Thailand: The Asian country records its first case of mpox. The person travelled from an African country on August 14.

Philippines: The country reported its first case of mpox virus, according to its health department on Monday, August 19. The case reported is a 33-year-old Filipino man with no travel history.&nbsp;

Pakistan: The health department of Pakistan confirmed that three cases of mpox were detected in the Khyber Pakhtunkhwa province on Friday, August 16. The patients had arrived from the United Arab Emirates (UAE), after which the infection was detected.

Sweden: The first case of the new strain of mpox virus recorded outside of Africa was confirmed by the Swedish authorities on Thursday, August 15. The person got infected during a visit to Africa, where a major outbreak has been reported.

Countries in Europe and Asia are on high alert, gearing up their vigilance and screening at entry points, along with preparing for potential outbreaks.

Mpox crisis: Argentina ship quarantined; Thailand records first case

Thailand reported a suspected first case of the new, more dangerous strain of Mpox on Wednesday (Aug 21), which the World Health Organization (WHO) has declared a global public health emergency.

The suspected patient landed in Bangkok on August 14 and was sent to hospital with Mpox symptoms the following morning, said Thongchai Keeratihattayakorn, head of the kingdom's Department of Disease Control.

However laboratory tests are underway to confirm the strain, but officials believe it to be from Clade 1. A 66-year-old European, who travelled to Thailand from an African country, has been quarantined for now and is under observation.

It may be mentioned here that Mpox cases and deaths are surging in Africa, where outbreaks have been reported in the Democratic Republic of Congo, Burundi, Kenya, Rwanda and Uganda since July.

The disease, caused by a virus transmitted from infected animals and spread between humans through close physical contact, results in fever, muscle aches, and large boil-like skin lesions.

While mpox has been known for decades, a new more deadly and more transmissible strain -- known as Clade 1b -- has driven the recent surge in cases.&nbsp;

Amid worries caused by the rise in mpox cases in the Democratic Republic of Congo and the spread to nearby countries, the WHO hastily convened a meeting of experts to study the outbreak.

A Public Health Emergency is the highest level of alarm under the International Health Regulations, which are legally binding on 196 countries.

"The detection and rapid spread of a new clade of mpox in eastern DRC, its detection in neighbouring countries that had not previously reported mpox, and the potential for further spread within Africa and beyond is very worrying," said Tedros.

"It's clear that a coordinated international response is essential to stop these outbreaks and save lives.

"This is something that should concern us all."

WHO's decision comes after the African Union's health watchdog declared its own public health emergency over the growing outbreak.

Tedros said the more than 14,000 cases and 524 deaths reported so far this year in DR Congo had already exceeded last year's total.

"The emergence last year and rapid spread of clade 1b in DRC, which appears to be spreading mainly through sexual networks, and its detection in countries neighbouring DRC is especially concerning," he said, citing Burundi, Kenya, Rwanda and Uganda.

He said many members feared the known situation in Africa was "actually the tip of the iceberg", because without surveillance, "we don't have the full picture".

Maria Van Kerkhove, the WHO's epidemic and pandemic preparedness and prevention director, insisted: "We can stop transmission of mpox with a concerted effort."

Two vaccines for mpox are recommended by WHO immunisation experts.

It’s harder to detect, with mutations not being picked up by existing tests. The variant is also believed to have caused dozens of deaths in Burundi, South Africa, and DR Congo.

Scientists now blame this variant for the outbreak in the regions that remained unaffected previously as enough preventive measures are not put in place.

"The most severe and lethal Clade 1 mpox, historically endemic to central Africa, has become predominant and is now found throughout the region," Zeil Rosenberg MD, executive vice president at Tonix Pharmaceuticals which is developing a mpox vaccine, was quoted as saying by Daily Mail.

"It's undoubtedly the most dangerous so far of all the known strains of mpox considering how it is transmitted, how it is spread and also the symptoms," said&nbsp;John Claude Udahemuka at the University of Rwanda during a separate briefing.&nbsp;

According to Leandre Murhula Masirika of the South Kivu health department, the new strain was first identified in the small mining town of Kamituga in South Kivu province in the east of the DRC in September.&nbsp;

The strain has spread to cities in the area in recent weeks and may already have reached neighbouring countries such as Rwanda, Burundi and Uganda.&nbsp;

Cris Kacita, the doctor in charge of operations in the country’s mpox control programme, told Reuters last week that roughly 8,600 mpox cases have been reported in Congo and 410 deaths so far this year.&nbsp;

The horror of Mpox has returned to scare the US as&nbsp;more than 570 cases of Mpox have been detected in the US so far in 2024 — almost double the number seen by this time last year.

JYNNEOS, the most popular Mpox vaccine in the United States, is administered in two doses spaced a month apart. Two weeks following the second dosage, a person is fully immunised, and booster shots are not currently advised.&nbsp;

One can check the eligibility for the vaccine and where to get the shots on the Centers for Disease Control and Prevention (CDC) website.&nbsp;

Various local health departments, such as those of New York City and San Francisco, also have their own vaccine site finders.

