.”

In New York City, more than 5,000 of the 42,000 employees of the public hospital system were unvaccinated as of Friday. They will be barred from hospitals starting Monday and from other care facilities beginning Oct. 7, and they will be placed on unpaid leave.

The hospital system anticipates the vaccine mandate could reduce the ranks of radiology technologists and phlebotomists, in particular, and some doctors have been urged to limit the amount of imaging and blood work they order next week, according to an internal message.

Firings under the new directive could prove particularly problematic for nursing homes, which are already facing staffing problems. The New York State Health Facilities Association, a trade group that represents about 250 nursing homes, has asked state officials to temporarily let unvaccinated nursing home workers keep working as long as they get tested regularly.

“While we are striving for 100%, we do not feel we will achieve that by Monday,” said Stephen Hanse, the association’s president.

The wider range of workers who must be vaccinated starting Oct. 7 includes those at diagnostic and treatment centers, adult care facilities, certified home health agencies and hospices.

Despite the potential staffing challenges, the Greater New York Hospital Association, which represents about 140 health systems and 55 nursing homes, supports the deadline.

“The mandate is the best way to ensure the safest possible patient care environment and protect the public’s health,” Kenneth E. Raske, the association’s president, said in a statement. “We will work with the state to address any staffing challenges that arise.”

The vaccination mandate for health care workers was issued Aug. 26 as an emergency order by the Health Department. It permits limited medical exemptions, which employers can determine, but does not currently allow religious exemptions.

The lack of religious exemptions has created some confusion, because an earlier order, issued Aug. 18 under former Gov. Andrew Cuomo, allowed such exemptions, and some hospitals had granted them. NewYork-Presbyterian, for example, granted 129 religious exemptions but withdrew them after the state’s policy changed, court documents show.

The conflict over religious exemptions has prompted several lawsuits accusing New York of violating either the First Amendment or federal law prohibiting employers from discriminating on the basis of religion. Many of the suits’ plaintiffs argue that they do not want to get a COVID-19 vaccine because cell lines derived from fetuses aborted decades ago were used in the development or production or testing of the vaccines.

In legal papers, most of the plaintiffs, most of whom are identified as John or Jane Doe, insist that as opponents of abortion they must abstain from getting vaccinated against COVID-19.

One plaintiff in a suit filed in Brooklyn, a NewYork-Presbyterian employee, is described in court papers as a Christian Scientist who opposes all vaccines and believes in the healing power of prayer.

For now, the fate of unvaccinated health care workers who have sought religious exemptions remains uncertain, and will not be decided for at least a few weeks. A federal judge in Utica has issued a temporary restraining order preventing the state from enforcing the mandate on those seeking such exemptions until Oct. 12, when he will decide what to do.

In the meantime, the mandate can take effect for all other workers.

At Northwell, the vaccination rate reached 91% on Friday, rising steadily as the deadline approached. Of the employees who still were not vaccinated, about 2,000 had applied for religious or medical exemptions.

Only a small group of people are being approved for medical exemptions, including those who are allergic to vaccine components, Maxine Carrington, the company’s human resources chief, said. Being pregnant or breastfeeding does not qualify. Unvaccinated employees without exemptions that have been approved or are pending have until 11:59 p.m. Monday to get a shot or they will be let go Tuesday, she said.

NewYork-Presbyterian set an earlier deadline for its workers to get vaccinated — Sept. 22 — and officials there said Thursday that fewer than 250 employees would lose their jobs. Still hundreds of employees remain unvaccinated: Around 200 got medical exemptions; more than 500 had pending requests for religious exemptions, according to a court filing.

The state’s emergency order does not dictate exactly how hospitals or other health care employers should enforce the mandate, stating only that compliance is expected by Sept. 27.

Vaccination rates vary among hospital staffs.

John R. Oishei Children’s Hospital in Erie County says that 100% of its employees have been fully vaccinated. At Maimonides Medical Center in Brooklyn, 75% of the staff is fully vaccinated, according to state data; nearly 90% have received at least one dose, a spokeswoman said.

Some hospital executives said they did not anticipate that any terminations would have much effect on medical care at their institutions.

At St. Barnabas Hospital in the Bronx, about 12% of the nearly 3,000 employees had not been vaccinated as of midday Friday, the chief medical officer, Eric Appelbaum, said in an interview.

