The Mumbai civic body has ordered a high-level probe into the death of a woman who fell into a stormwater drain during heavy rainfall in Andheri East, Mumbai. The tragic incident happened on Wednesday at 9:20 pm, the police said.

The 45-year-old woman, identified as Vimal Anil Gaikwad, was stuck in an open drain and drowned. She was taken to the Cooper Hospital after the local police and fire department were alerted. According to the authorities, she was declared brought dead to the hospital.

The Brihanmumbai Municipal Corporation (BMC) announced on Thursday, September 26, that a probe is ordered into the incident.&nbsp;

“BMC Commissioner Bhushan Gagrani orders a high-level inquiry into the incident where a woman died after falling into an open drainage. The report to be submitted within 3 days,” said BMC.

“This incident was unfortunate. I have ordered a probe. The responsibility will be fixed,” Gagrani told Hindustan Times.

A committee of three has been formed to look into the incident, which will be headed by deputy municipal commissioner (zone 3) Devidas Kshirsagar. Mumbai Fire Brigade Chief Ravindra Ambulgekar and Chief Engineer Avinash Tambewagh are also part of the committee.

“There was a cross culvert and Gaikwad fell in this. We recovered her body 50 feet away from where she fell. We are collecting evidence,” a civic officer said, reported Hindustan Times.

The India Meteorological Department has issued an orange alert for Mumbai and Thane as heavy downpours lash the city. A red alert has been issued for parts of Palghar also.

The authorities announced the closure of schools as train services have been affected on Thursday. At least four people have lost their lives due to rain-related incidents in Mumbai and nearby regions.

A 104-year-old woman from Chicago was on the cusp of making history. She had recently performed a daring skydive in a bid to make her name etched into the Guinness World Records, making her the oldest-ever woman to perform such a feat.

But before her work could get certified, her fate had other plans.

On Monday morning (Oct 9), Dorothy Hoffner was found dead by staff at Brookdale Lake View—a senior living community.

Hoffner had apparently died in her sleep on Sunday night, her close friend Joe Conant told AP news agency on Tuesday (Oct 10).

"She was indefatigable. She just kept going," he said, adding, "she was not someone who would take naps in the afternoon, or not show up for any function, dinner or anything else. She was always there, fully present. She kept going, always."

On October 1, Hoffner jumped off from a plane from 13,500 feet at Skydive Chicago in Ottawa, Illinois, 85 miles southwest of Chicago.

<h2>'Never wanted to break the record'</h2>

Apparently, this was not the first time she was skydiving. At that time, she was merely 100 years of age.

But her intention was not to break a record, Conant asserted.

Hoffner had so thoroughly enjoyed her first jump that she just wanted to do it again.

"She had no intention of breaking the record. And she had no interest in any publicity or anything. She wasn't doing it for any other reason than she wanted to go skydiving," he was quoted as saying.

<h2>'Working to ensure her name is entered posthumously'&nbsp;</h2>

Conant said he was working through paperwork to ensure that Guinness World Records certifies Hoffner posthumously as the world's oldest skydiver, but he expects that will take some time.

The current record was set in May 2022 by 103-year-old Linnéa Ingegärd Larsson of Sweden.

Skydive Chicago and the United States Parachute Association celebrated Hoffner in a joint statement Tuesday.

A 23-year-old woman in Nepal was&nbsp;found dead in an isolated hut on Monday&nbsp;where she was left alone as part of a long-banned ancient Hindu practice that banishes women from the home during menstruation, a media report said.

Many communities in Nepal view menstruating women as impure and in some remote areas they are forced to sleep in a hut away from home during their periods, a custom is known as &quot;Chhaupadi&quot;.

Police said that Gauri Bayak (Budha) was found dead by her neighbours inside the Chhaupadi hut (menstruation hut) yesterday at Turmakhad Rural Municipality-3 in Achham district, The Kathmandu Post reported.

&quot;The cause of her death will be known after post-mortem,&quot; Deputy Superintendent of Police Dadhiram Neupane said.

Villagers suspect that Gauri might have died of smoke suffocation from the fire she had lit to keep herself warm inside the hut.
