Muzan was the first-ever demon. In the Heian Period, there was a doctor who cared for the human Muzan, who was constantly unwell. The doctor tried the Blue Spider Lily medication, which caused Muzan’s body to transform into a demon.

Muzan has complete power over the demons he creates. However, there are two exceptions. The first is, of course, Nezuko, and Tamayo, who used to serve as Muzan’s underling.