The group included three doctors and a larger number of nurses, Appelbaum said. Most of the rest worked in nonmedical roles, including housekeeping and food services.

Appelbaum, noting that there had been a sharp uptick in vaccinations in the past 48 hours, said he was optimistic that a good number of unvaccinated staff members would soon get the shot. (There was a similar rush at the city’s public hospital system, officials said, with more than 2,000 employees getting vaccinated just this week.)

“Obviously,” Appelbaum said, “Monday’s getting close.”

Hospitals across the state are activating emergency staffing plans that they typically reserve for natural disasters or, more recently, surges in COVID-19 cases. Volunteers, students and retirees will fill vacancies, along with traveling nurses. Northwell, which has 77,000 employees, believes it can weather any loss of employees without the care of patients being affected. The Erie County Medical Center is not as sure. On Monday, the hospital had 553 inpatients, its busiest day on record. A big reason the hospital is so crowded is that it cannot discharge as many patients as usual to nursing or group homes, because they are also limiting admissions in anticipation of their own staff shortages because of the vaccine mandate. “This is creating an unprecedented crisis for us,” said Tom Quatroche, the Erie County Medical Center Corp.’s president. “I think we need more time to comply, and I’ve asked for that. For all the right reasons, the vaccine mandate was put in place. But the reality is it is creating a public health crisis in hospitals, with nobody to care for patients.” In New York City, more than 5,000 of the 42,000 employees of the public hospital system were unvaccinated as of Friday. They will be barred from hospitals starting Monday and from other care facilities beginning Oct. 7, and they will be placed on unpaid leave. The conflict over religious exemptions has prompted several lawsuits accusing New York of violating either the First Amendment or federal law prohibiting employers from discriminating on the basis of religion. Many of the suits’ plaintiffs argue that they do not want to get a COVID-19 vaccine because cell lines derived from fetuses aborted decades ago were used in the development or production or testing of the vaccines. In legal papers, most of the plaintiffs, most of whom are identified as John or Jane Doe, insist that as opponents of abortion they must abstain from getting vaccinated against COVID-19. One plaintiff in a suit filed in Brooklyn, a NewYork-Presbyterian employee, is described in court papers as a Christian Scientist who opposes all vaccines and believes in the healing power of prayer. For now, the fate of unvaccinated health care workers who have sought religious exemptions remains uncertain, and will not be decided for at least a few weeks. A federal judge in Utica has issued a temporary restraining order preventing the state from enforcing the mandate on those seeking such exemptions until Oct. 12, when he will decide what to do. In the meantime, the mandate can take effect for all other workers. At Northwell, the vaccination rate reached 91% on Friday, rising steadily as the deadline approached. Of the employees who still were not vaccinated, about 2,000 had applied for religious or medical exemptions. Only a small group of people are being approved for medical exemptions, including those who are allergic to vaccine components, Maxine Carrington, the company’s human resources chief, said. Being pregnant or breastfeeding does not qualify. Unvaccinated employees without exemptions that have been approved or are pending have until 11:59 p.m. Monday to get a shot or they will be let go Tuesday, she said. NewYork-Presbyterian set an earlier deadline for its workers to get vaccinated — Sept. 22 — and officials there said Thursday that fewer than 250 employees would lose their jobs. Still hundreds of employees remain unvaccinated: Around 200 got medical exemptions; more than 500 had pending requests for religious exemptions, according to a court filing. The state’s emergency order does not dictate exactly how hospitals or other health care employers should enforce the mandate, stating only that compliance is expected by Sept. 27. Vaccination rates vary among hospital staffs. John R. Oishei Children’s Hospital in Erie County says that 100% of its employees have been fully vaccinated. At Maimonides Medical Center in Brooklyn, 75% of the staff is fully vaccinated, according to state data; nearly 90% have received at least one dose, a spokeswoman said. Some hospital executives said they did not anticipate that any terminations would have much effect on medical care at their institutions. At St. Barnabas Hospital in the Bronx, about 12% of the nearly 3,000 employees had not been vaccinated as of midday Friday, the chief medical officer, Eric Appelbaum, said in an interview. Tanzania will spend $470 million buying vaccines and supporting economic sectors hit hard by the coronavirus, President Samia Suluhu Hassan said on Monday. 
 