Dr Jenni McQuiston, deputy director of the CDC's High-Consequence Pathogens and Pathology division, told ABC News, "Most of the cases that are being reported are either unvaccinated or under-vaccinated, meaning they either never received a vaccine, or they only got one dose."&nbsp;

It may be noted here, Mpox formerly known as monkeypox, is caused by a relative of the now-eradicated variola virus, which causes smallpox.

The Mpox virus'&nbsp;two broad types are,&nbsp;clade I and clade II. Clade I is more likely than clade II to cause fatal disease. However,&nbsp;clade I has not been detected in the US&nbsp;to&nbsp;date, but it is possible that travellers could carry the virus to the country from other places, such as the Democratic Republic of the Congo, the CDC cautioned in December 2023. Generally speaking, Mpox viruses can produce flu-like symptoms such as fever, sore muscles, exhaustion, and enlarged lymph nodes. However, not every case had these symptoms in 2022 when the outbreak peaked. The characteristic pox rashes of the illness usually begin as discoloured patches that scab over and peel off, followed by raised bumps, blisters, and big pus-filled pimples.&nbsp;

Depending on the situation, these rashes may start in the mouth and on the cheeks before moving to the other areas of the body, or they may start in the genitalia and anus.

People with severely weakened immune systems, including those with HIV; children younger than 1 year; people with a history of eczema; and people who are pregnant face a higher risk of severe disease from Mpox. More than 30,000 cases of Mpox have been reported in the US since the outbreak began in May 2022; the majority of those cases occurred in 2022. Infection rates have decreased by the beginning of 2023; between January 1 and March 23, 2023, 298 cases were reported in the US.&nbsp;

This year, there were over 110 cases in New York City, 64 in California, and 50 in Florida.

The number of cases reported weekly on a global level increased by 328 per cent in the week to September 10, as per the data. 
 
Most of the increase came from China, where in the month 500 new cases were reported. The WHO stated that China was experiencing “sustained community transmission” of the virus, which was detected for the first time as an imported case in September last year.

Previously, Mpox was known as Monkeypox but the WHO renamed it in 2022 so as to tackle the stigma and discrimination around it.

The Chinese Center for Disease Control and Prevention in July reported that the disease was spreading domestically for the first time, with 106 cases recorded in June. 
 
Since then, there has been a surge of 400 per cent in the cases with the real number of infections believed to be much higher compared to the official count. 
 
In August, five women tested positive for the disease which led to concerns that the virus, which has predominantly been detected in men who have sex with men, has been spreading more widely.

<h2>Mpox under same&nbsp;protocols as COVID-19</h2>

The government of China on Wednesday started treating mpox under the same protocols as COVID-19, which meant that emergency measures like restricting gatherings to curb the spread of the disease can be taken by the authorities. 
 
Since 2022, the majority of mpox cases were found in men who indulged in sexual intercourse with men and 92.5 per cent of cases in August were among that demographic.

China is currently witnessing world's fastest increase in cases of mpox (previously known as monkeypox), a recent report by World Health Organization (WHO) has said. The agency has recommended that the country acts quickly to bring the spread under control.

Asia has become as the new hotspot of the disease while countries in the Americas and Europe have mostly contained the mpox outbreak. The outbreak started in mid-2022.

Last year, countries like Japan, South Korea and Thailand witnessed sporadic cases. This year, these countries have reported weekly double-digit number of cases.

However, the latest WHO data says that China has overtaken all other countries in the world. In last three months, 315 confirmed mpox cases have been found. However, irregularity in reporting from the government makes it hard to gauge the true scale of the disease.

Countries have shown varying success in containing mpox, which is less contagious than Covid-19 but has affected thousands across the world. The success in controlling mpox outbreak is often attributed to proactive measures like vaccination. A report in South China Morning Post (SCMP) has suggested that action taken by Chinese government leaves a lot to be desired.

“Compared with the response to Covid-19 […] the [Chinese] response is certainly dramatically different,” says Yanzhong Huang, who is a senior fellow for global health at the Council on Foreign Relations in the United States. Yanzhong was quoted by MIT Technology Review.

“Even though [mpox] is less likely to develop into a large outbreak in the country, the Pollyanna attitude may encourage the spread of the disease among the at-risk population – unless they take a more active campaign against the disease.”

“Overall, compared to where we were last year, we’re definitely in a different place,” says Krutika Kuppalli, who is an infectious-disease doctor quoted by MIT Technology Review.

“We have much fewer cases, but we are seeing sporadic outbreaks in different parts of the world.”

Chinese government has been criticised for being opaque about reporting true extent of the problem in the country. The authorities do not post number of infections on a regular basis.

“We also need to understand more about the people that have been infected,” Kuppalli says, “such as […] the demographics, the clinical presentation, their immune status and about how they’ve been presenting to care. I think that type of information is important.”