Since Hassan took office after the death of then-president John Magufuli in March, the government has changed tack from playing down the pandemic to calling for social distancing and emphasising mask wearing in public. 
 
Issuing the first data on infections since May 2020, Hassan said there were more than 100 COVID-19 patients in Tanzania as of last Saturday, with 70 of them being provided oxygen. 
 
Half of the cash will be spent on vaccines, protective gear and other medical equipment, Hassan said, with the rest going to stimulate sectors that are reeling from the crisis. 
 
She did not give details about the sectors but tourism, one of the top foreign exchange earners, has been one of the worst hit. 
 
"There are many organisations which are ready to support us," she told Tanzanian news editors. 
 
There is currently no coronavirus vaccination programme in Tanzania, but the country has applied to join the World Health Organization-backed COVAX vaccine-sharing project. 
 
The government is working with healthcare partners to develop a plan to distribute vaccines. 
 
"Any Tanzanian who wants to be inoculated will be inoculated," Hassan said.

India has overtaken the US in the total number of COVID-19 vaccine doses administered so far, the Union Health Ministry said on Monday.&nbsp;&nbsp;

While India launched the COVID-19 vaccination drive on January 16, and has administered over 323.6&nbsp;million&nbsp;doses till date, the US which started its inoculation drive on December 14, 2020 has given over 323.3&nbsp;million&nbsp;doses.&nbsp;&nbsp;

"India achieves another milestone in COVID-19 vaccination and overtakes USA in total number of COVID-19 vaccine doses administered," the ministry said.&nbsp;&nbsp;

India's cumulative vaccination coverage exceeded 323.6&nbsp;million&nbsp;on Sunday. A total of 32,36,63,297 vaccine doses have been administered through 43,21,898 sessions, as per the provisional report till 7 am on Monday, the ministry said.&nbsp;&nbsp;

It said 17,21,268 vaccine doses were administered in the last 24 hours.&nbsp;&nbsp;

According to the data, so far 1,01,98,257 healthcare workers have taken the first dose while 72,07,617 have taken the second dose.&nbsp;&nbsp;

As many as 1,74,42,767 frontline workers have received the first dose while 93,99,319 have taken the second&nbsp;dose.&nbsp;In&nbsp;the 18-44 age group, 8,46,51,696 individuals have taken then first dose, while 19,01,190 have received the second dose.&nbsp;&nbsp;

In the 45-59 age group, 8,71,11,445 people have been administered the first dose, while 1,48,12,349 have taken the second dose.&nbsp;&nbsp;

Besides, 6,75,29,713 people aged above 60 years have received the first dose, while, 2,34,08,944 have taken the second dose.&nbsp;&nbsp;

The new phase of&nbsp;universalisation&nbsp;of COVID-19 vaccination commenced from June 21.

The situation in Africa, where new infections and deaths jumped by nearly 40% last week compared to the previous week, is 'so dangerous' as the Delta variant spreads globally, WHO director-general Tedros Adhanom Ghebreyesus said. 
 
"Our world is failing, as the global community, we are failing," he told a news conference. 
 
Tedros, who is Ethiopian, chastised unnamed countries for reluctance to share doses with low-income countries. He compared it to the HIV/AIDS crisis, when some argued that African nations were unable to use complicated treatments. 
 
"I mean that attitude has to be a thing of the past," Tedros said. "The problem now is a supply problem, just give us the vaccines." 
 
"The difference is between the haves and the have nots,&nbsp;which is now completely exposing the unfairness of our world - the injustice, the inequality, let's face it," he said. 
 
Many developing countries are much better than industrialised countries in carrying out mass vaccination of their populations against infectious diseases from cholera to polio, WHO's top emergency expert Mike Ryan said. 
 
"The level of paternalism, the level of colonial mindset that say 'we can't give you something because we're afraid you won't use it'. I mean seriously, in the middle of a pandemic?" 
 
COVAX, run jointly by the GAVI vaccine alliance and the WHO, has delivered 90 million COVID-19 vaccine doses to 132 countries since February, but has faced major supply issues since India suspended vaccine exports. 
 
"We have through COVAX this month zero doses of AstraZeneca vaccines, zero doses of SII vaccines (Serum Institute of India), zero doses of J &amp; J (Johnson &amp; Johnson) vaccine," said Bruce Aylward, WHO senior adviser. 
 
"The situation right now is dire."
